[Incidence of lung embolism before and after thrombectomy of deep leg and pelvic vein thrombosis].

According to scintigraphic and pathological studies, pelvic and thigh venous thrombosis led to pulmonary embolism in 50-60% of all cases. After thrombectomy, an incidence of 1% is reported, when assessed only clinically. In 29 patients with acute pelvic or thigh venous thrombosis, perfusion scintigraphic lung scans were performed immediately before operation and on day 1 or day 2 postoperatively. In 7 patients (24.1%) preoperatively and 12 patients (41.4%) postoperatively, segmental perfusion defects indicating pulmonary embolism were demonstrated. Thus the incidence of pulmonary embolism according to thrombectomy is much higher than previously suspected.

[The thrombin-antithrombin III complex ELISA test in diagnosis of surgery-induced blood coagulation activation].

In addition to measuring the routine blood coagulation parameters (Quick, PTT, TT, AT III, Fibrinogen, PLT), the Thrombin-Antithrombin III Complex (TAT) Test was performed preoperatively on 20 patients treated because of nodular goiter in the euthyreoid state and ten patients with symptomatic cholecystolithiasis. While there were no changes in the routine blood coagulation parameters, significant alterations of the TAT occurred in both patient groups. This indicates that TAT makes operation-induced activation of the blood coagulation system, which is impossible by just measuring the routine blood coagulation parameters alone, detectable.

[Disorders of microcirculation in colon anastomoses and their significance for the pathogenesis of suture dehiscence].

In this morphologic experimental study in the rabbit disturbances in the microcirculation at the site of colonic anastomoses were found in all suture techniques employed. They are caused by resection, intraoperative traumatization, the suture technique, and, secondarily, by abscess formation on the anastomosis. The suture-induced disturbances of the microcirculation lead to necroses of the mucosa, partial and complete necroses of the inverted cuff, and transmural necroses with destruction of the sero-serous contact zone. For the pathogenesis of anastomotic dehiscence only transmural and extensive complete necroses of the anastomotic bulge, including the serosal contact, are important. They result in transmural abscess formations which correspond to microscopic small secondary anastomotic leakages.

[Significance of sonography of the breast for surgical treatment of breast cancer].

In a prospective study of 119 patients with breast carcinoma the value of ultrasonography for surgical therapy was compared with the value of clinical results and mammography. The tumor localization (in 96.8%) and the exact preoperative staging following the pTNM-system (tumor size in 85.8%, lymph node involvement in 77.2%) were better predicted by ultrasonography than by mammography and clinical examination. Especially the multicentric-multifocal carcinomas (20 of 29 cases) were better detected. Preoperative ultrasonography especially improves planning a conservative management of breast cancer.

[Perioperative ultrasound examination of the breast in breast cancer].

127 cases of breast carcinoma were examined in a prospective study. By means of preoperative ultrasonography the staging following the pTNM-system validates the tumor size in 85.5% and the lymph node involvement in 77.2%. Multicentric-multifocal carcinomas were diagnosed by ultrasonography in 20 of 29 cases, merely 9 of 29 by mammography. Ultrasonography of specimen is recommended in cases of negative clinical findings and positive ultrasound examination to verify tumour excision. In addition to clinical exploration and mammography the ultrasonography of the breast obtains a diagnostic significance for planning a conservative management of breast cancer.

[Acute abdomen in eosinophilic colitis].

Eosinophilic colitis represents a seldom known syndrome characterized by peripheral blood eosinophilia, eosinophilic infiltrates of the gut and abdominal pain. Whereas an association with autoimmune disorders or with food allergy is described, the etiology of the disease remains uncertain. A case report, where the spontaneous benign course of the disease is shown, is presented and discussed. In the clinical practice it is important to evaluate this syndrome in the differential diagnosis of abdominal pain and peripheral eosinophilia in order to avoid unnecessary therapeutic measures.

[The prognostic relevance of tumor localization in colorectal cancer. A multivariate analysis].

Records of 292 patients with colorectal cancer, operated at St.-Josef-Hospital between 1973 and 1984 were studied using Cox regression model to elucidate the prognostic influence of tumor site. Neither in univariate nor in multivariate analysis any significant independent influence of tumor site on prognosis could be detected. Male sex, older age and more advanced tumor stage were independently associated with poorer prognosis.

