Correlation between total and EDTA/DTPA-extractable trace elements in soil and wheat.

Wheat and wheat products are more important sources of energy and nutrients in diets of people in many cultures compared to other foods. The daily consumption of wheat is about 200 g/d/person in Western Europe and North America. On the other hand, 400-450 g of wheat and wheat products are consumed daily by average Turkish people. Wheat samples collected from the Iskenderun region in 1995 and 1996 and Ankara and Istanbul regions in 1995 were analyzed for their trace element content by instrumental neutron activation analysis (INAA). In addition, 13 soil samples were collected from the Iskenderun region in 1996. Total soil samples were analyzed by INAA and atomic absorption spectrometry (AAS), EDTA-extractable elements by INAA, and DTPA-extractable elements by AAS. Correlation analysis and enrichment factor calculations were applied to the trace element results. In wheat samples, a strong correlation was found between the elements such as Sc, La, Sm, Rb, and K whose main source is soil. The concentration of Se appeared to show larger variations among different regions. No significant correlation was observed for elements such as As and Se whose main sources in the atmosphere are anthropogenic activities.

Nonweightbearing exercise during pregnancy on land and during immersion: a comparative study.

Seven women at 25 weeks' gestation exercised on land and in the water at 70% maximum oxygen capacity (VO2max) on a bicycle ergometer. Women had significantly lower heart rates and lower systolic blood pressures during immersion exercise. Women had a mean diuresis of 207 ml (6.5 ml/min) during water exercise compared with 98 ml. (2.4 ml/min) during land exercise. Fetal heart rates showed a tendency toward being higher after land exercise compared with water exercise. Six of seven fetuses displayed tachycardia after land exercise compared to one of seven after water exercise. Exercise on land and during immersion at 70% VO2max was well tolerated. However, exercise during immersion offers several physiologic advantages during pregnancy.

The encased ureter: bullet and bodkin pattern, a reliable radiographic sign.

Among the causes of constricted ureter is benign or malignant encasement. The diagnosis of ureteric encasement is frequently overlooked, even by well informed clinicians and radiologists. The most common benign cause is retroperitoneal fibrosis and the most frequent malignant causes are extension from an adjacent primary tumour, true metastases to the ureter and lymphoma. Lack of recognition of the process may lead to mistaken diagnosis of an inflammatory stricture or infiltrating transitional cell tumour, with resultant inappropriate management. A sign which appears to be almost specific for encasement, the bullet and bodkin configuration, is described here. Newer imaging modalities such as computed tomography and ultrasound, while helpful, are probably less sensitive and less specific than the retrograde ureterogram because a detectable mass is not always present.

Successful use of transureteroureterostomy to salvage ureterosigmoidostomy after anastomotic failure.

Four cases are presented of patients who had undergone ureterosigmoidostomy and in whom a unilateral ureteral anastomotic complication developed that was salvaged subsequently by a transureteroureterostomy. Although other forms of urinary diversion are now being utilized routinely in many centers, the ureterosigmoidostomy is still being employed worldwide, and urologic surgeons need to be versed in its potential problems and how to correct them.

Unique urinary-fecal calculus after cystectomy and ureterosigmoidostomy.

A 36-year-old m an had stage B1 bladder cancer-treated by cystectomy and ureterosigmoidostomy. Postoperatively, a urinary leak was managed successfully by transureteroureterostomy and temporary colostomy. In succeeding years a large calculus developed in the region where the bladder had been and it also involved the sigmoid colon. The huge stone was removed successfully. A nonabsorbable suture was found in the center of the stone. This is a unique complication of ureterosigmoidostomy.

The incidence and significance of seminal vesicle invasion in patients with adenocarcinoma of the prostate.

The incidence and significance of seminal vesicle invasion in patients with adenocarcinoma of the prostate were determined in 139 patients who underwent radical prostatectomy. Of the 36 patients who had seminal vesicle invasion, 27 (75%) also had capsular invasion and 13 (43%) had lymph node metastases. Disease progression rates were 31% and 58% at 5 and 8 years, respectively. Survival rate at 5 years was 90.6% and at 8 years it was 83.7%. Of the 103 patients without seminal vesicle invasion, 21 (20.4%) also had capsular invasion and 11 (12%) had lymph node metastases. Disease progression rates were 2.5% and 15.7% at 5 and 8 years, respectively. Survival rate at both 5 and 8 years was 98.4%. These data suggest that the majority of patients with seminal invasion do not have lymph node metastases on presentation and thus they cannot be detected by lymph node dissection before radical prostatectomy. Improved techniques for preoperative detection of seminal vesicle invasion can assist in patient selection for surgery and improve the results of radical prostatectomy.

Squamous metaplasia of the verumontanum with obstruction due to hypertrophy: long-term effects of estrogen on the prostate in an aging male-to-female transsexual.

We report on a 30-year-old male-to-female transsexual who had been treated with diethylstilbestrol and Premarin from 1954 to the present. A sex change operation was done. In May 1979 transurethral resection was performed for obstruction due to the enlargement of the verumontanum. Transurethral resection was repeated in January 1983 for obstructive symptoms. The interesting effect of the estrogens on the verumontanum and prostate is described and illustrated. The squamous metaplasia is compared to that seen in the prostate of a true hermaphrodite.

Vesicovaginal and ureterovaginal fistulas: a summary of 25 years of experience.

The difficult problem of a vesicovaginal fistula originally was cured surgically by Sims in 1849. During the last 25 years at UCLA and affiliated hospitals 68 patients have been treated by urologic surgeons for fistulas between the vagina and the urinary tract: 21 ureterovaginal and 47 vesicovaginal and urethrovaginal fistulas. The ureterovaginal fistulas often were complex and patients presented the most challenging diagnostic problem. However, they usually were repaired successfully by simple ureteroneocystostomy. Vesicovaginal and urethrovaginal fistulas were repaired transvaginally in 24 cases, with 70 per cent success at the first attempt and 92 per cent success with 2 attempts. Transabdominal or combined approaches were less successful. Only 58 per cent of the cases were closed at first attempt. The transvaginal approach required less operating time, and resulted in less blood loss and shorter hospital stays than the transabdominal approach and will be described in detail.
