[Clinical and pathogenetic features of neurotrophic corneal disorders in diabetes].

The corneal neurotrophic status was evaluated in 110 patients (220 eyes) with diabetes mellitus and in 20 normal controls (40 eyes). Corneal abnormalities (gerontoxon, limb vascularization, punctate keratopathy, endothelial dystrophy, relapsing erosion, corneal ulcers) were detected in 73.6% diabetics. Corneal changes depended on the type, duration, and compensation of diabetes mellitus. Based on the diagnostic markers (threshold sensitivity of the cornea, microcirculatory status, bulbar conjunctiva status, level of lacrimal production, cellular composition of the limb smears impressions, lacrimal levels of catecholamines, hydroperoxides, and lipids), three types of diabetic neuropathy of the cornea were distinguished: vegetative, sensory, and vegetosensory. Corneal changes were associated with signs of diabetic neuropathy in 55.6% cases and with autonomic nervous disorders of the cardiovascular system in 46.9% cases, which evidences a generalized involvement. The detected neurotrophic features of the cornea should be borne in mind when choosing pathogenetic therapy for diabetics with corneal complications and preparing the patients to ocular surgery.

[Central and regional hemodynamics of eye in different course of primary open-angle glaucoma with normalized intraocular pressure and their relationship with circulation types, sympathoadrenal activity and hydrocortisone level].

The relationship of systemic and local hemodynamics with neurohumoral factors has been evaluated in 104 patients with primary open-angle glaucoma with normalized intraocular pressure. Changes in the functional activity of the sympathoadrenal activity and hydrocortisone levels were in high correlation with the initial type of systemic hemodynamics: the hormonal activity of the sympathoadrenal system predominated in patients with eukinetic and hyperkinetic circulation with normal levels of hydrocortisone, while in patients with hypokinetic circulation the mediator (norepinephrine) component of the sympathoadrenal system combined with a high plasma level of hydrocortisone predominated. A principal characteristic of unstable primary open-angle glaucoma, discriminating it from the stable condition in all types of circulation, is the presence of deep autonomic disorders in the neurohumoral mechanisms of autoregulation of ocular blood flow, because of excessive accumulation of norepinephrine, epinephrine, and hydrocortisone in the lacrimal fluid at the expense of their decreased utilization by the receptor systems of ocular tissues and blood vessels.

[Differentiated approaches to the treatment of nonstabilized primary open-angle glaucoma with normalized intraocular pressure considering its pathogenic features].

Clinical efficiency of dedystrophic treatments for nonstabilized primary open-angle glaucoma (POAG) in the presence of normalized intraocular pressure is compared in 168 patients (246 eyes). In one group of patients ischemic angiopathy and hyperreactivity of optic vessel adrenoreceptors associated with hypokinetic central hemodynamics and constitutional metabolic status of the organism was corrected by cinnarisin and riboxin. Patients with predominating congestive angiopathy symptoms, hyper- or eukinetic circulation and "slow" acetylation were treated by pantothenic acid, endotelon, and hyperbaric oxygenation. In both groups epithalamine, eiconol, and magnetic laser therapy were used, if indicated. This helped stabilize the process in 91% patients with initial POAG, in 87.5% with well-developed condition vs. 66.1% and 38.2% patients treated by traditional therapy (period of observation 3 years).

[Characteristics of morphological and functional state of erythrocytes in patients with primary open-angle glaucoma with normalized intraocular pressure].

Disorders in erythrocyte morphology and function (impairment of erythrocyte membranes and decreased erythrocyte deformability), leading to impairment of capillary endothelium with release of Willebrand factor into the blood, increase of blood viscosity, and disorders in microcirculation and transcapillary exchange in the optic disk, were detected in 87.5% of 49 patients (70 eyes) with nonstabilized course of primary open-angle glaucoma with normalized intraocular pressure. Biochemical analysis of plasma lipids showed atherogenic hyperlipidemia with decreased antioxidant activity, which favored the development of these changes and was one of the factors determining the clinical course of disease.

[Relationship between constitutional type of systemic hemodynamics and formation of peripheral vitreo-chorioretinal dystrophies during pregnancy].

Dynamic observations of 86 pregnant women with peripheral vitreochorioretinal dystrophies (PVCRD) (121 eyes) showed that the condition progressed during pregnancy in 33.8% cases. Decrease of ocular hemodynamics and scleral rigidity were characteristic of pregnancy, being particularly pronounced in OPH gestosis. The highest incidence of PVCRD progress was observed in pregnant women with the hypokinetic type of systemic hemodynamics, being 30% in normal pregnancy and reaching 48% in OPH gestosis vs. 8.3 and 16.6 and 16.6 and 22.7% in the hyper- and eukinetic types, respectively.

[Study of the nervous-vascular reactivity of the bulbar conjunctiva in patients with primary open-angle glaucoma with normalized intraocular pressure].

