Prevention of cerebral infarction in the monkey by omental transposition to the brain.

The intact omentum of 13 monkeys was lengthened, placed subcutaneously, and laid on the left cerebral hemisphere prior to occluding the left middle cerebral artery. Two of these 13 monkeys developed left cerebral infarct and a right hemiparesis. Nine other monkeys had their left middle cerebral artery occluded without omental protection. All of these 9 developed a left cerebral infarct and 8 of them a right hemiparesis. Intact omentum may prevent a cerebral infarction when placed on the brain prior to MCA occlusion.

Renal cell carcinoma in childhood: report of one case.

Renal cell carcinoma is extremely rare in children. A case is reported here of renal cell carcinoma in a seven-year-old boy whose clinical manifestations were fever and right loin pain. Imaging studies revealed a solid mass in the right kidney and a metastatic nodule over the middle lobe of the right lung. Right nephrectomy and middle lobectomy of the right lung were performed. Pathologic examination revealed a renal cell carcinoma with predominance of clear cells. Despite Stage IV, the patient is still alive without chemotherapy and radiotherapy. It should be emphasized that renal cell carcinoma should be included in the differential diagnosis of pediatric renal solid masses, particularly in older children and adolescents.

Multiple types of cytokines constitutively produced by an established murine thymic epithelial cell line.

An untransformed murine thymic epithelial cell line (MTEC1) has been established. Without exogenous stimulation, the MTEC1 cells constitutively produced multiple types of cytokines, including IL-1, IL-6, IL-7, GM-CSF and chemotactic factor(s). Of which, IL-6, GM-CSF and chemotactic factor(s) were abundant; IL-1, moderate; and IL-7 at low level. MTEC1 cells neither produced detectable IL-3 nor TNF alpha. Thus, the MTEC1 cells may be useful not only in the evaluation of the signals required for thymic selection in vitro, but also useful in the analysis of the endogenous regulation of the autocrine cytokine production cascade.

Ureteropelvic fungal bezoar causing oliguric renal failure in a premature infant.

Systemic candidiasis with renal involvement is a relatively rare infectious process in young infants. The mortality rate in the cases reported to date has been lower after aggressive surgical and medical management. This report concerns a three-month-old, prematurely born infant with a fatal systemic candidiasis and acute renal failure. Renal pelvic fungal balls caused ureteropelvic junction obstruction in this patient. Renal ultrasonography demonstrated bilateral obstructive uropathy. Treatment consisted of surgical removal of the bezoar, together with open placement of nephrostomy tubes, parenteral amphotericin B therapy and peritoneal dialysis. This case emphasizes the need for a highly suspicion of renal obstruction by fungal balls in any high risk infant with systemic candidiasis and renal dysfunction.

Spontaneous rupture of the bladder in childhood: report of one case.

Perforation of the urinary bladder without history of antecedent trauma is a rare clinical occurrence in children, but should be considered in patients with voiding disorders, abdominal symptoms and unexplained metabolic disturbances. The present report concerns a young infant who presented with irritability, abdominal colic, decreased urine output; blood examinations showing mild leukocytosis, azotemia and metabolic acidosis. Further cystogram and cystoscopy confirmed rupture of the bladder. She had an uneventful recovery after a twelve days of conservative treatment with indwelling catheterization and nutritional support. A subsequent urogram revealed complete healing of the lesion.
