[The investigation of the immunomodulatory effect of san-hwang-sei-sin-tang].

San-Hwang-Sei-Sin-Tang is a famous ancient Kampo. In order to investigate the immunomodulatory effect of this Kampo, we can stimulate lymphocytes with PHA to study the lymphocyte transformation and IL-2 production as indicators of effect. In this study, we found that the extract of San-Hwang-Sei-Sin-Tang at the concentration of 0.1 mg/ml and 1 mg/ml could effectively inhibit the index of lymphocyte transformation. San-Hwang-Sei-Sin-Tang at the concentration of 0.01 mg/ml, 0.1 mg/ml and 1 mg/ml also have the tendency of inhibition of IL-2 production. In the future, San-Hwang-Sei-Sin-Tang may be developed as an effective immunosuppressant.

Characterization of a new human transitional cell carcinoma cell line from the renal pelvis, RTCC-1/KMC.

Tumors of the renal pelvis and ureter are relatively common malignancies in Taiwan. Studying the genetic or biochemical aberrations is a feasible pursuit that has great potential to further our understanding of urothelial cancer and may provide clinically valuable information. We now report a new long-term culture (RTCC-1/KMC) of human TCC derived from the renal pelvis, which is aimed to be used as a target for those studying in this field. The cultured cells exhibited anchorage independence and loss of contact inhibition. Chromosomal analysis revealed an aneuploidy line with a modal number of 50. Population doubling time was about 36 hours at the third passage. Expression of keratin proteins confirmed its epithelial origin. The genetic markers of the RTCC -1/KMC cell line were HLA-A11, B46, B60, Cw1, Cw7, DRw12 and DRw16. The human papillomavirus and herpes simplex virus genomes were not found in this cell line.

Chronic neutrophilic leukemia--a case report.

We describe a 77-year-old patient with chronic neutrophilic leukemia (CNL). Diagnosis of CNL was made by marked increase of mature neutrophils without increased immature granulocytes, splenomegaly and elevated serum vitamin B12 level and by ruling out any infection or occult malignancy. The chromosome analysis showed one abnormal karyotype out of 14 metaphases. The neutrophilic function with nitroblue tetrazolium test revealed impaired phagocytic function in this patient. The neutrophil count became normal gradually after hydroxyurea treatment. However, it still maintained a high percentage of mature neutrophils. Background literature and the prognosis of CNL are discussed.

Establishment and characterization of a continuous human basal cell carcinoma cell line from facial skin (I) cytological behavior of early passages.

Basal cell carcinomas (BCCs) are one of the most common skin malignant diseases of human beings in the course of a lifetime. BCC is a slow growing tumor of epithelial origin. Most of the BCC tumors have a normal diploid DNA configuration and have been successfully passaged for a short-term only. We now report a long-term culture (BCC-1/KMC) of human BCC derived from the undifferentiated type of facial BCC tumor on the thermal traumatic scar, which was aneuploidy and subculture for more than 100 passages. The population doubling time of BCC-1/KMC cells at the third passage was 38.86 hours. This immortalized and tumorigenic cell line expresses epithelial markers of keratin filaments and desmosomes. The genetic markers of this cell line have HLA-A2, A24, B27, B35, Cw3, DR2 and DR12. The BCC-1/KMC cells have successfully adapted to grow in a cheaper commercial medium (RPMI 1640) and at a moderate concentration of calcium (0.4 mM).

Rheumatoid factor in hepatitis B virus surface antigen positive patients.

Rheumatoid factor (RF) is often found in the sera of patients with nonrheumatic diseases in comparison with normal individuals. We collected 95 healthy HBs antigen positive adults and 98 healthy HBs antigen negative adults. RF was present in 14.74% (14/95) of the HBs antigen positive group while it was present in 3.06% (3/98) of the HBs antigen negative group (p < 0.05). RF was positive in 15.38% (2/13) of the HBe antigen positive patients whereas it was positive in 14.63% (12/82) of the HBe antigen negative patients (p > 0.05). RF was found positive in 16.90% (12/71) of the anti-HBe antibody positive group while it was positive in 8.33% (2/24) of the anti-HBe antibody negative group (p > 0.05). We conclude that the positive rate of RF in HBs antigen positive patients is significantly higher than in that of HBs antigen negative patients. The incidence of RF positivity of the HBe antigen positive group is not higher than that of the HBe antigen negative group. The positive rate of RF in anti-HBe antibody positive group has a tendency to be higher than in that of their negative counterparts though there was no significant difference between them.

