Isolation of a cDNA clone encoding a novel form of granzyme B from human NK cells and mapping to chromosome 14.

We have isolated cDNA clones from a human NK cell cDNA library that encode the serine protease granzyme B. Although the sequence of the entire coding region for the mature protein and the 3' untranslated region of the clone are identical to other cDNA isolates of this gene obtained from human T cell cDNA libraries, the 5' end of two clones is 103 bp longer than the previously described sequences and would encode a protein with a 54-amino-acid-long signal sequence. Experiments characterizing granzyme B mRNA suggest that transcripts that initiate at or before the 5' end of these clones comprise a detectable but infrequent class of granzyme B transcripts in NK and T cells. We have mapped this gene to human chromosome 14 in the region 14q11----14q32, distal to the T cell receptor alpha locus and proximal to the immunoglobulin heavy chain locus. The chromosomal location of this gene, together with the previously described high sequence homology between this gene and the mouse CTLA 1/ccp1 gene, make it likely that this is the human equivalent of the mouse CTLA1/ccp1.

Cation-dependent mannose 6-phosphate receptor contains two internalization signals in its cytoplasmic domain.

The signals required for rapid internalization of the bovine cation-dependent mannose 6-phosphate receptor have been localized to two distinct regions of the 67-amino acid cytoplasmic domain. One signal includes phenylalanine 13 and phenylalanine 18, while the other involves tyrosine 45. The former signal is more potent than the latter, but both must be present for the maximal rate of receptor internalization. Each signal shares similarities with the known internalization signals of other recycling receptors.

Sensitivity and specificity of various morphological features of cervical condylomas. An in situ hybridization study.

Fifty-seven cervical biopsy specimens or endocervical curettings showing condyloma, changes suggestive of condyloma, or no changes of condyloma were analyzed for presence of nuclear atypia (nuclear enlargement and irregularity in superficial epithelium), presence of multinucleated cells, and presence of perinuclear cytoplasmic clearing in superficial squamous epithelium. The findings were correlated with results of in situ hybridization with biotin-labeled human papillomavirus DNA probes. Moderate nuclear atypia was significantly more specific than perinuclear cytoplasmic clearing and 100% sensitive for predicting cases positive for human papillomavirus. Of the various morphological features analyzed, perinuclear cytoplasmic clearing had the lowest specificity for predicting positive results on in situ hybridization.

Distribution and frequency of neuro-epithelial bodies in post-natal rabbit lung: quantitative study with monoclonal antibody against serotonin.

The distribution, frequency and size of neuroepithelial bodies (NEB) were studied in lungs of rabbits during different stages of development (27-day fetus, newborn, 6, 11, 21, 28 and 56 days postnatally). NEB were visualized by immunostaining with monoclonal antibody against serotonin. Detailed quantitation of NEB was performed by use of camera lucida drawings of immunostained serial sections from the same anatomical region, i.e. the lower lobe of the left lung. The total number of NEB was counted and expressed per epithelial length of airway, surface area and volume. The size of NEB defined as surface area as well as the position of NEB in relation to the airway bifurcations was assessed in airways of different sizes. The overall number and size of NEB were found to increase during the immediate perinatal period followed by a sharp decline at 56 days of age. The number of NEB peaked at 6 days postnatally (mean 175.5 NEB/mm3 of airway epithelium) and declined significantly (3.0 NEB/mm3) at 56 days of postnatal age. The size of NEB reached its maximum at 11 days (mean surface area 659.54 microns 2, with the largest NEB measuring 1839.98 microns 2). By 56 days of age, NEB became significantly smaller (mean surface area 177.29 microns 2) consisting of small clusters of cells situated deep within the airway epithelium. At all ages, about half of all NEB (mean 47.6%) were localized within the small peripheral airways with up to 63.9% located at airway bifurcations. These findings indicate that the "functional activity" of NEB may be confined predominantly to the perinatal period. The postulated functions of NEB include those of intrapulmonary hypoxia-sensitive chemoreceptors and/or endocrine-paracrine activity in the lung. Such functions(s) may be important during adaptation to extrauterine life as well as for growth and development of the lung.

