Primary melanoma of the middle ear.

We present a case of a 52-year-old white female with what we believe is the first documented example of a primary melanoma of the middle ear. Multiple myringotomies and two tube insertions over a 14-month period were carried out until frank hemorrhage was encountered on the last one. Middle ear exploration found the lesion to be a primary melanoma. The supposed course of her diseased ear is discussed as regards the lag between effusion and frank blood when tumor was finally hit on the last myringotomy.

In-vivo measurement of glucose and alanine metabolism with stable isotopic tracers.

This report describes sensitive, precise microtechniques that allow quantification of glucose and alanine metabolism in vivo using stable isotope tracers. By combined gas chromatography--mass spectrometry and selected ion monitoring--deuterium and carbon-13 enrichment in blood glucose and alanine were measured with an error of less than 2 per cent. Estimation of glucose and alanine flux in dogs by stable isotope tracer techniques was compared with simultaneous measurements made in the same animal with conventional radiotracer dilution methods. Application of the described stable isotope methods to determination of glucose and alanine turnover as well as alanine-2,3-13C incorporation into glucose in adult men confirmed the safety and validity of these techniques for human investigation.

Cardiovascular responses in man to a stream of cold air.

Cardiovascular responses to a stream of cold air (0-10degrees C) directed towards the face and abdomen separately were examined in 12 human subjects. Cold air directed at the side of the face produced a 36+/-6 degrees increase in forearm vascular resistance and 22"/-4 degrees reduction in blood flow. Cold air on the abdomen did not produce a significant change in vascular resistance. The results are discussed in relation to angina pectoris evoked by cold wind.

Metastatic disease of the temporal bone.

This paper discusses metastatic disease of the temporal bone, and illustrates its discussion with a case history. The incidence, signs, symptoms, and treatment generally recorded are discussed. The histopathology of such lesions, as well as a documentation of those organ systems most likely to provide temporal bone metastatic spread, is noted. As the primary neoplasias of the temporal bone, metastatic disease may coexist with or mimic chronic otitis media. It is the wary surgeon who carefully analyzed his patient, the history, the intensity of recognizing neoplasia with or without associated chronic otitis media.

Parapharyngeal schwannomas.

Schwannomas of the parapharyngeal space are rare benign tumors arising from nerves located in the parapharyngeal space. Most of them occur as a mass behind the ascending ramus of the mandible, and bimanual palpation is important in ascertaining their exact location. The anatomy of the parapharyngeal space is described in detail. Treatment of these tumors is surgical through an external neck approach. A combined neurosurgical approach is indicated for tumors extending intracranially. Four cases of parapharyngeal schwannomas are added to the world literature (114 reported), with one extending intracranially through the jugular foramen.
