Evaluation of the components of the chylomicron remnant removal mechanism by use of the isolated perfused mouse liver.

The isolated perfused mouse liver was utilized to evaluate the relative contribution of various molecules believed to participate in the removal of chylomicron remnants by the liver. Sixty percent of asialofetuin was removed from the perfusate per pass; bovine serum albumin was not removed. Normal mouse livers removed chylomicron remnants more efficiently (40-50%/pass) than nascent chylomicrons (10-20%/pass). The fractional removal rate of remnants decreased as their concentration in the perfusate increased demonstrating saturability. Remnant removal by livers of low density lipoprotein receptor-deficient (LDLRD) mice paralleled that of normal mice at low remnant concentrations (0.05, 0.2 microg protein/ml); as concentration increased (4-16 microg protein/ml), removal by LDLRD livers was reduced. About 50% of the capacity to remove remnants was due to the LDL receptor. The role of the LDLR-related protein (LRP) was estimated using the receptor-associated protein (RAP). Four microg/ml of RAP inhibited only LRP; it reduced the removal of remnants by 30-40% in normal livers. When RAP was included in the perfusate of LDLRD livers, remnant removal persisted but was diminished, particularly late in the perfusion; the capacity was approximately 30% of controls. The present study has established that there is more than one mechanism operating for the removal of chylomicron remnants by the liver, provides estimates of the concentration of each to the removal of remnants, and indicates a method for further studies. It is concluded that in normal livers, the LDL receptor has the greatest capacity for removing chylomicron remnants. The LRP contributes to the process as well and a third component, perhaps "sequestration," accounts for up to 30% of the capacity for the initial removal of chylomicron remnants.

Enhanced efficacy of the combination of all-trans-retinoic acid and gamma-interferon in inhibiting proliferation of Tca8113 cells.

OBJECTIVE: To develop a more effective induced-differentiation or -apoptosis therapy for treating oral squamous cell carcinomas. METHODS: The combination of all trans retinoic acid (ATRA) and interferon-gamma (IFN-gamma) was tested for its antiproliferative activity against human tongue squamous cell carcinoma cell line (Tca8113 cells) in vitro. Clonogenic assay, MTT assay, and microscopic cell analysis were performed in this experiment. RESULTS: The results show that the combination of ATRA and IFN-gamma can produce synergistic antitumor effects, resulting from induction of apoptosis, on Tca8113 cells. CONCLUSION: The combination of ATRA and IFN-gamma is a more effective induced-apoptosis therapy for treating oral squamous cell carcinoma.

[Analysis of the treatment of 82 patients with inhalation injury].

OBJECTIVE: To learn the experiences and improve the treatment of inhalation injury. METHODS: Causes of death and treatment effects were analyzed in a group of 82 patients admitted from January 1994 to April 1998. RESULTS: Among the 82 patients, 33 patients were cured, 11 patients were improved and 38 patients died. Among the 38 cases of death, 24 patients died directly of inhalation injury and 14 patients died of other complications. CONCLUSION: The major causes of death of the patients with inhalation injury are acute intoxication, asphyxia, and ARDS(acute respiratory distress syndrome). For the treatment, tracheostomy should be carried out the earlier the better. Moistening of the airway and lavage may be effective in preventing lung infection, facilitating the ejection of phlegm and necrotic mucosa. The amount of fluid infusion should appropriately be increased and this will not lead to lung edema. Administration of corticol hormone in the early stage for a short period will also result in positive effects.

Clinical analysis of 69 patients with familial benign chronic pemphigus.

OBJECTIVE: To analyze the clinical feature, efficacy of treatment and prognosis in familial benign chronic pemphigus (FBCP). METHODS: Sixty-nine cases of FBCP were retrospectively analyzed. RESULTS: The ratio of male to female is 3.93:1 in 69 patients (55 males, 14 females). The mean age at the onset was 29.09 years (3-60 years). There was familial history in 27 families in all of the cases. The lesion usually involved in genital area, neck, axillae and popliteal fossa. Erythemas and vesicles on the soles were seen only in 1 case. Histopathologically 44 cases had special features of FBCP, and immunopathologically 8 cases were direct immunofluorescence (DIF) negative, in which one case had C3 linear deposition along dermoepidermal junction. The combined regimen was more effective. The low-dose X-ray could improve the effect. CONCLUSION: The disease is transmitted as an irregular autosomal dominant trait. The condition in males is more frequent than that in females, probably owing to the different level of female hormone in both sexes. Our patients have the same clinical features as those reported in the literature, but the erythema, vesicle lesions on sole have not been documented in the literature. The combined therapy should be adopted in this condition.

[Homozygous deletion and methylation of p16 and p15 gene in acute leukemia].

