Prostacyclin administration during cardiopulmonary bypass in man.

In a double-blind trial prostacyclin was given to 12 of 23 patients undergoing coronary vein grafting. The treatment group showed a preservation of platelet number and function, with a halving of blood loss in the first 18 h postoperatively. The heparin sparing effect of prostacyclin was confirmed, as were the vasodilator effects which were not troublesome.

An assay for erythrocyte chalone using chick embryo red blood cells.

As mature mammalian erythrocytes do not contain nuclei, erythrocyte chalone has previously been detected by its inhibitory effect on normoblasts, often obtained from a mixed cell source. Chick embryo blood consists entirely of mature nucleated red cells and was used to test the effect of heterologous erythrocytic extracts on haemopoiesis. The model was adapted to detect the presence of inhibitors of erythropoiesis as contaminants of partially purified leucocyte extracts.

Primary adenocarcinoma of bladder.

Since 1959, 10 patients with primary adenocarcinoma of the bladder were treated at the Virginia Mason Medical Center for an incidence of 1.5 per cent of all bladder cancers. This retrospective study shows that histologic grade and tumor location had a variable influence on survival whereas clinical stage appeared to have a direct effect on survival. The only long-term survivors were the 4 low-stage patients who underwent radical cystectomy.

Granulocyte chalone assayed in vivo in the mouse.

An in vivo system has been developed for estimating the inhibitory activities of granulocyte chalone which overcomes the objection to the use of mixed proliferating target cells. It depends on the identification of labelled mature granulocytes in the peripheral blood of mice that received a radioactive precursor of DNA synthesis, with and without granulocyte extracts, several days previously. Two extracts of bovine granulocytes were tested. The first demonstrated that maximum inhibition was achived when it was given 2 h before the isotope, suggesting that its activity was short lived. It also failed to depress the labelling index of peripheral lymphocytes, thereby demonstrating tissue specificity. The second was used to show that the dose-response relationship was exponential.

Urolithiasis associated with pregnancy.

Of 34,081 deliveries in a 12-year period 20 were complicated by urinary calculi during the pregnancy. Of the 20 deliveries 3 required intervention either during pregnancy or immediately postpartum. There were no maternal deaths and only 1 stillborn secondary to erythroblastosis. Analysis of symptoms, findings, procedures and outcome is presented in this uncommon but serious problem.

Severe prostatic calcification after radiation therapy for cancer.

Severe symptomatic prostatic calcification was seen in 3 patients who had carcinoma of the prostate treated initially with transurethral resection, followed in 2 to 4 weeks by definitive radiation therapy. This complication is probably preventable if an interval of 6 weeks is allowed between transurethral resection of the prostate and radiation therapy.

Granulocytic chalone is not a polyamine complex.

Recent studies have indicated that tissue extracts may contain sufficient amounts of free polyamines to account for the inhibitory effect of the extracts and hence mask any authentic chalone action. It has also been reported that bound polyamine may be the active moiety of some chalones. We show here that in our granulocytic and lymphocytic chalones polyamines, either free or bound, are not responsible for the inhibitory effects of the preparation.

Long-term ureteral replacement prosthesis.

A silicone rubber tube with bonded Dacron felt cuffs have been implanted in dogs as a ureteral replacement prosthesis. A one-way valve has also been developed to prevent reflux so the prosthesis may be put into the bladder. The prosthesis has been implanted for periods up to nine months. Although there are some problems associated with the prosthesis, most animals have maintained a normal serum creatinine, sterile urine, and a normal excretory urogram.

The effectiveness of in vivo root planing in removing bacterial endotoxin from the roots of periodontally involved teeth.

In this study, scaling alone resulted in endotoxin values considerably greater than the values for healthy root surfaces. However, the root-planed Samples contained only about 1 ng more of endotoxin than did the healthy root surfaces. This small difference can be accounted for by the presence of small flecks of calculus left after root planing. Considering that out of a total sample size of 48 surfaces there was only 1 ng difference in the amount of endotoxin between planed teeth and uninvolved teeth, the basic conclusion must be that root planing, as performed in this study, was able to render diseased root surfaces approximately as free of detectable endotoxin as were uninvolved, healthy root surfaces of unerupted teeth.

Effect of granulocytic chalone on acute myeloid leukaemia in man. A follow-up study.

Non-pyrogenic preparations of granulocytic chalone have been administered to patient with acute myeloid leukaemia (A.M.L.) who had relapsed after chalone-induced complete remission of the disease. Another regression of the leukaemia was obtained. Normal and leukaemic granulocytes were inhibited without effect on other cell types. Survival was prolonged in 4 other patients with A.M.L. Further investigations are needed to elucidate the therapeutic role of granulocytic chalone.

Effect of granulocyte chalone on acute and chronic granulocytic leukaemia in man. Report of seven cases.

This study reports results from the first clinical tests in which 7 patients with myeloid leukaemia (5 acute and 2 chronic leukaemias in metamorphosis) were treated from 4 to 45 days with granulocyte chalone, the tissue-specific endogenous inhibitor of granulopoiesis. It was observed that i.v. injection of partially purified chalone inhibits proliferation of leukaemic, and presumably also normal granulocytic cells, leaving all other cell types unaffected. Inhibition of leukaemic growth was distinct in 6 of the 7 patients; in 5 cases the inhibition was followed by acutal regression of the leukaemia, lasting up to several months in the absence of any maintenance therapy, and in one case the treatment led to a complete remission. Chalone treatment also resulted in an enhancement of erythropoiesis and megakaryopoiesis, and in phenomena some of which were totally unexpected, such as immunostimulation and a remarkable resistance to bacterial infections in the presence of extreme granulocytopenia. This study shows that granulocyte chalone is biologically active against myeloid leukaemia in man, but not that the therapeutic value of the impure chalone is superior to modern cytostatic drugs. Long-term therapeutic trials were not possible with the partially purified preparations available, mainly because of side-effects which prevented adequate dosing.
