A five-component conceptual model for drug usage review is discussed as it applies to hospitals. The components of the model are: (1) authority, (2) operational and demographic characteristics of the delivery system and the population served, respectively, (3) the existing profile of drug usage, (4) standards of appropriateness and review of drug usage, and (5) scheme of evaluation to measure the impact of review. A drug usage review program in a hospital should: (1) improve the level of patient care and often reduce the cost of care, (2) improve the management and use of hospital resources, (3) clarify the drug component of patient care and better integrate pharmaceutical services with other hospital services, and (4) improve the fact-finding capacity that can help identify and solve hospital problems.

The second case of multiple halo neurofibromatosis is reported. Halo neurofibromas are rare benign tumors that appear on the neck and trunk of adults. The clinical and histopathological differential diagnosis of the other neuroectodermally derived halo lesions, such as neural nevus, nevocellular nevus, spindle and epitheloid cell nevus, blue nevus, and malignant melanoma, also is considered. An autoimmune hypothesis is proposed to explain the depigmentation in the halo neurofibroma. The fact that the patient has café au lait spots that contain giant pigment granules probably indicates that he also has neurofibromatosis.

This study was designed to demonstrate that properties of a granulation with a given composition, prepared by the spheronizing technique, could be altered by slight changes in process variables alone to satisfy the requirements of the formulator. A complete factorial experimental design was found satisfactory for demonstrating the range of properties to be expected and for showing statistically significant main effects and any linear interactions between selected variables. Results using two levels of five variables showed that initial water content and spheronizer speed had significant main effects on all primary granulation properties studied. While only one formulation was studied, the data suggest that the factorial design can have utility in predicting the properties of granulations prepared at conditions within the limits imposed by the equipment or formulation.
