[Large droplet lipids in blood in the right ventricle].

In several cases of fatal blunt-trauma injuries or after exposure to severe burning, large-drop fatty substances, discernible to the naked eye, were found in the venous blood of the right ventricle. Thin-layer chromatographic separation of the extracted lipids demonstrated an increase in the triglyceride fraction. Analysis of the fatty acids by means of combined gas chromatography/mass spectrometry showed a shift towards oleic acid. The distribution pattern of the molecular weights was determined by means of DCI-mass spectrometry in order to characterize the triglycerides in detail. The total chemical findings indicated that the visible lipids present in the right ventricle had predominantly derived from the adipose tissues. In cases of post mortem burning (without vital traumata) no typical pulmonary fat embolism had developed despite massive lipemia in the venous blood.

[Delayed epipleural hematoma as a fatal complication following blunt chest injury].

Delayed epipleural haematoma was found on autopsy of a 92-year-old woman after a fall at her home. X-ray performed 3 hours after the blunt thoracic trauma showed fractures of 5 ribs on the right side without any indication of intrathoracic bleeding or masses. Dressing and moving the patient about one day later caused multiple injuries of intercostal arteries and subsequently led to epipleural haematoma and unexpected death. Origin and morphology of epipleural haematoma are discussed with reference to a pertinent report in the literature. Hospitalization, with utmost care in nursing attention, is recommended in the treatment of patients presenting with thoracic trauma.

[Studies on the possibility of immunoprophylaxis of carcinoma].

The extremely high mortality rate of cancer requires every effort to avoid or decrease the incidence of malignant diseases. In this paper the possibility of an immuno-prophylaxis of carcinoma is discussed. A study was undertaken, whether or not the incidence of carcinoma is decreased in patients after the change of the status of immunity developed by typhus. In the years after the war (1945-1947) over 5400 patients suffering from typhus had been reported in Vienna (according to the law in Austria) and about 600 died from this disease. In 1967 more than 2800 could be determined still alive and 535 had died in the meantime. The mortality rate of cancer was calculated and compared in age- und sex-adjusted groups with the relative cancer mortality rate of the whole population of Vienna in the average of the years 1959-1963. The statistical evaluation shows a lower cancer mortality of patients suffering from typhus at an earlier time. This difference seems to be significant (chi 2test, p less than 0.05) in women of the age group 30-50 and in men 35-65 years of age. Additionally in two regions of Austria (Oberösterreich, Burgenland) similar observations were made, but with no statistical significance because of the small numbers. Some diagnostic insecurities during war time require the restriction of conclusions. On the basis of clinical observations and results from animal experiments the possibility of an immuno-prophylaxis of carcinoma by unspecific stimulation of the defensive mechanism is supposed, and a prospective study of volunteers, whose level of defense is kept optimal by adequate frequent repetition of the stimulation, is suggested. The incidence of carcinoma of volunteers would have to be compared to a control group.

[Intraoperative fatal fat and bone marrow embolism of the lung in implantation of a hip endoprosthesis with polymethylmethacrylate bone cement].

We report on an intraoperative lethal fat embolism and bone marrow embolism of the lung caused by implantation of hip endoprosthesis using acrylic bone cement. Autopsy revealed massive fat embolism of the lung and multiple bone marrow emboli within branches of the pulmonary arteries. It is concluded that there is a close relationship between implantation of hip endoprosthesis and death because fat embolism and bone marrow embolism was the only available evidence on post mortem examination.

[Pump-gun as a weapon. Type of injuries, prohibition of weapons].

Pumpguns are shotguns with pump action whose injuries and wound mechanisms have several special features: extremely high kinetic energy of the shot (2500 to 3500 J) frequent cases of "Krönlein shots" (exenteration of the brain) punchmark/imprint immediately adjacent to the entrance wound from the front of the pipe magazine exit wounds from buckshot may be similar to pellet entrance injuries from a distant shotgun discharge the use of various shotgun cartridges (plastic ammunition, slug bullet, various lead pellets) within the same weapon. The change in the Austrian gun law and the banning of the pumpgun in 1995 is also discussed in the article.

[Maltreatment by police officers?].

In January 1990 amnesty international stated a report concerning 128 cases of alleged torture by the police in Austria since december 1984. Our material consists of 31 cases, three case reports are given in detail. Problems of medical expertise are discussed. It may be difficult to rule out injuries which are caused by accident or self inflicted. Some methods of torture (certain kinds of asphyxia for example) may be impossible to trace.

[Aspects, findings and problems in criminal cadaver dismemberment].

Nine cases of defensive corpse dismemberment are reported. Twice offensive components were regarded. All victims were identified within a short time. Eight crimes were solved, in six cases there had been a relationship between the murderer and the victim. One corpse was dismembered by means of an explosive.
