[Endometrial cancer: a current therapeutic and diagnostic protocol].

In the presented methodical scheme, consecutively and systematically are scrutinised the epidemiology, the risk factors, the etiopathogenesis, the histopathology and the clinical signs of the endometrial cancer. The diagnostic possibilities, as well as the staging principles are pointed. The current treatment tactics, therapeutic schemes according to the stage and follow-up corresponding to the requirements of the oncological doctrine are presented.

[Effectiveness of cervical screening - expectation and reality].

The aim of the present study was to establish the effectiveness of the conducted in Bulgaria cervical screening program for the period 1975-1996. The dynamics of the ratio invasive cancer/in situ cancer, as well as the share of the diagnosed from cytologically signalized women were analyzed. The tendency of deterioration of each of the indices was evident. The crude incidence varies between 12.7 and 22.7 x 10(5) women, while the frequency of CIS is from 3.2 and 6.2 x 10(5) women. This defines unfavorable ratio of the cases with invasive cancer toward CIS, which varies from 2:1 for 1982 to more than 5:1 for 1996 year. The study demonstrates the prevalence of a low rate of confirmed by biopsy, cytologically signalized women--from 15.6% and 30% (total for country--33.3%), except DOD Blagoevgrad--50.6%, Plovdiv--46.4% and Sofia-city--63.8%. The results of our study demonstrate the low effectiveness of to date conducted cervical screening program. The lake of a screening register is another weakness of the program, which lead to a deteriorated registration and reading of the detected precancer lesions, as well as, their diagnostic follow and therapy.

[Cancer of the vulva: modern diagnostic and therapeutic principles].

In the presented methodical sceme, consicutively and systematically are scrutinized the epidemiology. The risk factors, the etiopathogenesis, the hostopathology and the clinical signs of the vulvar cancer. The diagnostic possibilities, as well as the staging principles are pointed. The current treatment tactics, therapeutic schemes according to the stage and follow-up corresponding to the requirements of the oncological doctrine are present.

[The screening and diagnosis of ovarian cancer].

The ovarian cancer represents important medico-social problem for our country due to its significant frequency (26.1%) and high mortality (9/1000) among the oncogynecological diseases. The aim of the present review was to discuss the methods--for diagnostics of the ovarian cancer trough analyzing the possibilities for their implementation as a screening in the clinical practice. It is pointed out, that the secondary prophylaxis for ovarian cancer has its place as a selective screening for women with high risk. The proposed scheme includes: gynecological and ultrasound checks up and CA-125 investigation every two year after twenty-five ages of the woman.

[Cervical screening in Bulgaria. An analysis of the basic indices over the period 1970-1994].

The aim of the present study was to analyze the results from the cervical screening in Bulgaria for the period 1970-1994, by studying the following indicators: incidence, mortality, frequency of carcinoma in situ and stage distribution of the cases with invasive cervical cancer. The relatively constant incidence and mortality until 1987 were evident, after that they shoved increasing and for 1994 they are 19.7% and 7.6% respectively. The similar is the data for the correlation carcinoma in situ/invasive cancer and for the stage distribution of the cervical cancer patients. The obtained results showed, that cervical cancer remain medico-social problem for our country, because the introduced in 1970 cervical cytological screening at the moment is not effective enough.

[Cytological and histological parallels in atypical colposcopic findings].

The aim of the present study was to analyse the cytological and histological equivalents in 112 patients with atypical colposcopical findings. The results showed the follow distribution of the findings: for cytological examination in 66 of the patients-I gr. PAP, in 32-II gr. PAP, in 6 III-A gr. PAP, in 1-III-B gr. PAP, in 4-IV gr. PAP and in 3-V gr. PAP. The histological study of the obtained target biopsies established: 7 cases with normal epithelium, 82 with different findings of abnormal epithelium with or without nonspecific and specific inflammatory changes; 18 cases with dysplasia from I to III degrees, 2 cases with CIS and 3 with invasive cancer. The degree of correlation of the different findings, as well as the possible causes for hypo and hyperdiagnostic assessments are discussed.

