Evolution of diabetes mellitus in a case of Friedreich's ataxia.

A 12 year old girl with Friedreich's ataxia, in whom progression from chemical to severe insulin-dependent diabetes mellitus occurred over a two year period, is presented. During the phase of subclinical diabetes her plasma immunoreactive insulin response to oral glucose was markedly increased, but when she became symptomatic gross insulin deficiency had supervened. This report draws attention to the unusual disorders of carbohydrate metabolism and insulin secretion that may occur in a variety of neurological conditions.

Rapid analysis of pregnanediol in pregnancy urine.

A simple, inexpensive, and rapid method for the determination of pregnanediol in pregnancy urine by gas liquid chromatography is described. Automatic injection of the samples into the gas chromatograph allows up to 36 samples to be run overnight thus saving valuable technical time.The method described can easily be adopted for use in a routine steroid laboratory. The results obtained by this method have been compared with values obtained by the method of Klopper, Michie, and Brown (1955).

Evaluating hypertension control in a managed care setting.

BACKGROUND: We conducted a retrospective cohort study on a random sample of adult patients with hypertension in a large health maintenance organization to assess the feasibility of documenting blood pressure (BP) control and to compare different measures for defining BP control. METHODS: Three criteria for BP control were assessed: systolic BP less than 140 mm Hg; diastolic BP less than 90 mm Hg; and combined BP control, with systolic BP less than 140 mm Hg and diastolic BP less than 90 mm Hg. Four methods of assessing hypertension control by the above criteria were examined: proportion of patients with BP under control at 75% and 50% or more of their office visits; the mean of all pressures during the study period; and the BP from the last visit during the study period. RESULTS: The proportion of patients meeting each criterion for control was similar whether we used the mean BP for all visits, the last recorded BP, or control at 50% or more of visits. Control rates were substantially lower when the more stringent assessment, 75% of visits, was used. The proportion of patients with combined BP control at 75% or more of their visits was half that of the other methods. CONCLUSIONS: In this health maintenance organization population, results with the use of the simplest approach, the last BP measurement recorded, were similar to results with the mean BP. Our findings indicate that evaluation of BP control in a large health maintenance organization will find substantial room for improvement, and clinicians should be encouraged to be more aggressive in their management of hypertension, especially with regard to the systolic BP, which until recent years has been underemphasized.

The relationship between changing values of pregnancy-associated plasma protein-A in late pregnancy and the onset of labour.

The pattern of change of PAPP-A was established in three separate groups of patients. Day-to-day change from the 38th week was measured in six healthy patients whose pregnancies ended in spontaneous labour. The slow fluctuations of PAPP-A suggest that values found in labour are a consequence of events prior to the onset of labour. Increases of PAPP-A during the last few days of pregnancy and into labour were compared in patients going into labour spontaneously and patients who were induced. The induced patients showed a sharper increase in PAPP-A during this interval than the spontaneous onset group. A comparison at an earlier stage of pregnancy of PAPP-A increase was made between normal pregnancies going into labour on or before 280 days and those who went into labour later. Between 31 +/- 1 and 35 +/- 1 weeks, those who delivered in the earlier group showed a sharper rise in PAPP-A. It seems likely that the behaviour of PAPP-A in late pregnancy is the consequence of uterine activity and it seems unlikely that an increasing level of PAPP-A in itself has anything to do with the initiation of spontaneous labour.
