In this well-detailed article, the author considers the diagnostic process phenomenologically (as it might be seen by an outsider observer) and goes on to consider the process from an aspect that is more interesting, more important, and much more difficult to understand: what goes on in a physician's mind as he makes a diagnosis.
Pneumocystis carinii is an endogenous liposomally modified mitochondrion.
Pneumocystis carinii was first described over 70 years ago but its taxonomy is still uncertain. Doubts have been expressed as to whether Pneumocystis carinii is an organism at all; but no coherent alternative has been proposed to account for the pathogenesis of pneumocystosis. Consideration of recent observations on Pneumocystis, liposomes, pulmonary surfactant and mitochondria leads to the hypothesis that Pneumocystis carinii is derived from liposomal change in the type II pneumocyte mitochondria that are known to be secreted into the alveoli under a variety of stressful stimuli. This hypothesis is considered to be more consistent with the facts than the hypothesis that Pneumocystis carinii is a protozoon or a fungus or any other kind of exogenous microorganism.

Osmotic error in erythrocyte volume determinations.
Because of osmotic effects erythrocytes suspended in their native plasma do not have the same volume as the same erythrocytes suspended in Isoton. The discrepancy varies depending upon the osmolality and composition of the native plasma and the length of time the cells have been suspended in Isoton. consequently, the MCV recorded in an electronic particle counter (Coulter in this case) may differ markedly from the true in vivo MCV. A similar error affects the Coulter hematocrit, which is calculated from the MCV and the erythrocyte count. This matrix effect should be taken into account in any laboratory quality assurance program.

A proposal for a department of pathology improvement program.
A quality assurance program for Departments of Pathology was first proposed by William Beautyman, MD, while he was a member of the Board of Governors of the College of American Pathologists. Subsequently, this proposal has been reviewed in detail by the CAP Clinical Pathology Committee under the chairmanship of William Cole, MD, and more recently Mario Werner, MD. At this time, the Clinical Pathology Committee believes a thorough discussion of the merits, format, and content of the program is desirable to bring the concept to maturation. Only after being subjected to widespread scrutiny will it be ready for more formal consideration and possible implementation by the College. It is published at this time with such intent.
Fatal massive hepatic necrosis from varicella-zoster hepatitis.
An unusual case of varicella-zoster hepatitis is reported which resulted in fatal massive hepatic necrosis in a 64-year old white female. The patient had had chickenpox 30 years prior to death but no cutaneous zoster at any time. The liver showed typical eosinophilic intranuclear inclusions and herpesvirus virions were demonstrated by electron microscopy. Indirect immunofluorescent serologic studies confirmed remote and recent infection by varicella-zoster virus. The potential significance of an incidental splenectomy performed 14 months before death and the general role of immunosuppression in the incidence and dissemination of varicella-zoster are discussed. The features of 13 reported cases of herpes simplex hepatitis are briefly reviewed.