[The treatment of vaginal infections with Macmiror and Macmiror Complex].

UNLABELLED: The aim of the present study was to examine the therapeutic possibilities of the wide-spectrum medicament Macmiror & Macmiror Complex for the treatment of the vaginal infections. MATERIALS AND METHODS: The study included 52 nonpregnant women among 20 u 54 years (middle age 30.5) with different by kind and intensity colpitis complains. The following microbiological characteristic was established: in 26 cases Candida spp., in 19 Gardnerella vaginalis, in 1 Trichomonas vaginalis and in the rest 6 mixed infection. The treatment of the patients was done in combined scheme: peroral and vaginal administration simultaneously with local treatment of the partner. The control examination was performed bistagely: on 7-10 day and on 30-40 day. RESULTS: The good clinical and microbiological influence of the treated patients was established for the first control examination the effect was found in 89.5% u 84.2% go of the cases with bacterial vaginosis and in 84.6% u 69.2% in those with mycotic colpitis. Relatively high values were found also at the second control, respectively 83.3% and 72.2% for the amine colpitis and 69.5% and 52.2% for vaginal candidosis. The similar favorable influence received also in the rest of the patients. CONCLUSIONS: Our results do show good possibilities of the nifuratel and nystatin combination (Macmiror & Macmiror complex) to influence vaginitis with different etiology, which give us founding to consider, that the drug is suitable for the mixed forms of vaginal infection.

[Does cytological screening have a place in sexually active teenagers?].

UNLABELLED: The aim of the present investigation is to determine what is the place of cytological screening of the uterine cervix in sexually active teenagers. MATERIAL AND METHODS: The study was based on 83 girls aged between 14 and 19 (mean age 18.13 years) without childbirth in history, who visited our clinic with non-specific gynecological complaints. All patients were submitted to brush cytology applying the classical method and classifying the smears according to the Bethesda system (1991). RESULTS AND DISCUSSION: A high adequacy of the cytologic smears was found, reaching up to 98.8% and the following distribution of the findings according to the various criteria was observed: cells within normal limits in 34.1% of the patients, benign cellular changes due to reactive changes associated with inflammation in 34.1%, atypical epithelial cell abnormalities of undetermined significance in 18.4%, low grade SIL with HPV in 11.0%, without HPV in 1.2% and high grade SIL in 1.2% of thr patients. The role of cytological screening in sexually active adolescent girls is discussed in the context of the obtained results.

[Epithelial changes in the cervix uteri in pregnant women and in cervical oncogenesis].

The changes in the growth and differentiation of the cervical epithelium induced during pregnancy by hormonal influence and anatomical alterations are discussed regarding their magnitude on cervical oncogenesis. Question about the necessity of the self-sufficient centralized reporting of the pregnant women with precancer and cancer lesions is put on.

[Sexual activity, contraception and abortions among teenagers in Bulgaria].

Sexual activity in adolescence, the pregnancies, abortions and deliveries, STDs and the decisive role of the information about sex and sexuality are questions with great importance nowadays. The aim of present study is to find the changes in the sexual activity among the adolescents, in their motivation in beginning the sexual life, information that they have about sex and sexuality, in unwanted pregnancies, deliveries and about abortions in the last five years, following our former study. We present the data obtained from an inquire of 432 schoolboys and girls (aged 17-18), from 263 girls, aged 13 to 18, admitted for interruption of unwanted pregnancy in the Univ. Ob. & Gyn. Hospital in Sofia, Bulgaria, as well as from 132 girls after delivery in adolescence.

[The therapeutic potentials of the vaginal antimicrobial preparation Chlorchinaldin].

