[Comparative effectiveness of lotensin and capoten in patients with chronic cardiac failure].

AIM: To compare clinical response to captopril (capoten) vs benazepril (lotensin) in patients with chronic cardiac failure (CCF) as well as their influence on central hemodynamics, some indices of platelet hemostasis, myocardial ischemia degree, exercise tolerance. MATERIAL AND METHODS: 54 patients with CCF (NYHA FC II and III) entered the trial. 26 patients received captopril (capoten) in a dose 25-75 mg/day (group 1) while 28 patients were given benazepril (lotensin) in a dose 5-30 mg/day for 4 weeks (group 2). Group II was treated for the following 24 weeks. The results were assessed with electro- and echocardiography, bicycle exercise test, platelet aggregation measurement and by clinical symptoms. RESULTS: A positive clinical response was registered to both the drugs which improved the functional class, exercise tolerance, platelet aggregation, reduced the number of arrhythmia and myocardial ischemia episodes. Long-term treatment with lotensin resulted in further improvement of clinical and laboratory indices. Side effects, in which lotensin discontinuation is needed, were absent. Lotensin was more potent than capoten in reducing episodes of ST expression on ECG and episodes of painless myocardial ischemia.

[Mental disorders in patients with arterial hypertension with metabolic disturbances].

A battery of psychological tests has been used to study the psychic state and quality of life in 75 patients with mild and moderate arterial hypertension (AH) with or without (controls) different metabolic disturbances (diabetes mellitus, dyslipidaemia, obesity). Comparing to controls, the AH patients with metabolic disturbances suffered more frequently from different psychic disorders of neurotic register and their quality of life, general condition, activity and mood proved to be significantly worse. The authors conclude that psychological testing may be an important diagnostic tool in general hospital. It allows identification of the borderline mental disorders features, the correction of which appears to be of great importance for basic disease course and its therapy.

[Psychological traits and quality of life of patients with hypertension and metabolic risk factors].

Psychological status and quality of life were assessed in 75 patients with mild and moderate hypertension and metabolic disturbances. Patients with hypertension and metabolic risk factors compared with patients without these factors were characterized by higher frequency of neurotic disorders, worse quality of life and mood, lower activity. It is suggested that psychic disorders which are rather frequent in patients with hypertension affect negatively clinical course of underlying disease, complicate clinical picture of hypertension, and cause patient's denial of continuous antihypertensive therapy.

[Effects of nebivolol on microcirculation, platelet aggregation and blood viscosity in patients with essential hypertension].

AIM: To assess effects of a selective beta1-adrenoreceptor blocker acting in part via nitric oxide nebivolol on microcirculation, platelet aggregation and blood viscosity. MATERIAL: Thirty patients with mild-to-moderate essential hypertension (mean age 54,1-/+9,3 years). METHODS: Ambulatory blood pressure monitoring, laser doppler flowmetry, registration of spontaneous and ADP-induced platelet aggregation. Nebivolol (5 mg/day) or its combination with hydrochlorothiazide (12,5 mg/day) were given for 3 months. RESULTS: Treatment with nebivolol caused significant reduction of systolic and diastolic blood pressures. Spontaneous and low dose ADP-induced platelet aggregation significantly decreased during treatment while blood viscosity did not change. In patients with hyperemic and spastic-hyperemic hemodynamic type of microcirculation improvement of microcirculation occurred due to changes of myogenic tone of arterioles and precapillary sphincters and 'damping' properties of resistance arterioles. CONCLUSION: In patients with essential hypertension hypotensive action of nebivolol was associated with antiaggregation effect and improvement of blood flow in microvascular networks.

[Clinicofunctional features of arterial hypertension in chronic broncho-obstructive syndrome].

AIM: To describe clinicofunctional features of essential and pulmogenic hypertension in chronic bronchoobstructive syndrome, 24-h profile of arterial pressure (AP), intracardiac hemodynamics and to propose differential diagnostic criteria for these hypertension forms. MATERIALS AND METHODS: 24-h monitoring of arterial pressure (MAP), cardiohemodynamics, external respiration function (ERF) and blood gases examinations were made in 100 hypertensive subjects with chronic obstructive bronchitis and bronchial asthma. RESULTS: Significant differences were found between the groups of essential and pulmogenic hypertension by major values of MAP, echo-CG and ERF. Early disturbances in diastolic function of both the ventricles in essential and pulmonary hypertension in chronic bronchial obstruction arise long before clinical, x-ray and ECG signs of chronic pulmonary heart. CONCLUSION: The findings enable formulation of criteria of differential diagnosis of essential and pulmogenic hypertension and identify the latter as an independent nosological entity.

[The effect of a new hypolipemic preparation fluvastatin (Lescol) on rheological indices and hemostatic parameters].