Clinical evaluation of adrenoreception was carried out in 96 patients with various clinical patterns of primary open-angle glaucoma with normalized intraocular pressure by changes in vascular reactivity in the bulbar conjunctiva capillaries in response to epinephrine and norepinephrine. Neuro-vascular reactions of the bulbar conjunctiva in response to norepinephrine and epinephrine were different in patients with different patterns of primary open-angle glaucoma with normalized intraocular pressure. A significant correlation between the rate of progress of optic disk glaucomatous atrophy and the parameters of ocular adrenoreceptor structures of the eye was detected.

[Administration of epithalamine in the treatment of unstable glaucoma of different types after normalization of intraocular pressure].

The treatment effect and clinical efficacy of epithalamine solution was studied on 28 patients (42 eyes) with unstable glaucoma and normalized intraocular pressure (IOP). Epithalamine belongs to the group of cytomedines and it has a modeling effect on the functional activity of the simpatico-adrenaline system, adrenal cortex and anti-oxidizing activity. The clinical study showed that the use of epithalamine solution resulted in normalization of the biochemical activity of the neuro-humoral triphism (adrenaline, norepinephrine, and hydrocortisone). It leads to an increased diameter of arterioles of the bulbar conjunctiva and of the optic disk as well as to a decreased degree of congestive angiopathy and of intravascular aggregation of erythrocytes.

[The analogy of biological effects of ultra low chemical and physical doses].

The analogy of the biological influence of low intensity physical factors in the low-frequency range and chemical compounds in hyper-poor concentrations was estimated. A resonance mechanism of these effects was proposed, based on the presentation of the solution as a quasi-crystalline lattice with a definite frequency oscillation. The theoretical calculations of the active concentrations based upon the experimental frequencies of the biological effects are in good accordance with those found in the relevant literature.

[Effect of the low frequency physical factors on seeds].

The influence of weak low frequency (50 Hz-10 kHz) physical factors: sound, alternating electric current and impulse light on the germination and root growth rate of barley corns was estimated. Several mechanisms of this effect were proposed: change in penetration of membranes, in enzymes and microflora activity. The dependence between the germination activity of corn and the frequency was represented in the form of universal curve--"Low Frequency Activity Spectrum".

[Radionuclide methods of measuring mineral contents in the bone tissue].

Over the recent 10-15 years, osseous densitometers intended for measuring the content of minerals in osseous tissue have become very popular abroad. To record statistically significant changes in an osseous mineral, these devices should have reproducibility (variation rate) exceeding 2%. Mono- and biphoton densitometers may be distinguished by the type of the applied radionuclide. Biphoton densitometers are advantageous since they permit examining any part of the skeletal system of the patient or the whole body. Biphoton absorptiometry is based on measuring the transition of photons of two energies via the medium that largely consists of the two materials bone and soft tissue. The authors describe the results of examining parts of the peripheral skeleton with the aid a laboratory sample of a biphoton osseous densitometer for the peripheral skeleton.

[Experimental study of the efficiency of use of sodium hypochlorite in the complex treatment of purulent keratitis].

The experiment has provided evidence for the safe use of 0.1% aqueous sodium hypochlorite for ocular tissues and for its therapeutic efficacy in purulent keratitis. The therapeutic effect is achieved by accelerating the lysis of neurotic tissues, by suppressing the pathogenic microflora, by alleviating an inflammatory reaction, by stimulating epithelization and reparative regeneration, by activating the phagocytic properties of neutrophilic leukocytes by 1.8-times, by increasing their functional reserves by 1.3 times as compared to the similar parameters observed in the use of conventional treatments.

[Pharmacology of the venom from the scorpion, Buthus eupeus].

Reactions induced by venom from the scorpion Buthus eupeus and by toxin of the venom have been studied on isolated organs and tissues as well as on intact body. It has been shown that substances interfering with the conduction of nerve excitation, with the mediator storage and interaction of the latter with synaptic receptors prevent the reactions induced by the venom and toxin, whereas ganglion-blocking agents do not affect these reactions. The catecholamine content in rat blood plasma has been found to be increased. It is assumed that the venom and toxin that affect the postganglionic nerve terminals release mediator acetylcholine and noradrenaline.

[Current methods and equipment in densitometry of the bone tissue].

In the past 15 years there has been an intensive development of methods and equipment for non-invasive quantitative determination of the mineral status of the human skeleton in the USA and some Western European countries. The specialized osseous densitometers which are based on photon absorptiometry and which use various radionuclides or an X-ray tube as a source of radiation have extensively spread.

[A multifactorial analysis of the significance of diagnostic signs in the progression of the glaucomatous process].

Analysis of the principal symptoms in patients with primary and advanced open-angle glaucoma has shown that when the initial stage of glaucomatous progress progresses noticeable changes in the optic disk and visual field occur and hydrodynamic parameters deteriorate, as is the trophic status of the anterior segment of the eye. Negative changes in these parameters should be timely detected during follow-up of patients with primary glaucoma to correct the treatment policy.
