Protein S-100 immunostaining as a diagnostic tool for DMBA-induced melanotic lesions.

In the present study we demonstrate immunohistochemically the presence of S-100 protein in numerous melanotic and amelanotic lesions (benign, premalignant and malignant) induced in albino guinea-pigs on topical application of the carcinogen. Positive staining was also present in all their amelanotic metastases. We reiterate the value of this stain in the diagnosis of melanomas, in particular amelanotic melanomas.

Isolation and functional characterization of the von Willebrand factor-binding domain located between residues His1-Arg293 of the alpha-chain of glycoprotein Ib.

In the present report we describe the isolation of a functional domain of platelet membrane glycoprotein (GP) Ib which retains von Willebrand factor (vWF)-binding activity. Glycocalicin, a proteolytic fragment of the alpha-chain of GP Ib generated by an endogenous calcium-activated protease, was submitted to digestion with trypsin. The two resulting fragments, one of 45 kDa extending between residues His1 and Arg293 and representing the amino terminus of the alpha-chain, the other of 84 kDa corresponding to the previously described macroglycopeptide, were purified to homogeneity. Glycocalicin, as well as the 45- and 84-kDa fragments, inhibited the ristocetin-dependent binding of native vWF to platelet GP Ib. The concentration inhibiting 50% of binding (IC50) was between 1 and 5 microM with all these molecules. In contrast, the binding of asialo-vWF to platelet GP Ib, measured directly in the absence of ristocetin, was blocked by glycocalicin and the 45-kDa fragment with a similar IC50, but not by the 84-kDa fragment. Both glycocalicin and the 45-kDa fragment bound to purified surface-bound vWF in a ristocetin-dependent manner and with similar affinities. Monoclonal antibodies against vWF or GP Ib inhibited this interaction in a way consistent with their inhibition of vWF binding to platelet GP Ib. These studies demonstrate that the amino-terminal extracytoplasmic region of the alpha-chain, extending between residues 1 and 293, contains a functional domain that interacts with vWF in the absence of any other structure of the GP Ib complex or any other platelet membrane component. Whereas the ristocetin-dependent binding of vWF may involve also other domains in the macroglycopeptide region, the direct vWF-GP Ib interaction appears to be mediated only by a domain in the amino-terminal region of GP Ib alpha.

Epithelial cutaneous lesions induced in Dunkin-Hartley albino guinea-pigs by means of 7,12-dimethyl-benzanthracene.

We carried out a clinicopathological study of epithelial cutaneous lesions induced in Dunkin-Hartley albino guinea-pigs by means of the topical application of 7,12-dimethyl-benzanracene. By the end of the study we had observed 4451 lesions. Most frequently observed were epithelial hyperplasia and dysplasia (2544 lesions). Neoplasias consisted of 244 carcinomas in situ, and 88 squamous carcinomas. Basal-cell epitheliomas were few. Epidermal cysts were a frequent finding.

Melanocytic carcinogenesis in albino guinea pigs.

We describe an experimental model for the induction of cutaneous melanomas in albino guinea pigs by means of repeated topical application of 7,12-dimethyl-benzanthracene over 13 consecutive months. At the end of the experiment 87.5% of the animals presented multiple lesions of a melanocytic nature. The most frequent and the earliest detected was melanocytic hyperplasia, followed by dysplasia. These lesions occurred in 75% of animals and were observed from the 4th month onward. Nevus-type lesions appeared from the 12th month and affected 70% of animals, whilst melanomas occurred in 65%. Metastases were observed in 45% of animals, in lungs, lymph nodes, kidney, and adrenal gland.

[Molluscum contagiosum. Clinicopathologic and ultrastructural study].

26 usually multiple Molluscum contagiosum (MC) lesions corresponding to 14 patients (x age, 12 years old) mainly located in face and body, and unfrequently in genital area, were studied. Under light microscopy, we emphasize the presence of molluscum bodies corresponding ultrastructurally to large bodies of viral particles within a protein matrix. Intranuclear inclusions of suspected viral origin were noted too. Lesions previously embedded in paraffin showed a perfect preservation of their viral particles.

Changes of contact phase proteins during haemodialysis.

Functional and immunological activities of factors XII and XI, Prekallikrein, high molecular weight kininogen and C-1-esterase inhibitor were studied in eight patients before, during and after haemodialysis. During and after dialysis sessions a significant decrease in the functional activities of Prekallikrein and high molecular weight kininogen (p less than 0.05) could be detected, whereas their immunological levels did not change. Furthermore C-1-esterase inhibitor increased after dialysis (p less than 0.05). These results indicate activation of the kinin system during haemodialysis.

Intravenous and subcutaneous administration of desmopressin (DDAVP) to hemophiliacs: pharmacokinetics and factor VIII responses.

