[The effect of spasmolytics on dilatation of the uterine cervix].

UNLABELLED: Spasmolytics and spasmoanalgetic mixtures are administered to facilitate dilatation of the cervix during delivery and to shorten the first stage of labour. This medication is used in 70% deliveries at the 1st Dept. of Obstetrics and Gynaecology Masaryk University in Brno. The aim of the study was to analyze the spasmolytic effect on the process of cervix dilatation. The study comprises 108 uncomplicated deliveries (52 deliveries with spasmolytics and 56 deliveries without). We analyzed the effect of spasmolytics on the cervix, process of labour and outcome of delivery. Six kinds of spasmolytics were used, administered from once to four times during labour and most of them (85%) in the latent phase of the first stage of labour. In the group with spasmolytics there was statistically insignificant by prolonged active phase of the first stage of labour (126 vs 104 minutes) and more frequent administration of Oxytocin. Others parameters such as length of the second stage of labour, rate of birth injuries, the 1st minute Apgar score less than 7 and the necessity of neonatal resuscitation did not differ in the two groups. CONCLUSIONS: The application of spasmolytics did not significantly affect the process of delivery.

A multicenter European survey of the attitudes to contraception in women at high risk or with established cardiovascular disease.

OBJECTIVES: The survey was designed to study the attitudes of gynecologists from 11 European centers providing guidance in contraception to women at high risk and women with cardiovascular disease. METHODS: Attitudes were registered by means of a standard questionnaire. RESULTS: Most contributors reported that, in women with venous thrombosis (10/11), deep venous thrombosis (10/11), coagulation disorders (11/11) and stroke (8/11), they prefer to prescribe methods other than combined oral contraceptives (COCs). A history of myocardial infarction was considered a relative contraindication and some experts suggested that, in some cases, the use of third-generation COCs may be possible. COCs were not recommended in women with severe cardiovascular disease and in those over 35 years of age with light or moderate cardiovascular disease, heavy smokers (over 20 cigarettes per day), or those presenting with severe hyperlipidemia. CONCLUSION: The pill is not considered appropriate for women with clinically established cardiovascular diseases or in cases where more than two coronary risk factors exist. COCs may safely be given to women with elevated blood pressure as long as it is lower than 160/100 mmHg, in cases of light and moderate cardiovascular disease as long as the patient is less than 35 years of age, in women who are not heavy smokers, in the presence of a light or moderate degree of hyperlipidemia, and in uncomplicated diabetes mellitus provided that there are no additional risk factors. In these cases, third-generation COCs are preferred. The co-operation of the cardiologist is desirable in order to classify cardiovascular disease and for patient follow-up.

[Hypertension in pregnancy--chaos in terminology of literature world-wide].

The authors submit an account of the terminology of toxemia of pregnancy and hypertension during pregnancy which are used in the world literature. They draw attention to the lack of uniformity which leads to a distortion of statistics. Differences in the nomenclature and incorrect enlistment of hypertension may lead also to an inadequate type of treatment and conduction of delivery. The authors recommend the use of nomenclature based on the pathogenesis and on their own experience. They emphasize the difference in the pathogenesis, therapy, prognosis and prevention of different types of hypertension.

[Problems of epidemiology and diagnosis in the vaginal area. II. Measurement of vaginal pH values and its relation to vaginal diseases].

The authors divided a group of 600 women into 7 sub-groups based on the results of microscopic examination of native preparations, and using indicator papers of MERCK Co. (art. no. 9542) they assessed values of the vaginal pH. The highest values were recorded in a group of women with suppurative bacterial inflammations (mean pH 5.68) and the lowest ones in women with the finding of fungi (mean pH 5.0), cocobacillary flora without leucocytes (mean pII 4.39) and the normal physiological finding of Dörderlein's bacillus (mean pH 4.15). Values higher than 4.8 rule out the possibility of a normal finding in the vagina, lower values, however, do not rule it out entirely. Assessment of pH values in the vagina should be considered part of routine examinations of the vaginal environment.

[Epidemiology and diagnosis in the vaginal environment. III. Diagnosis and epidemiology of bacterial vaginosis].

