[Clinico-anatomical analysis of the causes of death in ruptures of intracranial arterial aneurysms].

Causes of lethal outcome in patients after subarachnoid hemorrhage due to cerebral aneurysm rupture were analysed. The major death cause in the operated patients was cerebral edema and dislocation, while in non-operated ones it was the penetration of blood into the ventricles of the brain. Cerebral edema and dislocation syndrome were mainly caused after surgery by ischemic changes in cerebral tissue. A relationship between the aneurysm localization and the extent of subarachnoid hemorrhage was revealed as well as between the hematoma localization and incidence of hemorrhage into the ventricles of the brain.

[Prediction of recurrent hemorrhages from ruptured arterial aneurysms in the acute period].

In a series of 61 patients the authors studied the effect of some risk factors on the development of recurrences of haemorrhage from arterial aneurysms of cerebral vessels in the acute period of their rupture. The highest risk of recurrences was associated with an elevated fibrinolytic activity of the cerebrospinal fluid, spasm of the cerebral vessels, aneurysms of the anterior connective and internal carotid arteries as well as the patients' grave condition. Consideration of these factors makes it possible to specify the indications for surgical or conservative treatment. Antifibrinolytic therapy conducted in 5 patients with elevated fibrinolytic activity of the spinal fluid elicited good response.

[Diagnosis and treatment of patients in the acute period of a subarachnoid hemorrhage as a result of rupture of arterial aneurysms of the brain].

The authors suggest a complex of measures for examination of patients with subarachnoidal hemorrhage (SAH), which makes it possible to elucidate the genesis of the hemorrhage and to decide on the tactics of treatment within the first hours after the stroke. They prove the necessity of hospitalization or transfer of patients with SAH to neurosurgical departments as well as of surgery in the acute stage of aneurysmal rupture.

[Principles in the organization and surgical treatment of patients with non-traumatic intracranial hemorrhage].

By taking into consideration the frequency of different forms of cerebral circulatory disorders, the authors determined the number of operations required to be performed for cerebral bleeding of various etiology in localities with different numbers of populations. The paper presents some results of treatment of patients with aneurysmal ruptures and hemorrhagic stroke at the Department of Neurosurgery, N. V. Sklifosovsky Research Institute of Emergency Care. The ways of improving the quality of care to patients with non-traumatic intracranial hemorrhage are outlined.

[The angiographic diagnosis of vascular spasm in ruptured cerebral aneurysms].

The authors compared the results of qualitative and quantitative appraisal of arterial spasm according to the angiographic findings in 85 patients with the severity of their condition and neurological symptomatology in the acute period of rupture of a brain aneurysm. The spasm was diagnosed by quantitative appraisal in 80% and by qualitative appraisal in 72.9% of cases. Appraisal of the spasm in various periods after rupture of the aneurysm showed it to be most intense on the 9th-10th day. The vascular spasm (VS) was the main cause of the neurological symptomatology in the acute period of rupture of the aneurysm. Constriction of the arteries to 1.65 in quantitative appraisal of the spasm and its extension to 3-4 segments and more in qualitative appraisal were attended by diffused consciousness and stem and focal hemispheric symptomatology. Simultaneous use of both methods for VS appraisal makes it possible to evaluate its extension and the degree of arterial constriction and distinguish the most severe forms of spasm.

[The assessment of brain stem function in the acute period of aneurysm rupture].

The functional status of the brain stem was assessed by recording evoked stem auditory potentials (ESAP) in 76 patients in the acute period of arterial aneurysm rupture. Abnormalities of ESAP were detected in 39.4% of the patients. The determinant of ESAP disorder in aneurysm rupture is the occlusive-dislocation syndrome caused by intracranial hematoma, more than 50 ml in volume, ventricular tamponade or extensive vascular spasm.

[The surgical treatment of traumatic brain lesions located sub- and supratentorially].

The article describes two patients with a rarely encountered pathological condition--traumatic damage to the brain of sub- and supratentorial localization. Timely diagnosis and early surgical treatment of multiple injuries to the cerebral hemispheres and posterior cranial fossa became possible with the introduction of brain computed tomography into clinical practice.

[Current guidelines for the diagnosis and treatment of severe brain injury].

Based on the international and Russian guidelines, the paper describes standards, recommendations and options developed on the principles of evidence-based medicine for severe brain injury. It also presents a spectrum of recognition, management, and differential treatment of victims with severe brain and skull injury.