When desmopressin (DDAVP) is given to mild and moderate hemophiliacs intravenously (i.v.) or subcutaneously (s.c.), there is a very large between-patient variability for peak levels of factor VIII coagulant activity (VIII:C). To evaluate whether or not between-patient variability is related to DDAVP levels achieved in plasma, we measured drug levels in 14 hemophilic volunteers (VIII:C 2 to 31 U/dL) who were randomly given 0.3 micrograms/Kg of i.v. or s.c. DDAVP and crossed-over to the other treatment after an interval of 15-30 days. Peak DDAVP levels (Cmax) were higher for i.v. DDAVP (p less than 0.02), times to peak levels (tmax) were shorter for i.v. DDAVP (p less than 0.001). There was no difference between the i.v. and s.c. routes for plasma DDAVP time curve (AUC) and half-life (t 1/2), but there was much larger variability for pharmacokinetic parameters with i.v. than with s.c. DDAVP. Post-DDAVP VIII:C increased 3.4 +/- 1.6 fold (i.v.) and 3.3 +/- 1.3 fold (s.c.) over baseline levels, with no significant correlation between peak VIII:C and DDAVP levels for either route of administration. These findings establish the s.c. route of DDAVP administration to be bioequivalent in effect to the i.v. route, albeit with less variability. At the DDAVP dosage used in this study and currently recommended for therapy, the VIII:C response is neither a function of the rate of absorption of the compound nor of the magnitude of its plasma concentration.

Normal pregnancy in a patient with a postpartum factor VIII inhibitor.

A patient who had developed a VIII:C IgG-Kappa antibody after her first delivery became pregnant for a second time. At that time the titer of the inhibitor had increased fourfold, remaining at high levels throughout the rest of the pregnancy. The neonate exhibited an inhibitor with characteristics identical to that of the mother which could not be detected 3 months after birth. No hemorrhagic complications in either of the subjects were observed.

A cytologic study of the sediments in pericardial fluid as it relates to a diagnosis of the mechanism of death.

We have studied the sediments obtained from the pericardial fluids of 70 cadavers subject to different causes of death. The fluids were taken in the course of corresponding legal autopsies. The samples were organized according to the cause of death and cellular predominance, in the following groups, respectively: hanging, multiple trauma, craniocerebral trauma, other violent deaths, myocardial infarction, pulmonary embolism, and other natural deaths. According to cell type followed these categories: Group 1 (isolated mesothelial cells), group 2 (isolated and plated mesothelial cells), group 3 (inflammatory cells and mesothelial cells, isolated and plated), and group 4 (inflammatory cells and mesothelial cells). The statistical analysis was attained through Pearson's coefficient. We have found a significant statistical relation (P less than or equal to 0.05) between the presence or lack of inflammatory cells and the mechanism of death. In those cases with a short survival period and without cardiac affectation, the presence of inflammatory cells was practically null. Furthermore, differences in the cross-sections of inflammatory cells reflected the duration of the death process, with elements characteristic of acute inflammation revealing acute cardiac process.

Hereditary thrombophilia: identification of nonsense and missense mutations in the protein C gene.

The structure of the gene for protein C, an anticoagulant serine protease, was analyzed in 29 unrelated patients with hereditary thrombophilia and protein C deficiency. Gene deletion(s) or gross rearrangement(s) was not demonstrable by Southern blot hybridization to cDNA probes. However, two unrelated patients showed a variant restriction pattern after Pvu II or BamHI digestion, due to mutations in the last exon: analysis of their pedigrees, including three or seven heterozygotes, respectively, with approximately 50% reduction of both enzymatic and antigen level, showed the abnormal restriction pattern in all heterozygous individuals, but not in normal relatives. Cloning of protein C gene and sequencing of the last exon allowed us to identify a nonsense and a missense mutation, respectively. In the first case, codon 306 (CGA, arginine) is mutated to an inframe stop codon, thus generating a new Pvu II recognition site. In the second case, a missense mutation in the BamHI palindrome (GGATCC----GCATCC) leads to substitution of a key amino acid (a tryptophan to cysteine substitution at position 402), invariantly conserved in eukaryotic serine proteases. These point mutations may explain the protein C-deficiency phenotype of heterozygotes in the two pedigrees.

Effect of hemodialysis on protein C levels.

The immunological functions of antithrombin III and protein C were studied in 8 patients before, during and after hemodialysis. After dialysis a significant decrease in protein C and antithrombin III levels was seen. The changes observed in both proteins after hemodialysis should be considered as a component in the genesis of the hypercoagulation state in these patients.

Lupus anticoagulant and false positive serological tests for syphilis.

A 41-year old male who presented for blood donation for the first time was rejected on the grounds of a positive reaginic serological test (VDRL and RPR) and a specific antitrepomena test (FTA) for syphilis. Later studies revealed the existence of a lupus anticoagulant and this was made responsible for the positive serology. Our observation suggests the need to search for the existence of a lupus anticoagulant in blood donors with non-specific or specific positive antitrepenema test.
