Complete atrioventricular canal: current surgical technique.

The technique we currently use for repair of complete atrioventricular canal requires 2 separate patches for closure of the atrial and ventricular portion of the defect. The common AV valve is left undivided and is sandwiched between these 2 patches. The cleft in the anterior mitral leaflet is left unsutured to create a trileaflet mitral valve. This technique has been employed in 10 patients (Type C: 9 patients; Type A: one patient; ages: 6 months to 4 1/2 years).

Kinematic analysis and simulation of vertebral motion under static load-part II: simulation study.

Based on the earlier work by the author on the development of a theoretical motion simulation model of an open-loop, kinematic chain, the present paper examines the motion characteristics of a human spine motion-segment. Based on the experimental data collected from the human spine cadaver specimens of lumbar spine, a static simulation model of a lumbar segment is proposed. The equivalent motion characteristic of the intervertebral joint is described by a spherical pair which is located for an intervertebral joint by fitting in a least-square sense a cone to a set of motion data describing the axode characteristics of the joint. The intervertebral disk and the ligaments are represented by an equivalent stiffness matrix whose elements are assumed to be constant over the entire range of mobility. The proposed model showed a good agreement with the observed motion data of an intervertebral joint.

Disappearance of echocardiographic mitral valve prolapse during ventricular pacing with retrograde artial activation.

M-mode echocardiography in a 68-year-old man with a permanent transvenous pacemaker revealed intermittent mitral valve prolapse. The mitral valve prolapse was present during normal sinus rhythm as well as during pacemaker beats either preceded by or not associated with any P wave activity. During pacemaker beats followed by retrograde P waves, the mitral valve prolapse was no longer seen. It is postulated that atrial contraction during ventricular systole produced a rise in left atrial pressure and prevented the mitral valve leaflets from prolapsing into the left atrium.

Vascular rings: management and a proposed nomenclature.

Over 18 years, 33 patients with congenital vascular rings underwent surgical treatment. All patients had full recovery except 1 who died late. At the one-month follow-up, 66.7% had complete relief of the presenting symptoms. Important aspects of the clinical picture, diagnostic methods, and treatment are presented. A coded nomenclature is proposed; we believe it to be accurate, inclusive, easy to remember, and readily referable to the embryological origin.

Pantothenic acid nutritional status in the elderly--institutionalized and noninstitutionalized.

Pantothenic acid nutritional status was evaluated in 37 men (32 noninstitutionalized and five institutionalized) and 54 women (33 noninstitutionalized and 21 institutionalized) 65 yr of age or older. A fasting blood sample, a 24-h urine specimen and a food consumption record for 1 wk were obtained from each subject. Hematological, anthropometric, and dietary parameters indicated these 91 subjects were similar to elderly populations evaluated in other studies. Pantothenic acid was analyzed by both radioimmunoassay and microbiological assay. By combining methods it was possible to differentiate various forms and derivatives of pantothenic acid. Free pantothenic acid, but not the phosphoderivatives, was observed in urine. Microbiological and radioimmunoassay estimations of pantothenic acid showed good correlation (r = 0.91). Pantothenic acid excretion in the institutionalized subjects (7.5 +/- 1.3 (x +/- SEM) mg/g creatinine) was comparable to the excretion levels in the noninstitutionalized subjects (5.9 +/- 0.6 mg/g creatinine). Those consuming pantothenic acid supplements had significantly higher excretion levels. Blood pantothenic acid values between the groups, noninstitutionalized (537 +/- 27.4 ng/ml) and institutionalized (615 +/- 47.3 ng/ml), were comparable. The average dietary intake of pantothenic acid for the elderly population studied was 5.9 +/- 0.1 mg/day with the institutionalized and noninstitutionalized subjects having a similar intake of 2.9 mg/1000 kcal. Institutionalized elderly males and females were consuming 2237 and 1962 kcal, respectively, while their noninstitutionalized counterparts were consuming 2201 and 1887 kcal.

Reconstruction of the new atrial septum in the Senning operation. New technique.

A new technique incorporating the left atrial appendage for the reconstruction of the new atrial septum is described as a modification of the original Senning operation for transposition of the great arteries. In reconstruction of the atrial septal flap, the balloon septostomy defect is closed by approximating the inverted left atrial appendage to the anterior margin of the flap. This technique completely eliminates the necessity for foreign material in the repair of the new atrial septum of the Senning operation and has been used in 21 patients with good results.

Refinements in coronary artery surgery contributing to improved survival.

Reviews of the literature and controlled studies of medical versus surgical treatment of coronary artery disease that include surgical results from 3 or 4 years ago may not be pertinent to therapeutic decisions made today. The results in our patients operated during 2 two-year periods (1974--1976, Group I and 1976--1978, Group II) are compared. The Group II patients had more severe coronary artery disease. They had significantly more grafts per patient (p less than .001), their cardiopulmonary bypass time was significantly reduced (p less than .001), they received less inotropic support (p less than .005) and spent less time in the intensive care unit (p less than .001). There was a significant decrease in the overall perioperative mortality (30-day) in the patients of Group II (p less than .055). The second two-year period coincided with the adoption of refinements in the intraoperative preservation of myocardium and also in surgical technique. These refinements are discussed in detail.

Tubular hypoplasia of the descending thoracic aorta. Surgical treatment with a PTFE bypass graft under moderate total body hypothermia.

An 8 1/2-year-old child presenting with hypertension was noted to have tubular hypoplasia of the descending thoracic aorta without collateral circulation on angiography. Surgical management consisted of bypassing the hypoplastic segment (10 cm) using a 10 mm PTFE graft under total body hypothermia to minimize spinal complications. The patient had an uneventful recovery with normalization of blood pressures postoperatively.

Anomalous origin of the left coronary artery from the pulmonary trunk: its clinical spectrum and current surgical management.

An anomalous left coronary artery arising from the pulmonary artery is a rare congenital anomaly which carries a serious prognosis with a high mortality in infants. Our experience in the surgical treatment of 9 patients with this condition during the last 5 years is discussed. The majority of patients were under the age of 2 years and presented with left ventricular failure. Surgical procedures included simple ligation of the left coronary artery, left subclavian to left coronary artery anastomosis, interposition vein graft, and direct reimplantation of the left coronary artery into the aorta. Patients treated by simple ligation survived the procedure but did not show improvement. Results with the subclavian artery to left coronary artery anastomosis were disappointing. Direct reimplantation or interposition of a saphenous vein graft appears to be the method of choice.

Congenital diverticulum of the left atrium.

Two cases of congenital diverticulum of the left atrial appendage are described. The first patient was a 3-year-old girl who had a cerebral embolism. The second patient was a 3(1/2)-year-old girl with signs of pericarditis. Treatment consisted of excising the diverticula. The limited literature on diverticulum of the left atrium as a congenital abnormality is also briefly reviewed.

Nutritional status of pantothenic acid in Indian pregnant and nursing women.

Investigations on nutritional status of pantothenic acid were carried out in 57 pregnant women and 24 lactating mothers belonging to the low socio-economic group. Fourteen non-pregnant, non-lactating women from the same group served as controls. Pregnant subjects had blood levels of the vitamin comparable to those of the control group while cord blood contained significantly higher concentrations of patnothenic acid than did the maternal blood. The urinary excretion of the vitamin in pregnant women was comparable to that in the control group. Blood and milk levels of the vitamin in the nursing women were in the reported range.
