[Mucocutaneous lymph node syndrome (Kawasaki-syndrome) (author's transl)].

Four cases of acute mucocutaneous lymph node syndrome (Kawasaki-Syndrome) are presented This syndrome of yet unknown cause is quite frequent in Japan but lately it is being encountered with growing frequency in Europe, too. The significant clinical and diagnostical features of the disease are laid out and differentialdiagnostical and therapeutical questions are discussed. 1-2% of the children die suddenly or coronary thrombosis. Analogies with infantile polyarteritis nodosa which can be equally lethal and Kawasaki-Syndrome are examined.

[Urinary levels of lysozyme in children with acute chronic recurrent urinary tract infection (author's transl)].

Urinary levels of lysozyme activity were measured in 32 children afflicted with acute or chronic urinary tract infection and compared to a group of 30 healthy subjects. A significant difference in the levels of lysozyme activity between the two groups could be observed. In the subjects with acute urinary tract infection the lysozyme levels in the urine were additionally determined at the beginning of the therapy, on the third day, the tenth day and three days after cessation of therapy. High lysozyme levels encountered at the onset of the infection showed under therapy a clear tendency to decrease and in all cases no lysozyme was present three days after therapy was completed. The possible causes of the pathological lysozymuria are discussed. The determination of lysozyme is thus an additional method by which to control the course and outcome of urinary tract infections.

Syndromes of compression of the median nerve in the proximal forearm (pronator teres syndrome; anterior interosseous nerve syndrome).

Entrapment of the median nerve in the proximal forearm is seen in two forms: the pronator teres syndrome, and the anterior interosseous nerve (or Kiloh-Nevin) syndrome. Both syndromes are rare, and they comprised approximately 1% of the compression syndromes of the upper limb which were treated operatively by the authors. The symptoms, signs, etiologies, and intraoperative findings are discussed. It is pointed out that certain of the clinical features may resemble those of irritation of the median nerve by a supracondylar process or Struthers' ligament. Although both proximal median entrapment syndromes have a favorable prognosis when treated non-operatively, the authors recommend operative treatment in cases in which there is no perceptible improvement following 8 weeks of non-operative treatment, since this is likely to speed and enhance recovery. Nine cases of the pronator teres syndrome (8 treated successfully by operation, 1 failure) and 2 cases of the anterior interosseous nerve syndrome (both fully recovered) are added to the cases reported previously in the literature.

Influence of flunitrazepam on body functions.

The effects of intravenous flunitrazepam (Rohypnol, Roche) on various body functions were studied and the following conclusions were reached: it is a very potent sedative/hypnotic even in small doses, it may cause depression of spontaneous breathing, it does not give rise to raised cerebrospinal fluid or intra-ocular pressure, it does not adversely affect renal function, and it can be used quite safely in combination with fentanyl or pentazocine in anaesthetic practice.

The influence of different ventilatory patterns on oxygenation and gas exchange after near-drowning.

After positive end-expiratory pressure (PEEP) valves for manually operated resuscitators became available it was of interest to find out whether PEEP should be applied at the scene of accident in nearly drowned patients. In 27 young pigs 25 ml of fresh water/kg body weight was instilled into the lungs via an endotracheal tube. After a period of apnoea (4 min) the pigs were ventilated artificially with 100% oxygen with either 5 cm PEEP or zero end-expiratory pressure (ZEEP). Within the PEEP group the AaDO2 returned to normal within the first 10 min after resuscitation, whereas the AaDO2 did not reach normal values for 2 h in the ZEEP group. The same observations were true for the PO2 values. Although the cardiac output decreased by almost 30% in the PEEP group, mean arterial pressures and heart rates remained unchanged. Lung weights and blood lactate concentrations confirmed that the same degree of water absorption and hypoxia had taken place in both groups. The conclusions from these experiments are that the application of 5 cm PEEP is part of the immediate measures of emergency treatment in nearly drowned victims.

[Comparative studies on perioperative infusion therapy in children].

Various infusion solutions--containing different amounts of sodium, potassium, chloride and carbohydrates--were compared within two groups of infants and children from six months to fourteen years of age. Among various parameters particularly blood electrolytes and urinary electrolytes were taken as important parameters to recommend certain types of the basic infusion solutions, to be used for the perioperative period in pediatric surgical cases.

[A new suction pump to be used in the field of emergency medicine (author's transl)].

In the field of emergency medicine there has been a need for an efficient suction pump that can be used under all types of conditions and included both in doctors' emergency kits as well as in the standard equipment of mobile units such as ambulance cars, helicopters etc. The devices available in the past had decided disadvantages in their construction and performance. A newly designed suction pump -- the AMBU Uni-Suction -- has been subjected to laboratory and practical tests concerning function, material and practical use, based on a specification of requirements for such a device. The results show that this universally applicable suction pump meets every requirement under the different test conditions. The effectiveness is good, irrespective of the mode of operation applied, i.e. by hand, by foot or by compressed gases. Maintenance and service are negligible and no susceptibility to malfunction was observed. In addition, the design, function and performance of an accessory unit, the Suction Booster, are presented. This device can be used effectively in combination with the new suction pump, or with any other type of suction unit to prevent dangerous aspiration in emergencies.

[Comparative studies during continuous epidural analgesia in obstetric (author's transl)].

From 1975 on, continuous epidural analgesia has been administered at the University Department of Anaesthetics in Ulm to women for pain relief during labour. One group of 350 pregnant women received bupivacaine hydrochloride, from 1. 5. 1976 to 30. 4. 1977, another group of 350 women received bupivacaine carbonate during the period from 1. 5. 1977 to 30. 11. 1977. Within these two groups the different effects of both local anaesthetic agents were compared in respect of the course of labour, the indications for epidural analgesia, the practicability of epidural analgesia, complications during labour, the rate of pain relief, the frequency of operative deliveries and the clinical condition of the newborn. The main advantages of bupivacaine carbonate for obstetric analgesia purposes are demonstrated.

[Fanconi's anemia (author's transl)].

A 6.6 years old girl is described who shows the typical symptom complex of Fanconi's anemia. Pathogenesis, clinical findings, cytogenetic results, therapy and prognosis of Fanconi's anemia are discussed.
