[A surgical technique for high aortic occlusions].

The authors analyze the technical aspects of aortic prosthesis in 37 patients with atherosclerotic, high occlusions of the aorta. The upper occlusion limit in this abnormality is shown to have two X-ray variants. Thoracophrenolumbotomy is considered to be the best access for proximal anastomosis in aortic prosthesis. The traditional operation technique for this abnormality has some disadvantages, the main limit of which is the necessity of shifting a patient from place to place for proximal and distal anastomoses. This makes the operation in these severe patients longer. The authors have proposed a novel procedure for aortic prosthesis by employing a special roller. The latter eliminates the necessity of shifting the patient from place to place during surgery, cuts surgical time by more than 2 hours, allows one to follow asepsis more carefully.

[Proximal anastomosis in surgery for Leriche's syndrome].

The author analysed the frequency of different variants of atherosclerotic affection of the aortoiliac-femoral segment in 300 patients who underwent operation. With consideration for these data, the surgical tactics and techniques of proximal anastomosis in aortofemoral shunting (prosthetics) in 216 patients with variants of Leriche's syndrome are appraised. In most patients the operation may be carried out through an extraperitoneal approach. The choice of the type of proximal anastomosis in aortofemoral shunting (prosthetics) should be guided by the character of the affection, the condition of the branches of the abdominal aorta, and the necessity for revascularization of one or both extremities in one stage. The chosen tactics and techniques of the operation made it possible to preserve 86.1% of the vascularized extremities.

[Results of the treatment of patients with Leriche's syndrome and ulcerous-necrotic foot changes].

A comparative analysis of two groups of patients with Leriche's syndrome was conducted: 88 patients suffered only from intermittent claudication (stage III of the disease), while 128 patients had pyo-necrotic changes on the feet (stage IV). All patients were subjected to revascularization of the ischemic extremities by restorative operations with the use of a synthetic prosthesis. The necrotic lesions of the feet were encountered mostly in patients with a combined affection. Due to the severe condition of the patients, however, one-stage operations on the ++aorto-popliteal and femoropopliteal segments were not undertaken. An aortic deep femoral shunt was formed, but if ischemia of rest was maintained reconstruction in the femoropopliteal segment was performed in a second stage. Comparative analysis of the immediate and late-term results in patients of these two groups was conducted. It is shown that the operative risk is higher in stage IV than in stage III. But operations in patients with necrotic changes can be considered well grounded because the late-term results (preservation of the extremity, frequency of deep suppurations) hardly differ from those in patients with intermittent claudication.

[Surgical tactics in injuries of the major cervical blood vessels].

The authors propose recommendations on the surgeon's tactics on the basis of their experience with the treatment of 29 patients with injuries of major cervical vessels and an analysis of 17 protocols of medicolegal autopsies of corpses with such localizations of trauma. The method of the temporal intravascular shunting of the artery is shown to be valuable. Of great importance is the correct organization of the activities of specialized surgical service for a successful treatment of the patients.
