[Chromatin proteins in rat liver cells, interacting with reactive oligothymidylate derivatives].

Upon alkylation of the rat liver chromatin with a hexadecadeoxyribothymidylate derivative bearing 4-(N-2-chloroethyl-N-methylamino)benzylamine residue on the 5'-terminal phosphate two nonhistone proteins were modified under conditions of the reagent's forming complementary complexes with poly(A) sequences in DNA. The sequence-specific nature of the reaction is proved by the competition experiments: free oligothymidylate prevented the proteins from alkylation whereas arbitrary oligonucleotide did not. Modification with the reactive oligonucleotide derivatives can be used for the identification of proteins located in the vicinity of the specific open DNA regions in chromatin.

[Evaluation of the biological significance of changes in the organism under external influence].

Estimation of the statistical reliability and the biological significance of changes caused by external actions are two independent parts of data analysis. Statistical reliability is a criterion of the reality. The principal unhealthy effects are decreasing of living term and increasing of morbidity. Other effects seem harmful on the basis of physiological analysis, and those are less reliable. Increasing of harmful effects frequency, but not the parameters values changes (if opposite is not proved), is a criterion of unhealthy effects.

[Deontological problems of instrumental diagnostic examinations].

Deontological problems arise in contacts with patients and in relation to attending physicians. The difficulties with patients arise more often in specialists who deal with patients closely (endoscopists, physiologists). They perform sophisticated and dangerous tests, are limited in discussing their results with patients, diagnostic decision is made by attending physician. It is noted that recording of diagnostic evidence becomes more detailed to prevent misinterpretation.

[Lung cancer in women in Moscow (epidemiology, etiology and surgical treatment)].

In the studied period (1973-1987) the lung carcinoma morbidity rate among females of Moscow, in general intensive indices (per 100,000 females), showed a tendency to reduce (18.4 in 1973 and 15.1 in 1987). A reverse tendency is mostly encountered in other countries. More than half of patients with lung carcinoma were over 70 years of age. The ratio of males with lung carcinoma to that of females grew from 3.0:1.0 in 1973-1977 to 3.6:1.0 in 1983-1987. In the period from 1979 to 1988, 380 females suffering from carcinoma of the lung were hospitalized to the thoracic department of the clinic. Operations were performed on 215 of them, 174 of the operations were radical. The ratio of operated on males to females was 7.4:1.0. The frequency of central carcinoma among females was 28.8%, which was half that among males (56%). The number of operations according to volume was as follows: pulmonectomy 37, lob- and bilobectomy 119, wedge resection 18. More than half of the female patients had the glandular form of lung carcinoma and its incidence was 3-5 times that among males who were operated on (10-15%). According to the authors, the principal etiological factors are smoking, a history of another malignant tumor, and the exogenous effect of the consequences of urbanization.

[Factors of random selection in epidemiological surveys].

In epidemiological surveys the association of variable under study, for example, morbidity and conditions of work, may be distorted by phenomena of selection. The most frequent effect of selection is "the effect of healthy worker" which consists in lower morbidity and mortality of workers as compared to the population at large. This effect can mask the pathogenic action of environmental conditions. Under other circumstances multiple effects of selection, on the contrary, may additionally enhance the association between morbidity and the environmental conditions. The elimination of selection errors is possible, if the analysis of data takes into account the broad range of variables capable of affecting the dependence. The issue of causative nature of dependence should be solved proceeding from the analysis of the aggregate data on the problem.

[Sequence specificity modification of double-stranded DNA with an alkylating derivative of oligodeoxyribonucleotide pT(CT)6].

It is shown that in slightly acidic solution (pH approximately 5.3) reagent CIRCH2NHpT(CT)6 (RCl = -C6H4-N(CH3)CH2CH2Cl) modifies a double-stranded DNA fragment (120 b. p.) containing A(GA)6.T(CT)6 sequence at a single nucleotide residue, viz. G29 located near to this sequence in the DNA chain. The location of this modification point suggests formation of a triple-stranded reactive complex with parallel orientation of the pyrimidine oligonucleotide moiety of the reagent and pyrine sequence of the target DNA. Analysing the modification extent dependence of the reagent concentration the association constant Kx between the reagent and DNA was calculated (Kx = (0.95 +/- 0.03).10(5) M-1, 25 degrees C, pH = 5.3, [NaCl] = 0.1 M). The modification by the reagent ClRCH2NHpT(m5CT)6 has the same quantitative characteristics as in the case of ClRCH2NHpT(CT)6.

[Surgical correction of duodenal obstruction caused by malignant tumor].

The results of treatment of 6 patients with duodenostasis, caused by cancer of the duodenum (2) and pancreas (3), metastasis of the sigmoid colon cancer (1) are analysed. Creation of duodenojejunoanastomosis with Brown's interintestinal stoma should be the obligatory condition of palliative operation. For diversion of the main food mass, the anterior gastroenteroanastomosis with Brown's interintestinal stoma was created.

[Surgical tactics in peptic ulcers after operations on the stomach].

Examined were 128 patients with peptic ulcer of the anastomosis and jejunum. Of them, 117 were operated on. In 58 patients, the major gastric resection was performed, in 8--gastric resection with gastroenteroanastomosis, in 38--truncal vagotomy, in 3--gastrectomy, in 10--operations in gastrojejunocolonic fistula. Complications after the operation developed in 22.2% of cases. Postoperative lethality was 5.9%.