In view of inducing action of hyperlipidemia on progression of nephropathy and relationships between thrombogenesis, atherogenesis and sclerosis, the authors examined fluvastatin effects on platelet-rheological hemostasis. The 12-week course in a dose 20-40 mg/ day produced minimal side effects while its hypolipidemic action was noticeable: a 18, 21 and 20% fall in concentrations of total cholesterol, LDL cholesterol, triglycerides, respectively. The platelet rheology underwent the following changes: spontaneous platelet aggregation went down from 2.58 +/- 0.3 to 1.64 +/- 0.27 r.units, ADP-induced platelet aggregation rose from 6.5 +/- 0.66 to 8.08 +/- 0.77 r.units. No marked changes were registered in hematocrit, plasma and blood density, red cell aggregation and deformability. Thus, active lowering of blood lipids was not associated with evident inhibition of platelet activity. The absence of this feedback in lipid-platelet relations probably indicates an independent significance of hemostatic disturbances in ischemic heart disease and needs further study.

[The therapeutic correction of disorders in thrombocyte-vascular hemostasis and of changes in the rheological properties of the blood in patients with arterial hypertension].

158 patients with essential hypertension received beta-adrenoblockers and were exposed to travelling impulse magnetic field, magnetolaser radiation. The study of platelet-vessel hemostasis and blood rheology revealed a relation of good clinical response and increased exercise tolerance with initial platelet dysfunction and rheological disorders which underwent positive changes in the course of treatment.

[Thrombocytic-vascular hemostasis and the rheological properties of the blood during the tenolol treatment of hypertension patients].

A total of 80 patients aged 34 to 65 years who suffered from stage-II hypertensive disease were followed up. The selective beta-adrenoblocker tenolol was used alone. Platelet hemostasis, cyclic nucleotides, thromboxane-prostacyclin system and blood rheological properties were examined. Tenolol was found to have a pronounced antihypertensive effect in the patients and to be beneficial in correcting impaired blood rheological properties and platelet homeostasis. It is concluded that the baseline blood rheological properties and the status of the hemostatic system largely determine the natural history of hypertension and the efficacy of therapy.

[The use of emakor in arterial ischemia of the lower extremities in outpatient practice].

Overall 43 outpatients with arterial ischemia of the lower limbs were treated by emakor, a new Soviet drug that exerts a concomitant alpha- and beta-blocking action. The treatment resulted in a remarkable positive effect which manifested itself in amelioration of circulation in the lower limbs, positive time-course of changes in peripheral hemodynamics and in computer-aided tomography data. The use of emakor is contraindicated in exacerbation of gastrointestinal diseases, chronic pulmonary diseases associated with the obstructive syndrome, and marked signs of venous outflow obstruction in the lower limbs.

[Role of hemorheologic and hemodynamic factors for the efficacy of therapeutic plasmapheresis in patients with ischemic heart disease].

Changes in blood rheological properties and central hemodynamic parameters were studied in patients with Functional Classes III-IV angina pectoris who had undergone therapeutical plasmapheresis. Drastic baseline changes in blood rheological properties, a reduction in myocardial contractility, and high total peripheral vascular resistance (hypokinetic circulation) were found to be indications for plasmapheresis supplemented to antianginal therapy. A positive clinical effect of this therapeutical method may be, to a definite extent, predicted in these patients. The first plasmapheresis procedure improved blood rheology due to the plasma component, making the high baseline levels of fibrinogen and total protein normal, lowered increased platelet aggregation, repeated procedures enhanced the electric superficial charge of red blood cells and the stroke index of the left ventricle.

[Effect of therapeutic plasmapheresis on rheological properties of blood in patients with ischemic heart disease].

Patients with angina pectoris, functional classes III-IV, were examined for changes in the rheological blood properties (red blood cell aggregation ratio, fluidity limit, apparent viscosity, electrophoretic mobility of red blood cells, fibrinogen, total plasma protein and ADP, induced platelet aggregation) under the effect of continuous plasmapheresis (PA). PA was established to produce a selective effect on the rheological blood properties depending on the initial level of its disorders. The most remarkable clinical effect was produced by PA in patients with initially high characteristics of hemorheology. The first PA session ameliorated hemorheology at the expense of the plasma component, reduced high platelet aggregation whereas the repeated sessions largely affected red blood cell function.

[Changes in the platelet-vascular system of hemostasis in patients with ischemic heart disease and hypertension after the treatment].

The status of platelet aggregability, cyclic nucleotides and the thromboxane-prostacyclin system was examined in 40 patients with Functional Class II-III angina pectoris treated with perdipine, a calcium antagonist, and in 40 patients with Stage II hypertensive disease treated with tenolol, a beta-adrenoblocker. The therapy was found to modify the initially abnormal parameters of platelet-vessel hemostasis both in coronary heart disease and hypertension. When the initial parameters of platelet functional activity were normal, the treatment with perdipine and tenolol caused no noticeable changes.