Short- and long-term results of transvenous catheter ablation of the atrioventricular conduction system.

Twenty-two patients with refractory supraventricular arrhythmias were treated by catheter-delivered high-energy shocks to the atrioventricular conduction system. All patients had a minimum follow-up period of six months (mean +/- SD, 15 +/- 9 months), at which time 21 of the 22 patients were free of symptoms and required no antiarrhythmic therapy. Permanent pacemakers were implanted in all patients. These results show that transvenous ablation or modification of atrioventricular conduction is a safe and effective technique to treat a wide range of supraventricular arrhythmias, and obviates the need for open-heart surgery for the interruption of atrioventricular nodal conduction.

Hydrazine stress in the diabetic: ornithine decarboxylase activity.

Streptozotocin-induced diabetes of 7 weeks duration increased male Sprague-Dawley rat kidney ornithine decarboxylase activity by 4.8-fold but did not affect the liver enzyme. Hydrazine treatment of 4 hr duration stimulated equally kidney ornithine decarboxylase activities of nondiabetic and diabetic rats. Hydrazine treatment increased liver ornithine decarboxylase activity in the nondiabetic rat but did not increase it in the diabetic rat. Since hydrazine stimulates ornithine decarboxylase activity prior to polyamine and protein syntheses, we speculate that the lack of hydrazine stimulation of ornithine decarboxylase in the diabetic liver may be related in part to the unrestrained gluconeogenesis and depressed Kreb's cycle activity: the latter being required for protein synthesis.

Immunological responses to chronic heat exposure and food restriction in rats.

Immunological variables were studied in rats chronically exposed to high environmental temperature (35 degrees C). Responses were compared with those of rats at 25 degrees C both fed ad libitum and pair fed to the decreased intake found in heat-exposed rats. Heat-exposed rats showed slower delayed-type hypersensitivity responses to keyhole limpet hemocyanin. They showed lower counts of peripheral blood total T cells (OX19+) as well as helper T cells (W3/25+) and smaller numbers of splenic T cells. The thymus was decreased in size. Increased levels of serum IgG antitetanus toxoid antibodies were found in heat-exposed rats. [3H]-thymidine incorporation into Concanavalin A (ConA)-stimulated splenic lymphocytes was decreased in pair-fed rats but not significantly altered in heat-exposed rats compared with controls. Heat exposure alters some aspects of both cellular and humoral immune function in a manner different from that induced by comparable food restriction without heat exposure.

Assessment of aortocoronary bypass graft patency by intra-arterial digital subtraction angiography compared to selective graft angiography.

The value of digital subtraction angiography (DSA), in assessing aortocoronary bypass graft patency, was evaluated by studying 22 consecutive patients with 54 aortocoronary saphenous vein bypass grafts, who had postoperative angina pectoris. Each patient underwent selective graft angiography and non-gated DSA. The DSA consisted of a run of 15 to 20 frames, taken at 2.3 frames per second during injection of diluted contrast in the ascending aorta, performed after a test exposure was made. Thirty-two of the 54 grafts (59%) were patent. Thirty of the 32 grafts were seen to be patent by both selective graft angiography and DSA. In addition, a further two grafts were found to be patent on DSA, but were not able to be selectively catheterised and were not seen on the conventional aortogram. Selective graft angiography revealed four tight proximal graft stenoses, of which one only was seen on DSA; and poor distal run-off in five grafts, two of which showed up as late filling grafts on DSA. In summary, the accuracy of intra-arterial DSA in assessment of bypass graft patency was excellent. All grafts seen to be patent on selective graft angiography were also seen by DSA alone and in addition two grafts which could not be selectively catheterised were found to be patent. However, in patients with postoperative chest pain, selective graft angiography is probably required as non-gated intra-arterial DSA does not provide sufficient information to assess graft stenoses, distal flow and the native coronary vessels.

Value of routine histopathological examination of appendices in Hong Kong.