[Acute postoperative psychosyndromes. A prospective study and multivariate analysis of risk factors].

The incidence of postoperative delirium was assessed in 92 patients on a general surgical intensive care unit. Postoperative delirium was diagnosed in 39 patients (42%). Most of the cases were diagnosed on the second postoperative day and the median duration was seven days. Ten patients with delirium (25%) had a lethal outcome, compared to a 13% mortality of the whole population. In univariate analysis the variables age, preoperative therapy for heart failure, respirator therapy, dobutamin therapy and lowest capillary pO2 on day one were significantly associated with later development of delirium, whereas preoperative peritonitis, and history of stroke or hypertension were only borderline significant. A predictive model with three parameters emerged from multiple logistic regression analysis: after correction for age (p = 0.001), respirator therapy (p = 0.020), and capillary pO2 on day one (p = 0.049) none of the remaining variables proved of additional significance. The statistical model yielded a predictive accuracy of 78%.

[Decision aids in determining therapy of breast cancer by preoperative staging using sonography and computerized tomography].

In a prospective study the preoperative ultrasonography of 54 cases of carcinoma of the breast was able to predict the pTNM staging precisely in 83.3% for tumor size and in 72.3% for lymphnode metastases. Multicentric-multifocal carcinomas were diagnosed by ultrasonography in 10 of 13 patients, better than by mammography (6 of 13). The CAT scan is indicated only in advanced tumor stages. The preoperative staging by ultrasonography enables with more precision planning and differentiated therapy of the carcinoma of the breast, especially in view of breast-preserving therapy.

[The significance of preoperative and postoperative sonography for biliary tract surgery. A prospective study].

112 patients with biliary calculi and destined for cholecystectomy were sonographically investigated in a prospective study carried immediately before the operation as well as between the third and fifth, and between the seventh and ninth postoperative day. Biliary calculi were sonographically detectable in 111 of the 112 patients (sensitivity 0.99). The sonographically measured bile duct diameter agreed well with the actual bile duct diameter (r = 0.898). There was a correlation between the bile duct diameter and the severity of the underlying biliary disease. A sonographic diagnosis was successful in 11 of 17 patients with choledocholithiasis (sensitivity 0.65) whereas 10 false positive diagnoses gave a specificity of 0.88. Gallbladder wall thickness above 4 mm had a sensitivity of 0.9 and a specificity of 0.72 for the detection of severe chronic or acute cholecystitis. A high rate (50%) of postoperative subhepatic fluid collections was a conspicuous finding but these were predominantly transient or only short-lived and attained clinical significance in only 7 out of 108 patients (6.5%).

[Objective criteria for the evaluation of small intestine function in postoperative ileus].

A continuous intrajejunal open-tip pressure determination was performed via catheter jejunostomy after major surgery on the upper gastrointestinal tract parallel to early postoperative enteral nutrition. In 15 patients, who tolerated enteral nutrition well intrajejunal pressure was about 10 mm Hg. In 7 patients with a rise in pressure up to 24 mm Hg complications occurred. From the 7th postoperative day all pressure values were below 15 mm Hg and enteral nutrition was tolerated well. It is concluded that till the 7th postoperative day small bowel function is individually different.

[Modified partial duodenopancreatectomy with preservation of the stomach, proximal gastric vagotomy and pancreatic duct occlusion].

From 1979 till 1984 a modified partial duodenopancreatectomy was performed in 22 patients with preservation of the stomach, proximal gastric vagotomy, and occlusion of the pancreatic duct. If occlusion of the duct was performed without a pancreatojejunostomy 6 of 12 patients developed a spontaneously healing pancreatic fistula. In 10 patients with duct occlusion and a pancreatojejunostomy no fistulae were observed. Exocrine and, if necessary, endocrine pancreatic insufficiency could be compensated sufficiently medically and by diet. In comparison to the original Whipple's operation preservation of the stomach seems to influence favourably the body weight and thus the quality of life. The proximal gastric vagotomy is performed to prevent anastomic ulcers.
