Influence of captopril on the vasopressor effect of serotonin in pithed rats.

In pithed rats intravenous administration of serotonin (5.0, 10.0, 20.0, 30.0 and 50.0 micrograms/kg) produced a dose-dependent increase in blood pressure. The pressor response to serotonin was significantly reduced by captopril (1.0, 5.0 and 50.0 mg/kg) in a dose-dependent manner. Captopril did not attenuate the response of the isolated tail artery to serotonin (5-HT) and did not amplify the dilator effect of serotonin in pithed rats pretreated with ketanserin (1.0 mg/kg). These data suggest that the action of captopril does not depend on its influence on serotoninergic receptors in blood vessels.

Hospital utilization and expenditures in a Medicaid population.

Determinants of hospital utilization and expenditures are analyzed for Medicaid enrollees in the State Medicaid household sample portion of the National Medical Care Utilization and Expenditure Survey who were continuously enrolled throughout 1980. Health status measures were the best predictors of both the probability of hospitalization and total hospitalizations. Children covered by Aid to Families with Dependent Children were the Medicaid enrollees least likely to be hospitalized. Number of hospital days, surgery, and California residence directly increased hospital expenditures. Conditions responsible for hospitalization increased hospital expenditures indirectly by increasing the number of hospital days and the probability of surgery.

[Effect of Polfos on the bleeding time. III. Changes in various parameters of the circulatory system].

The influence of Polfos on the blood pressure in rats was measured by direct and indirect methods. The influence of this pesticide on the heart rate was also examined. Two hours after Polfos administration an increase in blood pressure and a decrease in heart rate were observed. In the late phase of Polfos-intoxication no changes in the cardiovascular system were found. The results show that prolonged bleeding time in the initial phase of the intoxication might depend on its influence on the cardiovascular system in rats.

[Effect of Polfos on the bleeding time. II. Changes in selected parameters of the blood coagulation system and fibrinolysis].

The influence of Polfos on some parameters of the coagulation system and fibrinolysis in rats was examined. The present results indicate that Polfos had no effect on the coagulation system and fibrinolysis in the initial phase of intoxication (2 hours). In the late phase of intoxication (24 hours) a decrease in the prothrombin index, shortening of thrombin time, increase in the fibrinogen level and prolongation of euglobulin lysis time were observed. The results demonstrate that the normalization of the bleeding time in the late phase of intoxication depended on activation of the coagulation system and inhibition of fibrinolysis activation.

Effects of verapamil, diltiazem and nifedipine on some parameters of hemostasis in the rat.

Together with a mechanism such as activation of the coagulation system, vascular smooth muscle contraction and the activation of blood platelets are important processes in the hemostasis. Various calcium channel blockers besides those found to be vasodilators (1) have been shown to influence the functioning of blood platelets (2). They inhibit the aggregation induced by ADP, adrenaline, arachidonic acid or collagen (3). From these data it has been suggested that significant differences exist in the ability of calcium channel blockers to inhibit aggregatory responses (4). In order to elucidate further the possible role of calcium channel blockers in hemostasis we analyzed the effect of verapamil, diltiazem and nifedipine on the bleeding time, the amount of initial blood loss, platelet aggregation and some coagulation parameters in rats.

Medicaid Tape-to-Tape findings: California, New York, and Michigan, 1981.

Presented in this report is an overview of Medicaid enrollment, utilization, and expenditures in California during 1981. The California Medicaid program, called Medi-Cal, is the largest in the Nation in terms of program beneficiaries. During 1981, California had one of the most generous Medicaid programs in the country in terms of eligibility and covered services. At the same time, there were benefit limitations and reimbursement restrictions in place that were designed to restrict program expenditures. The data in this report were provided by the State to the Health Care Financing Administration as part of the Medicaid Tape-to-Tape Project. Data from Michigan and New York are also included for comparison purposes.

[Effect of Polfos on bleeding time. I. Changes in the formed elements of the blood in rats exposed to Polfos].

The influence of Polfos and its metabolite, 2,4 dichlorophenacyl bromide, on the bleeding time, count of platelets, platelets aggregation, count of erythrocytes, level of haemoglobin and count of leucocytes in rats were studied. The data indicate that Polfos prolonged the bleeding time in the 2nd hour after its administration. A decrease of the platelets count was observed only in the initial phase of the intoxication. In the study the platelets count, red cells count, hematocrit and the level of haemoglobin increased 24 hours after the administration of the pesticide. The results demonstrate that the prolongation of the bleeding time in the rats after the administration of Polfos partly depends on the changing of the platelets count.

