[Electromagnetic articulography (EMA) studies on orofacial movement functions].

For judging the influence of orofacial motor functions on the development and function of the dentoalveolar system objective methods are required. Electromagnetic Articulography (EMA) is a new device based on an inductive measuring principle. Using miniature receiver coils placed on the tongue dorsum or the velar margin, movements of multiple points on orofacial structures in the midsagittal plane can be recorded. The application of EMA on the examination of tongue and velum motor activity during speech production and deglutition is demonstrated. First results of sagittal tongue movement during swallowing in normals are reported. Examples from current studies, using EMA in the field of tongue dysfunction, orofacial motor coordination and velopharyngeal closure are demonstrated and the possible perspective of EMA-application is discussed for dental research and functional treatment of CLP-patient.

Findings of mandibular movement and the position of the mandibular condyles during maximal mouth opening.

An investigation with respect to position of the mandibular condyles in relation to maximal vertical mouth opening was undertaken. For this purpose, 51 subjects of different nationalities were examined. None of the 51 persons had a TMJ disorder. The mobility of the mandible in different directions was clinically measured. With the use of two different reference lines (Methods A and B), the amount of the movement of the condyle from closed to maximal mouth opening position was measured in the sagittal plane on lateral tomograms. All of the measurements were recorded, and the means, SD, and range of variation were calculated. Statistical analysis was subsequently performed. The clinical results are comparable to those of other previous clinical studies. As seen in the lateral tomograms, the condyle in 41 out of 51 subjects moved beyond the articular eminence during maximal mouth opening. It was occasionally situated higher than the eminence. In only ten subjects, the condyle reached only the top of the articular eminence during maximal mouth opening. A moderate degree of dependency and correlation was found between maximal vertical movement of the mandible and the amount of movement of the right and left condyles from closed to maximal open position of the mouth, as seen in tomograms. None of our subjects had any sign of luxation despite the position of the condyle beyond the articular eminence with maximal mouth opening. Therefore, the diagnosis of condylar luxation cannot be established by radiologic investigation alone.

[Balance of a therapeutic dwelling community -self-aid or care? (author's transl].

A therapeutic dwelling community (TDC) which has been in existence for more than 3 years, is described in respect to its history of origin and development outside existing psychiatric institutions or facilities. After having been confronted with the unsatisfactory follow-up care of a group of relatively young, in most cases only medium-term hospitalized schizophrenic patients, the initiators of TDC developed a concept which was destined to strike a balance between care and activation of self-aid within the TDC in order to promote the mental stability and autonomy of the former patients. A description is given of the tension arising from the concept, between the claims on the part of the residents for care to be given to them on the one hand, and the expectations on the part of the guiding personnel with regard to the self-aid potential of the inhabitants, the description being given for different levels. The structure, dynamics and results of group activities within the TDC are also described. In this connection, an attempt is made to show up the individual and group-referred limits of self-regulation of the inhabitants and of the therapeutic interventions; overstepping these limits endangers individual management of the TDC. An attempt is made to derive conclusions on the basis of the collected experience, which may be of importance for similar projects in respect of structure, financial jurisdiction, admission criteria etc.

[The treatment of premature labor with partusisten. Observations by 35 patients].

The authors describe the results of treatment of premature onset of uterine contractions with Partusisten in 35 patients. Therapeutical tocolysis was performed in 32 cases, prophylactic treatment after operations on the pregnant uterus in 2 cases, spray-tocolysis in uterine tetanus in third stage of labour in 1 case. The pregnancies could be prolonged for 7,1 weeks. No effect of the treatment could be achieved in case of contractions because of premature rupture of membranes and in chorionamnionitis.

[The value of double contrast arthrotomography combined with cinematography in the diagnosis of functional and structural TMJ alterations].

Double contrast arthrotomography combined with cinematography as a diagnostic instrument establishing functional and structural TMJ alterations is evaluated for its diagnostic value and reliability within the chain of diagnostic measures applied. In 131 patients double-contrast arthrotomography was followed by a comprehensive history of joint problems, and verification of the clinical findings as well as the arthrographic diagnosis and the post-arthrographic TMJ alterations. Our interest was focussed, among others, on the question whether arthrography alone would have any therapeutic effect or produce an alteration in TMJ function.

[Linear tomography and the Zonarc TMJ program. Reproducibility of condylar position of TMJ after surgical correction of dysgnathia].

In this clinical study the position of the mandibular condyles of 40 patients treated for malocclusion by surgical splitting of the mandibular ramus alone or combined with Le Fort I osteotomies was studied on lateral linear TMJ tomograms. Linear tomograms of the right and left TMJ with the mandible closed were taken from each patient prior to surgery and 6 months postoperatively. The postoperative changes in condylar position wee assessed in terms of direction and amount. Condylar displacements were found in 89% of the examined joints, most of them were less than 2mm. All types and combinations of displacement occurred, however, general trends in direction depending on the type of treatment could not be established with statistical significance. The type of surgical correction, its direction and amount did not seem to have an effect on the direction and amount of condylar displacement resulting. A comparison of the measured values with data in the literature showed that the amount of condylar displacement was markedly less--this seemed to be attributable to our precise evaluation method and a more sophisticated surgical technique.

[Results of arthroscopic surgery of the TMJ].

The article describes results of arthroscopic TMJ surgery in patients with limited jaw opening. In 21 patients, 16 women and 5 men, one or both joints were diagnosed arthroscopically and surgery was performed during the same session using special microsurgical instruments. The most important part of the procedure was loosening adherences under endoscopic view. Jaw opening increased by up to 20 mm in 19 out of the total of 21 patients. Complications such as are reported in the literature were not observed in any of our patients. Jaw opening decreased only in 2 patients due to lack of cooperation.

[Posterior dislocation of the articular disc--a rare case?].

The aim of this paper is to describe a very seldomly observed case of a unilateral dorsal luxation of the disc of the temporomandibular joint. The luxation happened during intense laughing in a movie-theater and was first noticed two days later. The diagnosis of this unilaterally shifted and fixed mandible was clearly seen on the double-contrast arthrogram. The therapy of repositioning the joint was done under short-duration general anaesthesia followed by a 2 weeks immobilisation of the jaws.

[2 objective procedures for the assessment of motor functions after surgery of the tongue and velum].

Commonly, only radiographic procedures have been used up to now for the assessment of intraoral movements during swallowing, speech and voluntary motor activities. In this article two methods for assessing intraoral function without involving radiation exposure are described: palatography and electromagnetic articulography. Palatography is particularly useful as a screening procedure for the evaluation of certain lingual motor activities in patients who had undergone tumor surgery. Electromagnetic articulography (EMA) allows vector analyses of lingual and soft palate movements. Data of normal subjects, surgically treated cleft palate and tumor patients are demonstrated. Both methods are biologically safe and non-invasive and do not interfere to any significant degree with the functions to be analysed.

[Primary sulcoplasty with hydroxylapatite augmentation for extreme ridge resorption].

Hydroxylapatite is commonly used for augmentation of the atrophic alveolar ridge. A new surgical technique is described for HA-augmentation and simultaneous vestibuloplasty on the severely atrophic jaw. The open surgical technique includes a lingually based mucoperiostal flap, the implantation of Hydroxylapatite particles in a vicryl mesh and the use of a positioning splint. First results show a sufficient depth of the vestibular sulcus and only few complications (circumscripted mucosal erosions). No lateral or lingual dislocation of the implanted HA-particles and no permanent mental nerve anaesthesia were observed. The prosthodontic results show increased stability and retention of the dentures following the one stage surgery.
