[25-hydroxy-vitamin-D in serum of newborns and infants during continuous oral vitamin D treatment (author's transl)].

Using the Haddad modified method, 25-OH-D were measured in the blood of the umbilical cord of 29 infants and in peripheral serum after 6 weeks. 16 infants were given a daily dosage of 1000 I. E., 13 infants 500 I. E. vitamin D against rickets. Further they were fed with an adapted milk containing 400 I. E. vitamin D/1. The mean cord serum values were 13 and 15 ng/ml. After treatment with 1000 I. E., 25-OH-D values around 54 ng/ml were measured after 6 weeks and under 500 I. E. daily, values of 37 ng/ml, respectively. Treatment using a dosage of 500 I. E. vitamin D combined with feeding with vitamin D fortified milk seems adequate, to prevent vitamin D depletion.

Human body composition as related to surface area.

There is a linear relationship between extracellular fluid and body surface area in man. During growth the body cell mass, fat-free solids, and body fat proportions of body weight increase while the extracellular fluid fraction decreases. Growth stops in men as soon as the body cell mass reaches 22.5 kg/m2 body surface area and in women when it reaches 16.9 kg/m5. The non-muscle cell mass and the muscle cell mass reach a 1:4 ratio in men whereas in women a 1:35 proportion--which is more favorable for pregnancy and lactation--is reached. The body composition of the newborn and the infant have fetal characteristics with a low body fat: surface area ratio and a low non-muscle cell mass: surface area ratio. The basal metabolic rate depends on the amount of non-muscle mass present, and the total energy change depends on body cell mass. This has been established by measurements of total body potassium, thiosulfate space, total body sodium, 24 h urine creatinine values and basal metabolic rate.

[Pluriglandular insufficiency due to transfusion haemosiderosis in Blackfan-Diamond anaemia (author's transl)].

The article reports on a boy aged 16 1/2 years suffering from transfusion haemosiderosis associated with corticoid-resistant blackfan diamond anaemia. Besides hepatosplenomegalia and cardiomegalia, the course of the disease was mainly determined by the deficient functioning of several endocrine glands. The diagnosis revealed primary hypothyroidism, hypoparathyroidism and partial insufficiency of the pituitary gland.

[Longterm controls after endoscopic papillotomy [EPT] (author's transl)].

In 117 patients who have undergone endoscopic papillotomy (EPT) long term controls have been done. 51 were controlled in the hospital and 66 did answer a questionnaire. The mean time interval to the EPT was mean = 21.6 months. Nine out of every ten patients had no complaints. One third had minimal changes of laboratory dates, which can easily be explained by second diseases. With nearly no exception there was a large orifice to the common bile duct at the upper brim of the papilla. There was no bilioduodenal pressure gradient in 75%. Duodenobiliary reflux could be demonstrated in 25% and aerobilia in 65%. Although there was a massive bacteriobilia in all cases, no signs of cholangitis could be found in any patient. As a result, no unfavourable effects of the EPT became obvious during this follow-up study.

[Vitamin D-metabolism and rickets (author's transl)].

By new methods Vitamin D metabolites can now be determined in small quantities of biological fluids. These methods have revealed further details about Vitamin D metabolism as well as the pathogeneses and therapy of the various types of rickets. A review of this topic will be presented.

[Total parenteral nutrition as primary therapy in Crohn's disease (author's transl)].

Total parenteral nutrition was given to 10 patients with acute regional enteritis for 3--12 weeks. The rationale of this therapy is elimination of potential food allergens and decrease of intestinal irritation. Complete remission of the disease could be achieved in 8 patients, thus surgical removal of the intestine involved could be avoided. One entero-cutaneous and two perinal fistulas healed spontaneously; two other perianal fistulas improved so much, that local surgical treatment was sufficient. If well planned, total parenteral nutrition can be performed without disturbance of metabolism and septicemia caused by the catheter can be avoided. No final judgement about recurrence of regional enteritis after total parenteral nutrition is possible at present due to as yet limited experience.

[Body composition during the first two years of life: analysis with the potassium 40 method (author's transl)].

The potassium 40 method was used to determine the total amount of potassium in 51 premature babies, 52 newborns, and 24 infants. The mean body composition was calculated from the total amount of potassium and the body surface for various age groups via the method presented. Changes in body composition during the first two years of life and its significance for energy exchange were discussed.

[Determination of lean body mass by skin fold measurements (author's transl)].

Skin folds were measured on 12 different body sites of 32 boys, aged 9--12, by using the calipers of Best, Lange, Harpenden and Holtain successively. We compared the measurements of the biceps, triceps, scapula, suprailiaca, abdominal and thigh skin folds. The results depended on body sites as well as on the type of caliper used. Therefore, conversion is recommendable only when absolute values of body fat must be taken into consideration, as when investigating population parameters in terms of functional performance capacity. Moreover conversion is necessary when using those nomograms which are built up mainly by measurements with an unusual caliper. Conversions must be omitted when comparing data by means of rank-correlations.