Verapamil inhibition of 5-hydroxytryptamine (5HT) uptake in rat platelets.

The effect of verapamil on 5-hydroxytryptamine (5HT) uptake into rat blood platelets was investigated in vitro. Verapamil produced a non-competitive inhibition (IC50 = 8.2 +/- 0.5 microM) of 5HT uptake. The administration of verapamil to rats also produced inhibition of 5HT uptake and reduced the concentration of this amine in the platelets. The present data indicate that verapamil has an influence on the uptake processes for 5HT in blood platelets in rats.

Hospital utilization and expenditures for Medicaid enrollees by major diagnosis group.

The distribution of Medicaid hospital discharges and expenditures by major diagnosis group for Medicaid enrollees in California, Michigan, and New York during 1982 are examined in this article. Although hospital expenditures represent a major component of Medicaid expenditures, the extent of variation in Medicaid inpatient utilization and expenditures across diagnoses and between States has not been previously studied. In this article, Medicaid inpatient hospital utilization and expenditure data by major diagnosis group from the Health Care Financing Administration's Tape-to-Tape data base are examined to determine whether significant interstate differences exist.

Normal serotonin uptake by blood platelets and brain synaptosomes but selective impairment of platelet serotonin storage in mice with Chediack-Higashi syndrome.

The Chediack-Higashi syndrome (CHS) is an autosomal recessive disorder reported in man and in several animal species including the "beige mice" (bg/bg). Among several manifestations of this genetic trait, deficiency of secretable substances - including serotonin - normally stored in platelet dense granules is a characteristic feature. The animal model of Chediak-Higashi syndrome used in the present study provides a unique opportunity to compare the kinetics of serotonin (5-hydroxytryptamine, 5-HT) uptake in platelets and brain synaptosomes in conditions of selective reduction of 5HT concentration in the platelets. The kinetics of 5HT uptake, as measured in the present study, was normal in synaptosomes and platelets from the same animals. The lower intraplatelet 5HT levels in bg/bg animals as compared to normal synaptosomes levels in the presence of normal uptake offer an indirect proof that the 5HT defect described in the CHS is due to an impaired 5HT storage mechanism. This is supported by the observation that spontaneous release of 5HT was markedly increased in platelets from CH5 mice but was normal in synaptosomes from the same animals. Thus platelets are a reliable model to study 5HT uptake, but not 5HT storage and release in brain synaptosomes.

Physician utilization and expenditures in a Medicaid population.

The determinants of physician visit utilization and expenditures for the full-year Medicaid enrollees in the State Medicaid household survey portion of the National Medical Care Utilization and Expenditure Survey are analyzed in this article. The regression analyses for the probability of a physician visit, for number of physician visits, and for physician visit expenditures underscore the importance of perceived health status as a determinant of both physician utilization and expenditures. Other important determinants of physician utilization and expenditures were regular source of care, State, enrollment group, sex, and family size.

Platelet 5-hydroxytryptamine transport and storage in myeloproliferative disorders.

Platelets of patients with myeloproliferative disorders (MD) such as polycythaemia vera (PV), chronic myelogenous leukaemia (CML), idiopathic myelofibrosis (IM) and essential thrombocythaemia (ET) have been found to have low 5HT levels measured both by a fluorimetric and a liquid chromatographic assay. Km and Vmax for platelet active uptake of 3H-5HT were not significantly different in controls and patients. Inhibition of 5HT reuptake by imipramine or induction of moderate release by fenfluramine were not sufficient to distinguish the group of MD platelets from controls, although some patients had less of a tendency to retain intraplatelet amine. The low platelet 5HT content found in our patients seems not to be the consequence of disturbed active transport of 5HT across platelet membrane. Although defective storage of this amine within the cell is probable, the results of the present study do not rule out the possibility that platelets from MD patients undergo in vivo activation by endogenous stimuli not inhibited by aspirin. 10 d treatment with aspirin did not result in any significant rise in intraplatelet 5HT concentration.

Medicaid program characteristics: effects on health care expenditures and utilization.

Relationships between State Medicaid program characteristics and program outputs are analyzed in this statistical report, using 1980 cross-sectional data from a variety of sources. The year 1980 furnishes a baseline against which program changes following the Omnibus Budget Reconciliation Act of 1981 and the 1982 economic recession can be evaluated. Utilization and expenditures are modeled separately for each aid category and each major service category. This use of multiple models allows for measurement of the effect of program controls that might not appear in models of total utilization and expenditures.