[Application of a human immortalized fibroblast cell line in laboratory diagnosis of autoimmune diseases].

Many autoantibodies reacting with cellular and nuclear components have been described in sera of patients with autoimmune diseases. The most important diagnostic markers for those diseases are antinuclear antibodies (ANA). The first choice for laboratory diagnosis of autoimmune diseases is to use cultured monolayer cells as a nuclear substrate. Up to now the HEp-2 cell line derived from a human carcinoma of the larynx, appears to be the most sensitive and specific nuclear substrate. The cultured fibroblast monolayer cells have also been applied to detect the ANA, although the application was not recommended by one study. Thus to evaluate the applicability of our immortalized human fibroblast cell line (CCFS-1/KMC) as a nuclear substrate, commercial HEp-2 MBL monolayer cells was used as the standard substrate. The results of this report showed the applicability of the CCFS-1/KMC cell line as a nuclear substrate to detect the ANA of autoimmune diseases. The sensitivity of this fibroblast cell line was the same as both of the HEp-2 nuclear substrates (HEp-2 and HEp-2 MBL). The specificity of the CCFS-1/KMC cell line was similar to the HEp-2 substrate. Since the specificity of both of the above substrates were lower than the standard nuclear substrate HEp-2 MBL, therefore, if the specificity can be improved by changing the protocols of the substrate preparation, the CCFS-1/KMC cell line will be a good nuclear substrate for detecting the ANA of autoimmune diseases.

Dermatofibrosarcoma protuberans of the hand: a case report.

Dermatofibrosarcoma protuberans is a slowly growing, low-grade sarcoma originating in the dermis and subcutaneous tissue. It generally occurs with equal frequency on the trunk and proximal extremities. Only three cases have been reported to occur on the hand. A case of dermatofibrosarcoma protuberans of the right hand anterior to Guyon's canal is reported, and the literature is reviewed.

Alteration of amino acid in plasma and cerebrospinal fluid of children with seizure disorders.

Amino acid levels in plasma were measured by amino acid autoanalyser in 130 convulsive children. The levels of taurine, serine and tryptophan were significantly lower in convulsive children as compared to normal control; in contrast, isoleucine, homocystine, GABA, histidine, arginine and ammonia were higher. The children with paroxysmal disorders (headache, dizziness and abdominal epilepsy) had the highest levels of isoleucine, histidine and arginine and the lowest levels of glutamate and cystein. Clinical seizure activity within 6 months prior to the test seemed to have no obvious effect on the plasma amino acid pattern, except for the levels of glycine and arginine tended to return to normal, and the level of GABA was significantly increased in patients with the seizure being controlled. The patients treated with carbamazepin as a single anticonvulsant had the highest GABA level compared to those with other anticonvulsants. Hyperglycinemia and hyperammonaemia were also noted in patients who took valproic acid. The levels of serine, isoleucine and phenylalanine in the CSF within 6 hours after convulsion were significantly lower than the normal control; while asparagine, tyrosine, lysine and arginine were significantly higher. The concentration of ammonia in the CSF was also elevated after convulsion as compared to the normal control. Amino acids play an important role in the generation of epilepsy and recently there has been an increasing number of studies to help determine their effects during an epileptic attack. However, there still is much debate and controversy on this topic. Therefore, further studies are needed and researchers should carefully consider factors that might affect the accurate assessment of the results.

Characterization of an immortalized human cell line derived from neonatal foreskin diploid fibroblasts.

A new human skin cell line, designated as CCFS-1/KMC, immortalized from human neonatal foreskin diploid fibroblast cells, has been subcultured successfully in vitro for more than 500 passages. This anchorage-dependent cell line possesses many common features of transformation such as morphological and cytoskeletal changes, hypotriploidy, infinite lifespan, increasing plating efficiency and saturation density, and decreasing serum requirement and population doubling time. Human papillomavirus (HPV) type 18 DNA was detected in the cell line before and after immortalization by the polymerase chain reaction (PCR) method. Tumorigenicity, however, was not demonstrated in vivo. The isoenzyme activity of the cell line shows activation of a placental form of alkaline phosphatase and a changing lactate dehydrogenase isoenzyme pattern that is different from transformation by carcinogens. Class I HLA and class II HLA antigens are constitutively expressed on this skin cell line. Here we report that these immortalized human fibroblasts derived from neonatal HPV-18-DNA-contained diploid fibroblasts possess double minute chromosomes (DMs), a karyotypic aberration usually found in cancer cells.