A retrospective study of the histopathological findings of more than 11,443 appendices submitted as surgical specimens over 14 years was performed in this department. In most cases routine histopathological examination added little clinically important information to other clinical and operative gross findings, but a variety of interesting and uncommon lesions were identified. In 85 cases clinically important pathological findings were first discovered on routine histopathological examination. These included enterobiasis, schistosomiasis, mucocele, trichuriasis, tuberculosis, ascariasis, endometriosis, mucinous cystadenoma, granuloma, carcinoid tumour, neuroma, clonorchiasis, primary adenocarcinoma and secondary carcinoma.

Serotonin in the human infant carotid body.

By immunocytochemistry serotonin was localized in the chief cells of the carotid body in human infants. Radioenzymatic measurement of the serotonin concentration revealed that it represents a significant proportion of the total amine content of the carotid body.

The efficacy of total parenteral nutrition in malnourished tumor-bearing rats.

The current study was designed to compare the efficacy of total parenteral nutrition (TPN), provided either before or after tumor removal, in replenishing protein stores in the malnourished tumor-bearing (TB) host. Fisher 344 rats bearing a transplantable methylcholanthrene (MCA)-induced sarcoma were used. After the animals were cachectic from their tumor, a central venous catheter was inserted. At the time of catheter insertion, the animals were randomized into two groups. Group I rats had the TB hindlimb amputated whereas Group II rats had the nontumor bearing (NTB) hindlimb amputated. After amputation, all animals were given TPN for 10 days then sacrificed. The results demonstrated that TB rats (Group II) had a significantly decreased tumor-free carcass weight in contrast to nontumor bearing rats (Group I). Furthermore, the TB animals had a significantly increased body weight gain after completion of TPN. This difference in body weight proved to be due almost entirely to the weight of the growing tumor. Compositional analysis revealed a significantly decreased protein stores and significantly increased fat stores of both the carcass and liver of the TB animals compared to those which the tumor had been resected. The results of the data suggest that TPN given to the sarcoma-bearing animal after tumor removal is better utilized for replenishing protein stores than when it is provided before removal of the tumor.

Ventricular tachycardia and sudden death in myotonic dystrophy: clinical, electrophysiologic and pathologic features.

A 37 year old man who presented with a cardiomyopathy, conduction defects and atrial and ventricular arrhythmias was found to have the neuromuscular manifestations of myotonic dystrophy. Despite implantation of a permanent cardiac pacemaker, antiarrhythmic drug therapy and antiarrhythmic surgery, sudden death occurred. The results of electrophysiologic studies, coronary arteriography and pathologic findings are described. This case confirms previous observations that ventricular arrhythmias, in addition to atrial arrhythmias and conduction disturbances, are cardiac manifestations of myotonic dystrophy and can lead to sudden death.

In vitro characteristics of pulmonary neuroendocrine cells isolated from rabbit fetal lung. I. Effects of culture media and nerve growth factor.