OBJECTIVE: To investigate the frequency of p16 and p15 gene inactivation in acute leukemia and to evaluate its clinical significance. METHODS: Fifty-six patients with newly diagnosed acute leukemia was studied. PCR technique was used to detect homozygous deletion of p16 and p15 gene, restriction enzyme-PCR technique was used to detect gene methylation, and TdT-mediated dUTP-digoxygenin end-labeling (TUNEL) technique was used to detect cell apoptosis. RESULTS: p16 and/or p15 gene inactivation was detected in 33 of the 56 patients, including 23/38 (60.5%) of the ALL patients (T-ALL 12/16, B-ALL 11/22) and 10/18 (55.5%) of the ANLL patients. For All patients, methylation was the major pathway of p16 and p15 gene inactivation. Patients with p16 and/or p15 gene inactivation had a delayed apoptosis, a poor response to chemotherapy, a lower remission rate and a shortened remission duration. CONCLUSION: The inactivation of p16 and p15 gene plays a key role in the pathogenesis of acute leukemia.

[A rat model of tongue mucosa squamous cell carcinoma induced by oral administration of 4NQO in drinking water].

OBJECTIVE: To establish a more realistic animal model for oral carcinogenesis which reveals histological and immunological characteristics similar to the human counterpart. METHODS: 0.002% 4-nitroquinoline 1-oxide (4NQO) in drinking water was administered orally to SD rats for 9-32 weeks. Then the rats were killed and their tongues were removed for histological assessment. RESULTS: Gross changes included leukoplakia, erosion, ulcer and papillary appearance on the dorsum of the posterior tongue were present during carcinogenesis. Their corresponding histopathological findings ranged from hyperplasia (HP), mild-moderate dysplasia (mmDP), severe dysplasia (sDP) and in situ carcinoma (ISC) to well-differentiated invasive squamous cell carcinoma (SCC). The severity of lesions corresponded to the duration of administration. The tongues in rats treated with 4NQO for 9 weeks showed HP (80.0%) and mmDP (20.0%); those for 13 weeks showed mmDP (66.6%) and sDP (33.3%); those for 16 weeks showed sDP (55.5%) and ISC (44.4%); and those for 32 weeks showed sDP (12.5%), ISC (12.5%) and SCC (75.0%). The incidence of tongue cancer in rats treated with 4NQO for 9 weeks, 13 weeks, 16 weeks and then observed for 32 weeks was 50.0%, 62.5%, and 77.8%, respectively. No metastases were found. CONCLUSION: 4NQO reliably produced preneoplastic and neoplastic tongue mucosa lesions, which morphologically and histologically mimic human oral carcinogenesis. The rat tongue, a target organ of 4NQO, is not an immunologically privileged site like the hamster buccal pouch. Thus, the model should be appropriate to study molecular mechanism of neoplastic transformation and to assess new treatment modalities of premalignant and malignant lesions of the human oral cavity.

[A study on the pathogenicity of acute hepatitis G infection].

OBJECTIVE: To study the infection due to hepatitis G virus (HGV) in liver tissues of patients with acute hepatitis and to investigate its pathogenicity. METHODS: HGV NS5 antigen was detected by using immunohistochemical method in the liver biopsy tissues of patients with acute hepatitis but serologically negative to hepatitis virus antigens from A to E. The clinical and pathological data were also analysed. RESULTS: 37 samples were tested, HGV NS5 antigen was detected in 14 (37.8%). 4 of the 14 were positive for HGV NS5 antigen alone (acute hepatitis G group) and 10 other cases were also positive for HBsAg, hepatitis C virus (HCV) NS3 antigen or EB virus antigen (superinfection group). 7 cases were HBsAg positive alone (acute hepatitis B group) and the remaining 16 cases were negative for all the antigens mentioned above (non-A - G hepatitis group). There was no difference among the 4 groups in the detecting rate of Fas antigen, histological activity index (HAI), serum alanine transferase (ALT) and total bilirubin (TBil). CONCLUSION: It is suggested that HGV appears to play some role in the pathogenesis of acute viral hepatitis and the degree of liver injury in acute hepatitis G may be as severe as that of hepatitis caused by other types of viruses.

[Detection of TNF-alpha and NCF in the serum and BALF of patients with sarcoidosis and evaluate their clinical significance].