Spontaneous production of growth factors for human lymphocytes from a human papillomavirus type 18-contained foreskin fibroblast cell line.

An immortalized fibroblast cell line, designated as CCFS-1/KMC, derived from human neonatal foreskin fibroblasts, contained human papillomavirus (HPV) type 18 DNA. Since this newly established cell line could spontaneously secrete activating factors for normal human blood lymphocytes, the synthesis and release of potent inflammatory cytokines from this cell line were checked. To determine the presence of cytokines in the supernatant collected from the cell line, tests by a cytokine-specific ELISA and a mitogenesis bioassay were done. The cell line could spontaneously produce several immunoreactive cytokines, such as tumor necrosis factor (TNF), interleukin 1 (IL-1) and interleukin 6 (IL-6). It also could stimulate the mitosis of human blood lymphocytes and Raji lymphoblast cell line. These cytokines were present in the same fraction of isoelectric points (pI) from 5.4 to 5.6. This study suggests that non-immune bystander cells may exert immunomodulatory effect on the immune cells via the production of potent inflammatory cytokines during viral infection.

Somatostatin, substance P, prolactin and vasoactive intestinal peptide levels in serum and cerebrospinal fluid of children with seizure disorders.

Seventy patients aged from one month to 18 years with seizure disorders were classified into three groups: I. Patients who had hard control seizure attacks even under medication; II. those who had occasional seizure attacks (less than 6 times per year) and III. those who had no seizure attacks after receiving medication for at least one year. Blood samples were taken for somatostatin, substance P, prolactin and vasoactive intestinal peptide (VIP) assays. Lumbar puncture was made in 32 children and CSF samples were also assayed for neuropeptides. Somatostatin levels in serum were significantly elevated in group I and group II (P = 0.05, ANOVA) but not in group III and control group. Similar observations were made in substance P, prolactin and VIP studies. In CSF, the somatostation can better indicate the difference between epileptic and normal children (comparison with group I, P greater than 0.001; with group II, P less than 0.001; even with those who were seizure free after medication, P less than 0.05). In conclusion, the levels of several neuropeptides (somatostatin, substance P. prolactin, VIP) were elevated in children with seizure disorders both in serum and CSF. The present investigation provides a new category for the understanding of the pathogenesis, treatment as well as prognosis of seizure disorders.

The medical complications of drug abuse.

There is a vast array of abused drugs, and only a few of the more commonly used substances have been discussed. Patients with drug abuse frequently present with atypical syndromes and diseases. These individuals usually have less social, medical and economic support, making them more susceptible to the diseases that are associated with poverty, such as tuberculosis, and the complications of diseases such as hypertension, congestive heart disease and diabetes that require long-term care. Our strategy in the evaluation of these patients should consider all these aspects of medicine. A meticulous assessment and comprehensive care are necessary to render quality care for these complicated human and toxicological problems.

Lymphocyte transformation in patients with HBsAg-positive chronic hepatitis, liver cirrhosis, and hepatocellular carcinoma.

To evaluate cellular immunity, mitogen-induced lymphocyte transformation was assessed in 146 patients with chronic hepatitis B virus (HBV) infection, including 63 patients with chronic hepatitis (CH), 34 with liver cirrhosis (LC), and 49 patients combined with liver cirrhosis and hepatocellular carcinoma (HCC). The PHA-induced transformation indices (TI) were lower in patients with chronic HBV infection, especially in the LC and the HCC groups. The Con A-induced TI were also diminished in patients with chronic HBV infection. The mitogen-induced TI in patients with CH showed no significant difference in regards to different histological patterns, HBeAg/anti-HBe statuses, sex groups, age distributions, and SGPT levels. However, the mitogen-induced TI were substantially higher in CH patients with elevated serum AFP levels, especially in the group with AFP levels over 100 ng/ml. From these data, we conclude that: (1) impaired PHA-induced TI in patients with LC may partly explain the high risk factor of LC in the occurrence of HCC; (2) the reduction in TI induced by optimal concentration of Con A reflects the defective suppressor cell activity in patients with chronic HBV infection, even in patients with HCC; (3) no significant difference among the control group and patients with chronic HBV infection in PWM-induced TI implies that the response of B cells may be normal in patients with chronic HBV infection; (4) elevated mitogen-induced TI in CH patients with high AFP levels indicate that the activation of lymphocyte may contribute to the severity of inflammation and tissue damage.
