DES-related clear cell adenocarcinoma of the vagina in pregnancy.

The occurrence of diethylstilbestrol- (DES)-related clear cell adenocarcinoma of the vagina in a pregnant patient who carried the pregnancy to term is reported. The tumor, observed directly over an extended period, did not appear to be influenced by elevated plasma estrogen levels during pregnancy. Problems of diagnosis, histopathologic finding, and management are discussed.

Sacral agenesis: diagnosis, treatment and followup of urological complications.

Sacral agenesis is an uncommon congenital anomaly that usually is accompanied by other orthopedic anomalies but often occurs singularly. We report our experience in the treatment and long-term followup of 4 cases of pure sacral agenesis. We strongly stress the need for early diagnosis and long-term careful followup. In our group the diagnosis was often overlooked and followup was poor. Studies must be undertaken to determine the type and degree of neurogenic vesicopathy in each child since this may vary from one patient to the next. We urge the early and long-term use of intermittent catheterization.

Hydatidiform mole with coexistent fetus.

Eight cases of hydatidiform mole with coexistent fetus are presented with special considerations of diagnosis and management. Although the incidence of this condition was one in 22,000 pregnancies, it is emphasized that the combination of a mole with a fetus should be included in the differential diagnosis of patients who have signs and symptoms of gestational trophoblastic disease in the presence of a fetus. Recommended treatment is immediate termination of the pregnancy. Appropriate follow-up with a sensitive and specific gonadotropin assay is required to differentiate minimal tumor activity rom normal levels of pituitary luteinizing hormone. None of the patients in this series developed maligant trophoblastic neoplasia.

Monitor of chemotherapy in gestational trophoblastic neoplasm by radioimmunoassay of the beta-subunit of human chorionic gonadotropin.

Radioimmunoassays and bioassays based on the reactions of the native molecule of human chorionic gonadotropin (HCG) fail to differentiate HCG from pituitary luteinizing hormone (LH). An assay based on the beta-subunit of HCG which detects HCG exclusively has been used in our laboratory to monitor patients undergoing chemotherapy for gestational trophoblastic disease (GTD). We have been able to differentiate minimal, persisting tumor activity from normal levels of pituitary gonadotropins and have based therapy on these findings. Alternatively, treatment has been terminated when HCG is no longer detectable in the serum. Tumor activity has been detected in the beta-subunit assay at a time when biologic activity in the urine indicated remission.

Surgical versus medical treatment of occlusive disease confined to the left anterior descending coronary artery.

The results of saphenous vein bypass grafting and medical treatment were compared in 53 patients with stable angina pectoris, high grade occlusive disease confined to the left anterior descending coronary artery and normal or minimally impaired left ventricular function. Survival, incidence of myocardial infarction, relief of angina and response to exercise testing were evaluated. In the 29 surgically treated patients, followed up a mean of 24 months, there were two late deaths (7 percent) and five myocardial infarctions (17 percent). Twelve patients (41 percent) were free of angina and the majority had increased exercise performance when tested up to 18 months postoperatively. In the 24 medically treated patients, there were no deaths and one myocardial infarction (4 percent) in a mean follow-up period of 37 months. Six patients (25 percent) were free of angina. Less improvement in exercise performance was observed than in the surgically treated group. This subset of patients with isolated left anterior descending coronary artery disease has a favorable prognosis that is not enhanced by bypass grafting. Surgical treatment is more effective than medical treatment in relieving angina and improving exercise performance in the early years after coronary arteriography.
