[Experiences with intracapsular cataract operation in myopic patients].

This paper reports on postoperative results and follow-up in 74 eyes of 51 patients suffering from myopia of between -5 and over -20 D. They underwent intracapsular cataract surgery for presenile or senile cataract. The operations were performed under local anesthetic after oculopression; a microscope was used. Only in 2 cases was there a loss of vitreous during the operation. After a 5-year follow-up a defect in the anterior membrane of the vitreous was found in 7 eyes. Forty-one eyes attained a visual acuity of between 0.4 and 1.0. Careful intracapsular cataract extraction by a skilled surgeon is justifiable for myopic patients. Degenerations of the periphery of the retina were easy to coagulate with the laser after the operation.

[Results of sclera-laminating goniotrephination in neovascular glaucoma].

Sclera-laminating goniotrepanation is a modification of the Elliot-Fronimopoulos operation. In this procedure the scleral flap is removed, so that a shallow depression is formed in the sclera; this helps to prevent an ectatic bleb. No cicatrization of the trephine hole was observed in the authors' cases. The modification is only used by them in glaucoma cases with poor prognosis. Forty-six patients, 39 of whom were suffering from neovascular glaucoma associated with occlusion of the central retinal vein or diabetic retinopathy, underwent the operation and the results were followed up. As far as regulation of intraocular pressure and elimination of pain are concerned, the results were encouraging. Severe hemorrhage into the anterior chamber without spontaneous absorption only occurred in 2 cases.

[On the question of cutaneous changes associated with paraproteinemia. Light- and electron-microscopic study (author's transl)].

This report deals with a case of paraproteinemia (IgG Kappa) associated with skin indurations and yellowish papules on the trunk, edema of the face, and crystals in the cornea. In addition to the absence of elastic fibers and reductions of collagen fibers, light microscopy revealed single and/or multinucleated dermal cells with slight streaky patterning of the cytoplasm. These cells contained some sudanophilic (the greatest quantities of sudanophilic substances were observed in the deep corium) and partly birefringent material. In particular after warming, numerous spherocrystals appeared, which give Maltese cross birefringence. The presence of cholesterol esters can be assumed. The light-microscopic localization of acid phosphatase (incubation in Gomori medium) coincides in part with the distribution of bifringent lipids. Electron microscopy showed the cells with streaky contents to be chiefly histiocytes with numerous cristalline bodies in their cytoplasm. The cristalloids with a periodic inner structure are always surrounded by a single membrane. Identical bodies can be detected in endothelial cells of capillaries and epidermal keratinocytes. After pronase P incubation they disappear completely. We assume that the deposits correspond to phagocytized paraprotein. In addition, some histiocytic cells contain numerous lipid droplets in their cytoplasm. There is disintegration of some cells with crystalline inclusion, particularly in the deep corium. These cells show not only diffuse intracytoplasmic but also extracellular distribution of acid phosphatase activities. Particularly in the pericellular space of such cells numerous "banded structures" appear in connection with collagen degradation probably initiated by lysosomal enzymes. Amyloid could not be detected either light- or electron-microscopically.

[Measuring the breath of fusion in reduced monocular and binocular visual acuity (author's transl)].

The range of fusion was measured with the synoptophore with different objects in 100 students. The visual acuity of the tested persons was reduced monocularly and binocularly by fogging occluders. The range of fusion decreases more strongly in the case of monocularly reduced vision than in the case of binocularly reduced vision to the same degree. Therefore it is not the absolute value of the reduced vision which is responsible for the decrease in the range of fusion but rather the difference between the visual acuity of both eyes. This result is important for the therapy with fogging occluders, prism foils and penalisation methods.

[Keratoplasty in performed ulcus corneae (author's transl)].

The best therapy in cases of perforated ulcus corneae is the immediately performed perforating keratoplasty with fresh material as the following up of 7 patients shows. Also in cases of perforated metaherpetic ulcus a satisfying improvement of the vision over a long period of time is possible. The surgical technique is described.

[Postoperativ binocular function after full prismatic correction in patients with concomitant convergent strabismus and exuberance of convergence (author's transl)].

We investigated the cases of 50 non amblyopic children with concomitant convergent strabismus and exuberance of convergence, in which a prolonged prismatic correction was performed. 31 children had a 5 degree higher angle of squint in near vision before the application of prism. In 13 out of 31 the angle of squint, which was measured at the gaze for distance, increased more than 6 degrees. The ratio of normal retinal correspondence (NRC) to abnormal retinal correspondence (ARC) is 14:17. 19 patients developed only after prismatic correction of the angle of squint an exuberance of convergence. This phenomenon we call secundary exuberance of convergence. In this group the angle of squint for distance vision increased in only three children more than 6 degrees, while 6 children had a decrease by up to 11 degrees. The ratio NRC: ARC is 7:12. Postoperativ follow-up of binocular function over 4-5 years showed, that the prognosis in case of primary exuberance of convergence is worse than in secondary.
