[Effect of hyperbaric oxygenation on the circulatory function in rheumatic mitral valve disease].

The effect of a hyperbaric oxygenation course (10-14 exposures, one exposure daily at 0.7-1.0 ATA) upon the circulation function was studied in 61 patients with rheumatic mitral valve disease combined with systemic circulation insufficiency. The obtained results (reduction of the heart volume, acceleration of the blood stream within the heart and the body, increase of the stroke index, of the blood flow in the muscles of the extremities, reduction of the arterial and central venous pressure) demonstrate that the contractility of the myocardium and redistribution of the cardiac output between separate vascular regions is improved under the effect of hyperbaric oxygenation. The authors suggest that the increase of contractility in the right or left heart predominantly depends on the prevalence of mitral stenosis or circulatory insufficiency.

[Prevention and treatment of postoperative broncho-pulmonary complications with ultrasonic aerosols].

The method and results of applying ultrasonic aerosols in 485 patients, operated upon on abdominal organs, are presented. Ultrasonic aerosols were included in the complex of prophylactic and therapeutic measures in the postoperative period. Prophylactic measures were started immediately after patient's being returned from the operating room. Special attention was given to patients in whom ventilatory disturbances developed during anesthesia, to patients with obesity, chronic bronchitis, emphysema, pneumosclerosis and also those, who negated the rationality of prophylactic measures. The use of finely dispersed aerosols after the method suggested by the authors allowed a 6 times reduction in the incidence of pulmonary complications and more than twice shortening of the postoperative pneumonia course.

[Peritonitis of appendicular origin].

The results of treatment in 2437 patients with peritonitis of the appendicular origin are analysed. The causes of development of this complication, therapeutic aspects and also most frequent tactical and technical errors, made while operating patients with peritonitis, and their influence on issues of the lesion are discussed.

[Causes of development of peritonitis after appendectomy].

According to the authors' data peritonitis developed after appendectomy in 122 of 18347 patients, operated upon for acute appendicitis, was an extremely grave complication of this lesion and the related surgical intervention. The main causes for development of postoperative peritonitis were local and diffuse peritonitis (101 cases), more rarely--technical and tactical errors made during the operation. Due to diagnostic difficulties a purposeful treatment for postoperative peritonitis was undertaken with a delay, reoperations were performed in late terms and not in every case either. The mortality due to postoperative peritonitis made 23 per cent.