The aim of this study is the clinical testing of the vaginal wide-range, antimicrobial, quinolone--Chlorchinaldin/0,2/by POLFA, which has a strong antibacterial, antifungal, trichomonal and keratoplastic effect. The medicine was used on 43 patients with complaints of aggravated fluor. The clinical, colposcopic and microbiologic study showed: candidiasis--in 16 trichomoniasis--in 8, bacterial vaginalis--in 3 and anaerobic vaginitis--in 2 patients. The rest of the women (14) had various aerobic microbial findings. The treatment was daily using one vaginal tablet, every night for 10 days. A control examination was carried out one week after the any of therapy. The results showed a positive clinical effect, since in 67.4% there were negative microbiological findings. The lack of effect in cases with bacterial vaginosis and anaerobic vaginitis was painted out. These results give reason to believe that the wide antimicrobial range of Chlorchinaldin B will be complementary to the currently used drugs in the treatment of vaginal infections.

[Cervicographic screening of women in early pregnancy].

Ninety six patients in early pregnancy were subjected to cervicographical screening. The follow findings were obtained: 50 (52,1%) negative cervicograms, 44 (45,8%)--suspicious and 2 (2,1%) with technical defects. The accuracy of the investigation was verified through colposcopy and target biopsy. A sensitivity of 59,1%; specificity of 77,5%; positive predictive value of 44,8%; negative predictive value of 85,9% and effectiveness of 73,1% were established. The applicability, possibilities and limitations of the method are discussed. Adapted structured cervicography report form according Stafl's criteria is proposed for the practice.

[A parallel comparison between cytological and cervicographic screening in early pregnancy].

The aim of this study is to compare the diagnostic possibilities of cytologic and cervicographic screening of the cervix uterus in patients with early pregnancy. Fifty-four patients between the ages of 20 and 37 were examined cytologically and cervicographically in a prospective screening study. The dependability of each of the diagnostic tests was measured histologically (with the patient's approval) using punch biopsy under colposcopic control. The histologic results showed the presence of CIN in 9 women (16.7%), 8 of them with CIN I and one with CIN III. The rest of examined patients (45) had normal of various inflammatory findings. The analysis of the obtained results confirm a statistically significant difference between the two diagnostic, screening methods. The cervicographic examination is considerably more sensitive in finding CIN-44.4% compared to 22.2% of the cytology. Whereas the cytological method has a higher specificity as a test in determining the healthy patients-91.1% versus 51.2% of the cervicography. 12 or 22.2% of the cervicograms were technically defective.

[The use of the Bethesda-terminology cytological system in clinical practice. A pilot study].

The aim of the present study is to summarize the one-year experience in applying the Bethesda terminological system (BTS) for assessment of cervical cytology. The study was based on 164 women, aged from 18 to 45, of which 98 with early pregnancy and 66 not pregnant. The materials for cytological examination were prepared by classical techniques and then coloured with hemalaun-eosin. Cytological findings were interpreted according to the revised BTS (1991), and an assessment of the adequacy of smears, possible infectious and reactive changes was made. Intraepithelial lesions were classified in two categories--low and high grade lesions. The advantages and shortcomings of the proposed new terminological, cytological classification system for assessment of the uterine cervix are discussed.

[The colposcopic, cytological and histological changes in the cervix uteri of women under 20 with a first pregnancy].

The study includes 42 pregnant women chosen at random under the age of 20, in their first trimester presented for voluntary termination of pregnancy. Colposcopic, cytologic and histological test was performed. Chronic, often acute, non-specific inflammatory changes were observed, probably connected with early sex life. The establishing of a histologically atypical results in 33.3% of the cases as well as the observation of 21.7% falsely negative results from the colposcopic study support the theory of pregnancy as a factor causing complication for diagnosis. The relatively low sensitivity of the cytology method and 16.7% false negative results none the less the existence of high specificity (100%) is due to coexistence of low-stage dysplasia (I and II stage) and chronic non-specific inflammatory processes and the early, pilot character of the research.
