Measurement of pulmonary erythromycin concentration in patients with lobar pneumonia by means of positron tomography.

The concentration achieved in the infected tissue is of fundamental importance in the use of antibiotics. Erythromycin labelled with the positron-emitting nuclide carbon-11 and positron tomography were used to compare local concentrations of this antibiotic in the pneumonic and unaffected lungs of five patients with lobar pneumonia. The time course of uptake of erythromycin into the extravascular compartment (i.e., the intracellular, interstitial, and alveolar compartments) of the pneumonic lung was measured. The mean extravascular concentrations obtained during the first hour after an intravenous injection of 270 mg erythromycin lactobionate were similar in the pneumonic lung and the unaffected lung (5.5 +/- 2.2 and 6.6 +/- 2.2 micrograms/g, respectively). An effective concentration of erythromycin was reached in the pneumonic lung within 10 min of the injection and was maintained throughout the period of measurement (60 min).

Preparation of [1-11C]acetate--an agent for the study of myocardial metabolism by positron emission tomography.

A method of obtaining an injectable solution of acetate labelled in the carboxyl group with the short-lived positron-emitting radionuclide, 11C (t12 = 20.4 min), is described. In the method labelling is achieved via the carbonation of freshly prepared methylmagnesium bromide with 11C-labelled carbon dioxide produced by the 14N(p, alpha)11C nuclear reaction. The method is fast (20 min) and produces sterile, apyrogenic [1-11C]acetate in high radiochemical yield (72 +/- 12%) and in high specific activity (greater than 18.5 GBq/mumol: greater than 0.5 Ci/mumol). The radiochemical purity of the radiopharmaceutical was found to exceed 95% by thin layer and high pressure liquid chromatography. Evidence presented shows that [1-11C]acetate has considerable value as an agent for investigating myocardial metabolism by positron emission tomography.

Active water-insoluble derivatives of papain and other enzymes based on preformed diazonium-type supports.

Papain (EC 3.4.22.2) has been coupled to supports of titanium (IV) oxide and cellulose, which are particulate and pre-coated with diazotised 1,3-diaminobenzene, giving water-insoluble and stable derivatives which possess low proteolytic activity but high esterolytic activity. In addition the reversible binding of zinc (II) at the active site of papain has been exploited to inhibit protectively the enzyme during its linkage to the aforementioned supports, thereby yielding water-insoluble derivatives of papain having superior activity upon reactivation with EDTA. Application of the improved procedure of enzyme coupling to macroporous cellulose particles gave a water-insoluble derivative of papain having further enhanced proteolytic activity. Other properties of the water-insoluble derivatives of papain and of similarly prepared water-insoluble conjugates of urease (EC 3.5.1.5) and cholinesterase (EC 3.1.1.8) with cellulose are also reported.
