STUDY DESIGN: This was a prospective study. OBJECTIVES: To evaluate the results of education and early rehabilitation in the prevention of back pain chronicity in coal mine workers. SUMMARY OF BACKGROUND DATA: A new mine was established in central Queensland, Australia. Preventing chronicity is important in the treatment of back pain in the industrial setting, because back pain is often refractory to treatment. Back pain patients also constitute the majority of compensation claims. METHODS: A back pain program was instituted that comprised work force education, early injury reporting, first aid at the mine, and changing workplace psychosocial perceptions. Management employees were actively involved. The time off work, number of claims per hundred workers, and costs per claim were compared with another mine in the area. RESULTS: The median time to return to work was 10 days. In the study group the number of claims and costs per claim were significantly less (P < 0.01) as compared to the control group. CONCLUSIONS: This was an easy to institute, inexpensive back pain program, which succeeded in preventing back pain chronicity in the studied group of mine workers, with no worker being off work for more than 60 days.

Eight paediatric patients undergoing major surgery for correction of scoliosis who were treated postoperatively with hypotonic saline and 5% dextrose have been studied. Plasma sodium, renin and aldosterone, and urine volume, sodium and osmolality were measured. These patients had an impaired ability to excrete a sodium-free water load. In the first 60 h urine volume remained reduced, while in the first 36 h urine sodium remained concurrently high. If the first 36 h postoperation are considered, the sodium-free water given was quantitatively retained and the serum sodium at 36 h was significantly correlated with the amount of free water given (P less than 0.01). To minimize postoperative hyponatraemia and the associated shift of water into the brain causing cerebral oedema, it is recommended that no more than 50 ml/kg sodium-free water be given until urine sodium falls and volume increases.
A 59-year-old man with ischemic pancreatic disease, polyarthritis, and cutaneous nodules has shown histopathologic findings indicative of disseminated fat necrosis in a percutaneous biopsy specimen from the right knee. The histopathologic findings in the synovium included necrotic fat cells, distorted fat cells and adjacent lymphocytes, lipid laden histiocytes, and giant cells. In prior histopathologic studies of the joint involvement associated with this disorder, fat cell necrosis has been found only in the periarticular tissues, and the synovium has appeared normal or showed nonspecific inflammation. However, the present study shows that the synovial membrane may also be the site of fat necrosis and an associated inflammatory reaction; thus patients with this disorder may manifest arthritis in addition to periarthritis.