In a group of 600 examined women clue cells were detected in the native preparation in 72 women (12%), a positive amine test was recorded in 81 women (13.5%). 93.1% patients (67) with the finding of clue cells had a positive amine test. The values of the vaginal pH vary between 4.8 and 5.5. According to the accepted diagnostic concept bacterial vaginosis was detected in 68 women (11.5%). The most frequent concurrent infection was trichomoniasis. Finally the authors recommend to transfer the diagnosis of vaginal discharge from laboratories to surgeries, from fixed stained smears to native smears supplemented by the amine test and assessment of vaginal pH.

[Epidemiology and diagnostics in the vaginal area. I. Study of the vaginal flora using microscopy and evaluation of the microbial picture of the vagina].

The microscopic characteristic of the vaginal microflora by means of microbial vaginal smears (MVS) according to Jírovec, Peter and Málek corresponds no longer to contemporary ideas on the physiology and pathology of the vaginal environment. The authors revealed on comparison of the results of MVS and the native preparation that in a group of 600 women selected at random the MVS diagnosis allowed to escape 80 cases of trichomoniasis (46.8%), 56 cases of mycotic infections (34.1%) and cases of suspect bacterial vaginosis with the finding of key cells. After elimination of suppurative and non-suppurative bacterial discharge the accurate diagnosis of which involves cultivation, it was revealed that in 156 women (26%) the pathological vaginal discharge was not diagnosed at all or its diagnosis was incomplete. The category of MVS type I, III and V or VI are highly specific but less sensitive. In the category MVS type II the specificity declined substantially, the sensitivity of the test is slightly higher. Predictive values in categories MVS III and MVS V or VI are high, much higher than values obtained in categories MVS I and in particular MVS II. In the diagnosis of vaginal infections transmitted by sexual intercourse native microscopy is irreplaceable. Its assett is rapid and more accurate diagnosis of diobscure findingsseases and the possibility to start immediate and aimed treatment. In case of obscure findings or relapses cultivation is essential.

[Epidemiology of vaginal mycoplasmas].

The authors made in 456 women mycoplasmatological examinations of the vaginal secretion by cultivation. Mycoplasma hominis was obtained in 158 women (34.6%). U. urealyticum in 295 women (64.7%) In 120 women (26.3%) both microorganisms were present. The prevalence of vaginal mycoplasms is markedly influenced by the sexual activity of women. The relative risk of colonization of the vagina with M. hominis rises with the frequency of sexual intercourse more than four times. In case of a larger number of sexual partners the relative risk of colonization of the vagina with U. urealyticum is 7.7 times higher than in women with a single partner.

[Changes in the myometrial cell structure during pregnancy and its effect on electrical resistance of the myometrium measured in vitro].

The authors investigated ultramicroscopically the intercellular connections in the myometrium of pregnant women. In a small group an increased frequency of these connections was observed in association with labour, but unrelated to the period of pregnancy. By measuring the electric resistance of the myometrium in vitro a lower electric resistance of the uterine musculature was recorded in women where uterine activity was under way. The authors assume that the increased presence of free connections reduces the electric resistance of tissue and makes more rapid spread of information in the myometrium possible. In the long run it is one of the factors which lead to spontaneous uterine activity.

[Interlocking of twins].

We report about a very rare case of locked twins in cephalic presentations. A difficult forceps was necessary. In spite of cleidotomy and pushing the other twin out of the pelvis the fetus was in a deep distress and the resuscitation was unsuccessful. The second twin followed spontaneously, alive and well.

[Isolation of mycoplasmas from female genitalia].

The author tested ways of collection, transport and storage of material for mycoplasmatological examination. The use of tampons on a stick during transport in urea substrate medium proved useful. The site of maximum occurrence of mycoplasmas was the posterior vaginal vault. On examination of the vaginal secretion of 804 women Mycoplasma hominis was isolated in 29.6% and Ureaplasma urealyticum in 65.2% of the cases. Concurrent isolation of Mycoplasma hominis and Ureaplasma urealyticum was recorded in 22.1% of the women. The results of the examination do not suggest the participation of mycoplasmas in the development of aminocolpitis. In pregnant women there is a greater probability of colonization of the vagina by U. urealyticum.