OBJECTIVE: To evaluate the role of cytokines released by the inflammatory cells and immunocytes from patients in pathogenesis of developing sarcoidosis. METHODS: With well- microchemotaxis and allergy immunology, the level of tumor necrosis factor-alpha (TNF-alpha) and neutrophil chemotactic factor (NCF) was measured in the serum and BALF of 11 sarcoidosis and 7 IPF and 8 normal subjects (non-smokers). RESULTS: The levels of TNFalpha (11.9 +/- 3.2, 11.7 +/- 3.0 ng x L(-1)) and NCF (191 +/- 51, 203 +/- 44 cell x 10HP(-1)) of BALF in the patients with sarcoidosis and IPF were significantly higher than these in control group (P < 0.01) and were higher than those in serum. The level of TNF-alpha in the BALF of patients with sarcoidosis was positively correlated with the percentage of lymphocytes (r = 0.73, P < 0.01). The activity of NCF in the BALF of patients with IPF was positively correlated with the percentage of neutrophils (r = 0.89, P < 0.01). CONCLUSIONS: It indicated that TNF-alpha and NCF might play an important role in the pathogenetic process of the sarcoidosis and IPF, and can act as the marker of activity of these diseases.

[Phacoemulsification and posterior chamber intraocular lens implantation in high myopia].

OBJECTIVE: To evaluate the therapeutic effect of phacoemulsification and posterior chamber intraocular lens (IOL) implantation in high myopia. METHODS: Phacoemulsification with implantation of a foldable IOL or a PMMA rigid IOL was performed on 28 eyes of 21 patients with senile, complicated or congenital cataracts through a 3.2 mm small scleral tunnel incision or through a 5.5 mm incision respectively. The average age of the patients was 51.32 years (21 - 70 years). The mean axial length was (27.56 +/- 1.21) mm (26.00 - 33.41 mm). The mean keratometry was (43.38 +/- 1.36) D (42.00 - 46.54 D). The mean diopter of IOL was (+ 9.59 +/- 3.65) D (+ 4.00 - + 15.00 D). RESULTS: Postoperatively, the uncorrected or corrected visual acuity 0.5 or better at 1 day, 1 week, 1 month and 3 months was in 63.6%, 86.4%, 95.5% and 95.2% patients respectively; the patients having achieved the best visual acuity of 1.0 or better were accounted for 9.1%, 31.8%, 50.0% and 57.1% respectively. CONCLUSIONS: Phacoemulsification and the posterior chamber IOL implantation offers the advantages of rapid and better rehabilitation of visual acuity and more stabilization of postoperative astigmatism in patients with high myopia; whereas for eyes with the axial lengths over 30 mm, the recovery of the visual acuity was less satisfactory.

[Clinical significance of preoperative B-ultrasonography for traumatic cataract].

OBJECTIVE: To approach the clinical significance of B-ultrasonography in detection of the break of posterior lens capsule in traumatic cataract. METHODS: 30 patients (30 eyes) of traumatic cataract underwent preoperative B-ultrasonography and intraoperative microscopic examination to evaluate the status of posterior lens capsule. RESULTS: 14 eyes showed posterior lens capsule unbroken in B-ultrasonography. Of them, 12 eyes were confirmed by surgery. 16 eyes showed rupture of the posterior lens capsule in B-ultrasonography. Among them 14 eyes were confirmed by surgery and in 2 eyes there was no break at the posterior lens capsule. CONCLUSIONS: B-ultrasonography for preoperative traumatic cataract is harmless, painless, simple in operative procedure and accurate, thus it can be used as a routine examination to show the status of posterior lens capsule in traumatic cataract.

[Time trend of mortality rates of cerebrovascular diseases in Hong Kong].

OBJECTIVE: To describe the time trend of mortality rates of cerebrovascular diseases (CVD) in Hong Kong for the period of 1976 to 1995, and to compare them with those in the more developed countries around the world. METHODS: Mortality rates of CVD were standardized directly using world standard population in 1961. Annual percent changes of mortality rates of CVD were estimated with a non-linear regression. Birth-cohort analysis was carried out at an interval of five-year of age to compare their age-specific mortality rates of CVD in different birth cohorts. Time trend of cause-specific mortality rates of CVD both in males and females was fitted with a simple linear regression model. RESULTS: Considerable downward trend of CVD mortality was observed during the past twenty years. Mortality rates among men and women decreased by 50.96% and 37.85% in 1995, as compared to those in 1976, respectively. Although in general, a greater trend in CVD mortality was observed during the last 10 years, as compared with that in the previous 10 years, an annual increase in CVD mortality of 1.34% was found among males aged 35 to 44 years during the latter 10 years. CVD mortality for both males and females decreased steadily by 1% per year. CVD mortality for females will catch up with that for males by the year of 2013, if such a trend continues. Birth-cohort analysis showed that those born more recently at the same age-group had lower mortality of CVD (except for those born after 1955). Data from hospital admission showed that improvement in the treatment for CVD could have contributed partly to the decrease in its mortality. CONCLUSION: Time trend of mortality of CVD in Hong Kong was similar to that in many other economically developed areas around the world.
