[Problems in case identification using a semi-structured psychiatric interview as exemplified by an epidemiological study in general practices (author's transl)].

From 1973 to 1974 research psychiatrists studied 1,274 patients in general practices in the county of Traunstein (FRG). On the basis of an interview developed by Goldberg et al. (1970), it could be shown that objective symptoms registered by the interviewers generally differentiate better between cases and non-cases than do subjective symptoms. According to the weighting suggested by the developers of the interview, singly weighting the subjective and doubly weighting the objective symptoms, a total score was compared with the psychiatric diagnosis made by the interviewer. At a cut-off of 20 points, the overall agreement reached an optimum of 83.1% and increased only slightly due to a-posterior weighting of the items. A cluster analysis of the symptoms showed that patients demonstrate significantly different combinations of symptoms according to the nature of their mental disorder.

Diagnosis, Differential

[Sensitivity of various study technics in blood stains].

Quadratic pieces of fleece measuring 16 mm2 were soaked with 10 different blood-samples in the dilution steps of 1:1, 1:10, 1:100, 1:1000, respectively, and were tested in blood group typing and identification tests of forensic serology. The above spezified dilutions correspond with 5 microliters, 0.5 microliter, 0.05 microliter and 0.005 microliter of blood, respectively. The detection limit of the microspectrometric test for blood was the dilution 1:10, of the porphyrine test a dilution above 1:100, whereas the preliminary test for blood (peroxidase) succeeded always up to a dilution of 1:1000 and the species determination by the radial immunodiffusion test in agar gels succeeded in most cases op to a dilution of 1:1000. The detection limit of the anti-human globulin inhibition test was between the dilution steps 1:10 and 1:100 when non-titrated and undiluted anti-human globulin serum was used. Gc- and ABO-grouping were possible up to a dilution of 1:100 and were thus the most sensitive grouping systems. Phenotyping of the enzyme-systems and the Gm/Km-system usually required stains with considerably higher blood concentrations i.e. stains of undiluted blood.

ABO Blood-Group System

[Gm/Km determination in sperm traces].

In the forensic laboratories of the Federal Republic of Germany and West-Berlin 23 different semen stains and in our own laboratory 20 semen stains were typed in the gm/km-system doing 125 and 61 (own) test respectively. Examination was carried out by means of the haemagglutination method, which has been used successfully in typing bloodstains. Our critical assessment based on earlier experiences with semen stains was now confirmed and statistically evaluated: typing was successful in about 35-50% of the tests, but besides false-negative results, there was also a considerable percentage (4-10%) of false-positive ones. Therefore for the present it seems best to exclude the gm/km-typing of secretion stains from forensic investigations in order to avoid false incriminations or exonerations of suspects.
