Taxonomy of anaerobes: present state of the art.

Results of nucleic acid studies, which indicate relationships among strains and species more clearly than do usual phenotypic tests, have led to new bacteriologic nomenclature. Some major changes in Bacteroides include the recognition of three species (Bacteroides melaninogenicus, Bacteroides denticola, and Bacteroides loescheii) formerly grouped in B. melaninogenicus subspecies melaninogenicus; two species (Bacteroides intermedius with two closely related homology groups and Bacteroides corporis) formerly grouped together as B. melaninogenicus subspecies intermedius; and two species (Bacteroides buccae and Bacteroides oris) formerly thought to be human isolates of Bacteroides ruminicola subspecies brevis. Former subspecies of Bacteroides fragilis have been elevated to species rank; and Bacteroides uniformis and an unnamed group ("3452A"), recognized. A new genus, Capnocytophaga, with three species, is composed of strains formerly classified as Bacteroides ochraceus or Centers for Disease Control (CDC) group DF-1. Strains previously thought to be human isolates of "Vibrio succinogenes" and related organisms that derive energy by reduction of fumarate or nitrate with formate or hydrogen have been reclassified in Bacteroides (Bacteroides gracilis), Campylobacter (Campylobacter concisus), or in a new genus, Wolinella.

Physical factors determining the utility of radiation safety glasses.

Ten types of lenses were tested for their radiation protection properties in three experimental circumstances: (a) Their good geometry (scatter free) attenuation was determined as a function of HVL in a direct x-ray beam. (b) Their dose reducing properties were determined under simulated clinical fluoroscopic conditions, using both ion chambers and thermoluminescent dosimeters (TLD's) while the lenses, mounted in frames, were worn by a head phantom. The head was oriented in four different directions to simulate clinical conditions in a fluoroscopic room. (c) Their dose reducing properties were measured with TLD's while the glasses were worn by radiologists during clinical fluoroscopic procedures. Although several of the lenses could attenuate a direct x-ray beam several thousand times, none of the glasses offer more than a factor of 8 reduction in eye exposure when the glasses are worn during fluoroscopy. This is attributed to backscatter from the fluoroscopist's head and deficient lateral shielding. Some of the glasses may even be causing an increase in eye exposure when the TV monitor is located 90 degrees or more from the patient.

Compton scatter with polychromatic sources for lung densitometry.

A mobile lung densitometer using conventional x-ray tubes, NaI detectors, and principles of two-source, two-detector Compton scattered densitometry, is described. The device is capable of one to two per second density measurements from a 45-cm3 volume with a precision of 5%. The expected in vivo accuracy (2%-3%) is determined by using an anthropomorphic phantom with replaceable lung inserts. The unintentional detection of multiple-scattered x rays results in a small density-dependent error. This error is predictable and relatively insensitive to differences in surrounding absorbers such as the chest wall. With this device, dynamic in vivo densitometry of the lung in the clinical laboratory and intensive care unit will be possible.

Skeletal muscle lymphoma in a bullmastiff.

A 16-month-old, neutered male bullmastiff was presented for acute onset of massive swelling of the right hind limb. Primary skeletal muscle lymphoma was diagnosed based on cytopathology, surgical biopsy, and necropsy findings. Cutaneous metastases developed during the hospitalization, and additional metastases were found in the heart and thoracic wall. Primary skeletal muscle lymphoma is a rare form of lymphoma in dogs and should be considered as a differential diagnosis for acute, soft-tissue swelling of the limb.

Massive myoglobinuria precipitated by halothane and succinylcholine in a member of a family with elevation of serum creatine phosphokinase.

Massive myoglobinuria developed in a patient given halothane and IV succinylcholine, Marked elevations of serum CPK were found in the patient and several family members. Myopathic changes in electromyogram and lack of neuromuscular symptoms and physical findings prompted the diagnosis of familial nonprogessive muscular dystrophy. Other hereditary muscular diseases were eliminated by medical workup. It is recommended that patients with known myopathy or unexplained elevations of serum CPK not receive the combination of halothane and succinylcholine.