Pulmonary neuroendocrine (NE) cells, dispersed throughout the airway mucosa as single cells and as innervated clusters (neuroepithelial bodies), were isolated from rabbit fetal lung and studied in short-term culture. The effects of culture media and nerve growth factor (NGF) on in vitro maintenance, differentation, and cell kinetics of isolated NE cells were examined. For demonstration of NE cells in intact lung, during cell separation and after culture, immunostaining for serotonin, formaldehyde-induced fluorescence method, histochemical reaction for acetylcholinesterase, and electron microscopy were used. The isolation procedure consisted of mechanical and enzymatic dissociation of lung tissue followed by separation of isolated cells on a discontinuous gradient of Percoll, resulting in 5- to 10-fold enrichment in NE cells. Cell fractions enriched in NE cells were cultured up to 7 days either in supplemented alpha-minimal essential medium with fetal bovine serum or in defined, hormone-supplemented, serum-free medium. NGF (2.5 S 5 to 50 ng/ml) was added to both serum-supplemented and serum-free media; cultures without NGF served as control. The number of serotonin-immunoreactive NE cells maintained in serum-supplemented medium (0.5% fetal bovine serum) increased significantly (p less than 0.05) on days 4 and 7 compared with cultures grown in serum-free medium. NE cells maintained in serum-supplemented medium incorporated [3H]thymidine and their labeling index was significantly increased (p less than 0.01) on day 7, whereas few or no NE cells were labeled in cultures grown in serum-free medium. NGF had no effect on the maintenance or kinetics of NE cells. Cultured NE cells formed elongated (unipolar or bipolar) neurite-like cytoplasmic processes with a button-like ending, regardless of the presence of NGF. Amine accumulated in perinuclear cytoplasm and in button-like endings. Staining for acetylcholinesterase (strongly positive in intact neuroepithelial bodies) was not detectable after separation into single cells, culture, or exposure to NGF. This study demonstrates that NE cells can be isolated from rabbit fetal lung and maintained in short-term culture. Low concentrations of fetal bovine serum enhanced the in vitro maintenance of NE cells, whereas NGF had no such effect. A feeder layer may be also important, since NE cells were closely associated with lung epithelial or fibroblast-like cells. The formation of neurite-like processes appears to be an expression of paracrine/paraneuron-type cell differentiation not mediated by NGF.(ABSTRACT TRUNCATED AT 400 WORDS)

Arrhythmogenic right ventricular dysplasia. Clinical, electrophysiological, and pathological features.

Arrhythmogenic right ventricular dysplasia (ARVD) describes the syndrome of recurrent ventricular tachycardia of right ventricular origin and cardiomyopathic changes of the right ventricle. We report the clinical, electrophysiologic, and angiographic characteristics of four patients who presented with ventricular tachycardia of left bundle branch block configuration, and in whom the right ventricular origin of tachycardia was confirmed by endocardial mapping, and a diagnosis of ARVD was substantiated by histological examination. ARVD should be suspected in all patients with ventricular tachycardia of left bundle branch block configuration, especially in young adults with an otherwise normal heart. Once suspected, diagnosis can often be established by non-invasive investigation. Surgical treatment may be difficult because of the diffuse nature of right ventricular involvement.

The effect of dobutamine on exercise performance in patients with symptomatic ischemic heart disease.

The effect of dobutamine on exercise performance was assessed in 20 patients with ischemic heart disease (CAD) and a positive stress test. These patients had a wide range of resting left ventricular ejection fraction (range 22% to 69%, mean 42%). Each patient entered a double-blind crossover study in which two identical exercise radionuclide ventriculograms were performed in patients on dobutamine, 5 micrograms/kg/min intravenously, or placebo. Dobutamine increased resting left ventricular ejection fraction. Although ejection fraction fell with dobutamine during submaximal exercise, it remained higher than with placebo. At peak exercise, ejection fraction fell to the same level on dobutamine as with placebo. Dobutamine diminished exercise time and time to ischemia while peak pressure-rate product was unchanged. Four of 20 patients developed complex ventricular premature beats, all while on dobutamine. Although useful when administered to resting patients with acute left ventricular failure, dobutamine's effects may be deleterious in exercising patients with chronic ischemic heart disease.

Effects of statistical quality, sampling rate and temporal filtering techniques on the extraction of functional parameters from the left ventricular time-activity curves.

Several techniques for the processing of a series of curves derived from two left ventricular time-activity curves acquired at rest and during exercise with a nuclear stethoscope were evaluated. They were three and five point time smoothing, Fourier filtering preserving one to four harmonics (H), truncated curve Fourier filtering, and third degree polynomial curve fitting. Each filter's ability to recover, with fidelity, systolic and diastolic function parameters was evaluated under increasingly 'noisy' conditions and at several sampling rates. Third degree polynomial curve fittings and truncated Fourier filters exhibited very high sensitivity to noise. Three and five point time smoothing had moderate sensitivity to noise, but were highly affected by sampling rate. Fourier filtering preserving 2H or 3H produced the best compromise with high resilience to noise and independence of sampling rate as far as the recovery of these functional parameters is concerned.
