[Comparative studies on early and late operations in aneurysm surgery].

In a five years period from January 1979 to December 1983 241 patients with angiographically verified aneurysms underwent operative treatment in the Department of Neurosurgery at Hannover Medical School. The final results six months after surgery are presented with special regard to the following subjects: preoperative grading (Hunt and Hess scale) and postoperative outcome (Glasgow Outcome Score) timing of surgery: early surgery within 72 hours (88 patients) versus delayed surgery (153 patients) incidence of rebleeding outcome of patients with space occupying intracerebral hemorrhage due to ruptured aneurysms.

[Experimental studies on the behavior of intracranial pressure under prostacyclin application].

In recent experimental studies the beneficial therapeutic effect of the arachidonic acid metabolite prostacyclin (PG 12) has been demonstrated for the treatment of different states of impaired brain perfusion following subarachnoid hemorrhage or focal cerebral ischemia. Prostacyclin has been recommended especially for the possible prevention and treatment of cerebral vasospasm after aneurysm rupture. Because prostacyclin is a potent vasodilator of the cerebral circulation it is an essential prerequisite for a possible therapeutic use of PG 12 that no negative side effects concerning the intracranial pressure are allowed to occur during intravenous infusion of the drug. In our study we therefore investigated the course of intracranial pressure during continuous experimental perfusion with prostacyclin in four different dosages. The results and our conclusions concerning further experimental or clinical studies with prostacyclin are presented.

[Demographic and social characteristics of patients with Crohn disease in an urban area. A study with neighborhood hospital controls].

120 patients with Crohn's disease were matched for age and sex with a control group out of the neighbourhood (NK) and a second control group out of the hospital (HK). In a personal interview the marital status, the educational attainment, the professional status and the status of invalidity were registered: Marital status, professional status and educational state were not different. Patients with Crohn's disease had higher qualification in medium school types (32,5% against 21,7% in NK and 23,3% in HK, p less than 0,05). Invalidity was significantly higher in Crohn's disease with 17,5% against, 3,3% in NK and 1,7% in KK (p less than 0,001).

[Prevention of acute stress hemorrhages of the upper gastrointestinal tract with pirenzepine and antacids. A controlled comparison between pirenzepine and cimetidine].

H2-antagonists, antacids and pirenzepine are effective drugs for prophylaxis of stress ulcer bleeding. Combined medication seems to be superior to single medication, but investigations of the effectiveness of a pirenzepine-antacid prophylaxis have not been carried out. We therefore evaluated the effect of this combination on gastric pH and on prevention of stress bleeding in ICU patients. In a prospective controlled randomized study 33 patients in each group received 50 mg. pirenzepine intravenously as a basic medication and alternatively 2000 mg. cimetidine i.v. or 2-hourly 10 ml. antacid. Gastric pH was determined three times per day. Stress bleeding was defined as bloody gastric juice, haematemesis or melaena. Two stress bleedings could be detected in each group. The pirenzepine-antacid combination proved to be superior to ensure a gastric pH of more than 3.5 than the pirenzepine-cimetidine group. After removal of the nasogastric tube the 2-hourly application of antacids proved to be impracticable.

[Carpal tunnel syndrome following arteriovenous forearm shunt in chronic dialysis patients--a review of 24 surgically treated patients].

In a period from January 1981 to September 1985 271 patients underwent surgery because of a carpal tunnel syndrome (CTS). In 24 patients (30 operated hands) with permanent kidney failure the CTS followed an arterio-venous shunt procedure in the forearm. Although there is a considerable variability concerning the interval between dialysis and onset of symptoms of median nerve entrapment CTS in patients with chronic hemodialysis is considered to be a late complication. In contrast in patients with idiopathic CTS a considerable time-lag has been noted between onset of symptoms and operative treatment which may be due either to diagnostic difficulties and/or delayed referral. The clinical picture presents with typical signs of a median nerve compression, known as brachialgia paresthetica nocturna. Operative treatment which consists of surgical division of the transverse ligament in local anaesthesia without interfascicular neurolysis is followed by immediate and usually longlasting pain relief. The possible etiology of CTS in patients with chronic hemodialysis which seems to be due to multiple factors is discussed.

[Clinical aspects, surgical treatment and prognosis of lumbar intervertebral disk displacement in childhood].

This paper reports on 11 girls and 3 boys aged from 12 to 16 who, having suffered prolapses of lumbar discs, underwent surgery and were followed up between 18 and 87 months postoperatively. These 14 patients represent 0.25% of those who have undergone disc surgery in the authors' clinic over the last 11 years. Clinical peculiarities are pointed out and, in the light of the positive results (13 patients completely free of complaints) the prognosis is deemed to be very favourable; surgical treatment is therefore recommended of conservative therapy fails.

[Spinal dysraphia and disordered ascension of the spinal cord in adults].

Spinal dysraphism and tethered cord syndrome which frequently present with clinical symptoms during infancy, childhood and adolescence are rarely encountered in adults. In this retrospective study covering a period of 15 years were report of 12 patients aged from 22 to 71 years with delayed symptoms from the tethered spinal cord. The clinical signs and symptoms which consisted of progressing neurological, urological and orthopedic malfunctions as well as the aspects of current neuroradiological diagnosis including myelography, CT-scan and MRI are presented. The possible pathophysiological mechanism responsible for the delayed onset of the sequelae of the tethered cord are discussed. It is concluded that neurosurgical therapy should be performed in cases of progressing clinical deterioration and the results of surgery in our patient group are presented.

[Radical surgery and vertebral body replacement in aneurysmal bone cysts of the spine].

6 cases of aneurysmal bone cyst of the spine are presented. The clinical picture, diagnostic procedures and operative treatment will be discussed. Although aneurysmal bone cyst is a benign, non-neoplastic lesion its spreading tumor growth with destruction of important bony structures of the vertebral body, consecutive instability and cord compression presents a considerable menace to the mostly teenage-patient. Spinal computerized tomography is the diagnostic method of choice. The goal of surgical therapy of the aneurysmal bone cyst of the spine should include radical tumor excision in order to avoid recurrent tumor growth followed by vertebral body replacement and osteosynthetic stabilisation.

The expression of angiogenin in tissue samples of different brain tumours and cultured glioma cells.

BACKGROUND: As a potential angiogenetic factor the 14.1 kDa polypeptide angiogenin induces neovascularisation. MATERIALS AND METHODS: We investigated the angiogenin expression by immunoblotting and an ELISA in 60 tissue specimens (40 gliomas, 20 other intracranial tumours), in 22 glioma cell cultures and in 4 supernantants of cultivated glioblastoma cells. RESULTS: We could show that angiogenin is detectable in different kinds of intracranial tumours with the highest amount in meningiomas and the lowest amount in low grade astrocytomas. In tissue specimens, a significantly higher angiogenin expression was measured in meningiomas compared to gliomas and metastases. Angiogenin could be detected in primary cultivated glioma cells, but not in the permanent cell lines. There was a significant correlation to the malignancy within the gliomas with an increase of angiogenin concentration according to the higher grade of malignancy. CONCLUSIONS: Our data suggest that angiogenin may contribute to the malignant transformation of gliomas and could perhaps advise that the physiological role of angiogenin is not restricted exclusively to angiogenesis. Based on these findings the clinical importance of angiogenin for therapeutic decisions in malignant brain tumours remains unclear and further analyses on m-RNA-levels are required.
