[Adenoid cystic carcinomas of the head and neck (cylindromas)].

In the years 1958-1997 111 patients with adenoid cystic carcinoma in the head and neck of different localization were treated at the Department of Otolaryngology--Head and Neck Surgery. Majority of the tumors originated of the major salivary glands, mainly in the parotid gland (47 cases). The most common location among the tumor of the minor salivary glands was the palate (14 persons). The tumors arising from other mucous glands, most frequently has located in the maxillo-ethmoideal complex (20 cases). The authors stress diagnostic difficulties, the necessity of a proper selection of the surgical method and postoperative radiotherapy, describing the treatment results in the patients. Radial surgical procedure of the adenoid cyctic carcinoma yelds optimal local control and survival rates.

[Pleomorphic adenomas in the materials of the Department of Otolaryngology of the Academy of Medical Sciences in Poznań].

In the years 1958-1997 were surgically treated in the Department of Otolaryngology Karol Marcinkowski University of Medical Sciences in Poznań 365 patients with pleomorphic adenomas. Majority of the tumors originated of the major salivary glands was parotid gland (273 cases). The given materials confirms the viewpoint that enucleation in the pleomorphic adenomas of the parotid salivary gland--based on total removal of the tumor together with the capsule and pouch constitute a completely sure surgical procedure.

[Breast carcinoma metastasis to paranasal sinuses].

The presented case is breast cancer metastatic into the paranasal sinuses. Accompanied symptoms suggested thrombosis of cavernosus sinus with very fast growing eye and neurological symptoms caused by metastases to brain tissue. Breast cancer metastatic into the paranasal sinuses is extremely rare and usually ends with death, as it is a result of neoplastic dissemination and does not react to systemic treatment. This type of neoplasm was for the first time presented in literature by Robin in 1938. Until 1994 nine cases of patients suffering from this type of neoplasm have been described. All of them died.

[Swallowing disorders in patients after the laryngeal cancer surgery].

Anatomical and functional changes of the oral cavity, pharynx and larynx due to malignant tumor surgery and irradiation, cause breathing, swallowing, phonation and lung protection disorders. Deglutition disorders and choking are observed in all patients who have undergone supracricoid laryngectomy. We presented 33 patients after supracricoid laryngectomy with simultaneous CHEP (6) or CHP (27) reconstructions. 18 patients had one arytenoid cartilage removed, in 1 patient both of them were removed. Removal of the stomach tube took place between 29-36th day after the surgery. 4 patients showed symptoms of the aspiration pneumonia. Difficulties in swallowing liquids persist in some cases even a few years after the surgery. Re-education and rehabilitation of swallowing is the predominant problem and the most important goal after the CHP and CHEP surgery.

[The ultrasound examination of salivary gland].

The results obtained after the examination of 107 ultrasound cases of changed salivary glands were presented. The characteristic features for changes most often appearing in salivary glands were presented. In case of tumours and cysts the diagnosis was formed on the basis of the ultrasound character of the change (low homogenas, density in cysts, higher density, non homogenous character in tumours), the degree of separation from the surrounding tissue (clear capsule in cyst and benign tumours, lack of capsule, indistinctive image in change of malignant character, in case of infiltration of surrounding tissues). The diagnosis of inflammation was made depending on the character of the change of the parenchyma of salivary gland--the increase of the gland with the lowering of density characteristic for acute inflammation, the reduction of size, increased sonographic density, enlarged leading out (duct), the echo of concrement is characteristic together with the accompanying acoustic shade for bigger concrements.

[Malignant or necrotizing otitis externa].

The authors report one case of the malignant external otitis treated with Ciprobay in 72-year-old diabetic female. They are calling attention to the diagnostic difficulties in the initial period of the disease due to similar clinical manifestations in the chronic otitis. The control examination after 7 months found no recurrence of the Pseudomonas aeruginosa infection.

[Lymph nodes in cancer of the tongue].

The results of lymphnodes examinations in 59 patients operated upon because of the lingual cancer were described. In 39% of cases the metastases were found on the side of lesions. In 31 of patients with palpable lymphnodes the metastases were found in 54.8% of cases. In 28 patients without palpable lymphnodes the metastases were found in 21.4% of cases. Thus 45.2% cases were false positive and 21.4 false negative. In lingual marginal localizations of lesions the metastases were found more frequently as well as in more developed clinical stages. In these patients because of the frequent false positive lymphnodes the radical neck dissection should be performed, in special cases also bilaterally.

[Primary cancer of the tongue treated at the Otolaryngological Clinic, Medical Academy, in Poznań 1963-1987].

The analysis of 191 patients with primary tongue carcinoma treated in ENT Clinic of Medical Academy in Poznań during the years 1963-1987 was presented. Since 1983 there were the dramatic increase in patients number. The patients age was between 20 and 87. Statistically significant was the males number. The half of the patients were in their 6th decade of life. In the last 5 years period the patients were younger of mean 5 years. Their symptoms lasted about 4 months. The earliest was the tongue pain. Tobacco smokers and alcoholics predominated. The localizations were in general on the edge and on the basal part of the tongue. Histologically the most frequent was the planoepithelial cancer in the second clinical stage. The 5 years survival rate was about 20%.
