Gelfoam and autologous clot embolization: effect on coagulation.

An episode of disseminated intravascular coagulation following therapeutic gelfoam embolization to control bleeding from esophageal varices in a patient with liver disease is presented. We have since followed 13 patients prospectively (six control and seven gelfoam/autologous clot) to determine the effect of this procedure on clotting. We were unable to show significant differences between the two groups as measured by the prothrombin time, activated partial thromboplastin time, thrombin time, fibrinogen and platelet count. However, fibrin (ogen) degradation products were significantly elevated (p less than .01) in the gelfoam/autologous clot group. We suspect this occurred secondary to clot lysis at the site of embolization. No subsequent bleeding diathesis attributable to this abnormality occurred in any of the patients.

CT appearance of appendicitis and its local complications.

During evaluation for occult fever or nonspecific abdominal pain CT will occasionally identify inflammatory disease of the appendix as the underlying cause. In these cases CT may also provide useful information about the presence of associated mesenteric inflammation, abscess, or perforation. Five cases are presented in which CT provided clinically useful information supplementing that gained from barium studies and clinical presentation. When a periappendiceal lesion is found, the extent and nature of inflammatory changes are shown directly rather than inferentially.

Problems in CT diagnosis of ruptured abdominal aortic aneurysms.

In a review of 17 cases in which the CT diagnosis of abdominal aortic aneurysm rupture was prospectively made and for which surgical correlation was available the CT diagnosis was correct in 10 and incorrect in seven. Two of the false positives were retrospectively reinterpreted as negative for rupture. Characteristics of 10 surgically confirmed cases revealed a spectrum of appearances not always in agreement with prior published reports, probably due to the age and magnitude of the aortic leaks at the time of diagnosis. The most common characteristic of aneurysm leak (eight of 10 cases) was an abnormal soft tissue collection located adjacent to the posterior aspect of the aneurysm, a sign not previously emphasized in other reports. Renal displacement was also a valuable sign but was present in only three cases, all of which had large hemorrhages. Other characteristics of aortic rupture such as density of the collection and sharpness of its margins were not found to be generally useful.

Use of steel pins in hair transplantation.

Steel pins are used to stabilize hair transplants by preventing transplanted plugs from becoming elevated, depressed or turned in the wrong direction. The pinning procedure is less traumatic to hair follicles than is tight bandaging and bleeding is more easily controlled. Patients so treated have improved final appearances. The authors' technique of using steel pins is described.

Reduced metabolic complications in total parenteral nutrition: pilot study using fat to replace one-third of glucose calories.

The limitations of glucose-based TPN solutions are high glucose concentration, high osmolality, lack of fat, and essential fatty acids, which result in glucose intolerance and hepatotoxic effects. We replaced one-third of the calories in a standard amino acid-glucose solution with Liposyn 10% for 14 days in 23 critically ill men who needed total parenteral nutrition. Serial measurements included weight, albumin, glucose, triglyceride concentrations, and liver function tests. Serum osmolality was calculated, and found to remain constant. Body weight and serum albumin were maintained. Minor changes occurred in hepatic enzymes which were physiologically insignificant. Glycosuria occurred in 15%. Adverse side effects of hypoglycemia, hyperosmolar coma, and hypertriglyceridemia were avoided. Our results show that the addition of fat reduced glucose and hepatic related metabolic complications.

Axillo-femoral (PTFE) and infrainguinal revascularization (PTFE and umbilical vein). Vascular Registry of the New England Society for Vascular Surgery.

The Vascular Registry Committee of the New England Society for Vascular Surgery (NESVS) has prospectively evaluated prosthetic bypass grafts. A record of these operations and follow-up data were voluntarily submitted by members of the Society during the last 13 years. The 5-year primary patency rate for 103 axillo-femoral procedures was significantly greater for the bifemoral group (55%) than for the unifemoral group (14%). For infrainguinal reconstruction (402 cases), the 5-year primary patency rates were as follows: 57% (fem-pop AK PTFE claudication); 46% (fem-pop AK PTFE rest pain); 32% (fem-pop AK PTFE necrosis); 26% (fem-pop BK PTFE claudication); 17% (fem-pop BK PTFE rest pain); 21% (fem-pop BK PTFE necrosis); 69% (fem-pop AK umbilical vein); and 45% (fem-pop BK umbilical vein). None of the femoral tibial or peroneal prosthetic bypass grafts remained patent for 5 years. Umbilical vein bypasses may have had a higher patency than PTFE bypass; however, they did develop aneurysmal changes.

Fluoroperm extended wear RGP contact lenses: II. Front toric fitting.

Seventeen eyes (10 patients) were fit with front toric rigid gas permeable (RGP) Fluoroperm lenses as part of an extended wear study. These 17 eyes required 40 fittings (an average of 2.35 fittings per eye); only six eyes were fit successfully the first time. What occasioned the large number of fittings was unacceptable visual acuity due to incorrect lens axis. Most fittings had been done with a polymethylmethacrylate (PMMA) fitting set; and it became obvious that the final Fluoroperm lenses rotated differently from the PMMA diagnostic lenses. Ultimately, the use of a Fluoroperm prism-ballasted fitting set resulted in 16 of 17 eyes (94%) seeing well with a front toric lens. We advise that, for front toric fittings, the diagnostic lens be of the same material as the final lens.

Fluoroperm extended wear RGP contact lenses for myopia, hyperopia, aphakia, astigmatism, and keratoconus.

One hundred fifteen eyes of 62 patients were fit with paflufocon-A (Fluoroperm) rigid gas permeable (RGP) contact lenses. Patients included myopes, hyperopes, aphakes, and keratoconic and astigmatic patients; the latter patients were fit with bitoric designs for large amounts of corneal astigmatism or front toric designs for residual astigmatism. Thirty of the 52 patients (58%) who completed the study were able to wear the lenses continuously without complications; six (12%) were able to achieve extended wear for up to one month. The average visual acuity across the study was 20/20. Three complications were observed, resulting in a complication rate of 6%.

Near infrared reflectance spectroscopic determination of protein nitrogen in plant tissue.

A rapid, nondestructive method is described for the determination of protein nitrogen in plant tissue, using near infrared reflectance (NIR) spectroscopy. Procedures for instrument calibration are discussed. Comparisons between Kjeldahl nitrogen and NIR nitrogen are made for corn leaf tissue from Georgia and Indiana. Multiple correlation coefficients for other plant tissues such as peanuts, soybean, wheat, pecan, bermuda grass, and bent grass are also shown.
