Inhibition of erythropoiesis by plasma component(s) from sheep, goats, and rabbits.

Plasma from hypertransfused and normal sheep and experimentally induced anemic and normal goats was fractionated by ultrafiltration. Fractions obtained were assayed for erythropoiesis stimulatory factor (ESF) or erythropoiesis inhibitory factor (EIF) activity (or both) in the posthypoxic polycythemic mouse assay. The most potent sheep plasma-inhibitor fraction was found in the retentate on a membrane with a cutoff at mol wt 50,000. The most potent EIF fraction from anemic goats passed into the ultrafiltrate, but the comparable EIF from normal goats remained in the retentate on a membrane with a cutoff at mol wt 500. The yield of the most potent EIF fraction was higher from anemic goats than was the yield from normal goats. During thin-layer chromatography, EIF extracted from goat plasma fractions had the same mobility as did prostaglandin F2 alpha. Cohn rabbit plasma fraction IV-4 had an inhibitory factor, and a benzene extract of the fraction contained a component that had the same mobility as did prostaglandin F2 alpha. Cohn rabbit plasma fraction V contained a component that potentiated an erythropoietin-generating factor.

Reassessment of the diagnostic value of the vectorcardiogram in cystic fibrosis (correlation with clinical score, pulmonary function tests and echocardiogram).

In 21 children with cystic fibrosis, the vectorcardiogram (VCG) was correlated with various indices of the severity of the disease. The best correlation was found between (1) the azimuth angle of the spatial QRS loop area, representing the rightward and posterior shift of the main electrical forces, and pulmonary function tests (PFT): FEV1 % predicted (r = -0.671, P less than 0.005) and VC% predicted (r = -0.607, P less than 0.005), and (2) the right ventricular anterior wall index measured echocardiographically (r = 0.472, P less than 0.05). More VCG parameters correlated significantly with PFT than with the echocardiogram. This shows the major influence of pulmonary changes on the VCG of those patients. In several subgroups with different clinical scores, the VCG were analyzed and compared to normal limits of age- and sex-matched controls. In this small series, the VCG was a specific but not very sensitive method for predicting the degree of severity of the disease. Echocardiography and vectorcardiography can be considered as two complementary techniques in the evaluation of cystic fibrosis.

Residual right-to-left shunt following repair of atrial septal defect.

Information about 5 patients with residual right-to-left shunts following repair of an atrial septal defect is presented. In each patient the defect was located low in the atrial septum adjacent to the inferior vena cava. During operation, either the eustachian valve of the inferior vena cava had been mistaken for the lower margin of the defect or the lower portion of the defect was not closed; blood flow was then diverted from the inferior vena cava into the left atrium. To prevent this occurrence, the inferior margin of the atrial septal defect should be closed first.

Operative repair of atrial septal defect without cardiac catheterization.

Thirty-three children with secundum or sinus venosus atrial septal defects underwent operative correction at the University of Minnesota Hospitals between January, 1972, and December, 1973. Nineteen of these patients (58%) did not have cardiac catheterization as part of their preoperative evaluation, since clinical assessment of the child, with or without corroborative evidence obtained by echocardiography and radionuclide angiocardiography, provided sufficient information to permit recommendation of operation. In the majority of patients with uncomplicated atrial septal defect, cardiac catheterization is not a necessary part of the preoperative evaluation and may be reserved for those patients with unusual clinical or laboratory features.

Patent ductus arteriosus complicating respiratory distress syndrome.

PDA was diagnosed in 76 (19%) of 396 patients with RDS. Evidence of excessive pulmonary blood flow and CHF developed in 15 (20%). CHF was not recognized prior to 10 days of age in any patient. All 15 with CHF were initially managed medically. Seven improved; three died of other causes. Five patients who failed medical management were surgically treated; all survived operation, but only two were improved.

Inhibition of Friend virus (FVP)-induced erythropoiesis by an erythropoiesis-inhibitory factor (EIF).

An erythropoietin-independent murine erythroleukemia (FVp) has been used to evaluate the effects of an erythropoiesis-inhibitory factor (EIF) isolated from human urine. The consequent inhibition of FVp-induced erythropoiesis suggests that EIF exerts its effect independently of ESF. The inhibitory effect of EIF on FVp-induced erythropoiesis may reveal a potential for the physiological control of some types of erythroleukemia.

Response in mice to an erythropoietin-generating factor.

The bioassay for erythropoietin-stimulating factor (ESF) was compared in 3 strains of mice. One strain was made polycythemic by hypertransfusion and the other 2 by hypoxia. An erythropoiesis-generating factor (EGF) measured in the 3 strains generated approximately the same amounts of ESF, determined with log dose-log response curves for the respective techniques, after incubation with normal rabbit serum (NRS). The amount of ESF generated by EGF without prior incubation with NRS was also approximately the same in the 3 strains of mice.
