Morphofunctional examination of the heart in rats with the alcohol withdrawal syndrome.

Using histochemical methods, light and electron microscopy, authors examined rat heart 2-6 hours, 1, 3, and 7 days after discontinuation of forced intoxication with alcohol. At the same time, they assessed the contractile function and creatine phosphokinase (CPK) activity in the isolated perfused heart, and the development of animal destruction. Ethanol withdrawal was followed by escalation of vascular disorders in the heart, dystrophic changes in the subcellular structures, considerable polymorphism in enzyme distribution and activity, and formation of foci containing disintegrating myocytes with contractures. The contractile function was impaired and CPK release increased in the isolated heart. The changes were most marked 3 days after ethanol discontinuation to disappear after 7 days. Two to seven days after ethanol cessation, 13.1% of rats perished. Cardiac injury due to alcohol withdrawal syndrome may be one of the factors leading to the development of alcohol cardiomyopathy and a cause of sudden death in patients with documented alcohol abuse.

[The effect of carnosine on the healing of a lung wound].

The influence of carnosine (beta-alanine, alpha-histidine) on the process of lung wound reparation was studied in 90 guinea-pigs. Its efficacy was evaluated microscopically, histologically and by means of electronic microscope. It was established that carnosine, as compared to controls nearly twice accelerates reparative processes in the injured lung by activation of fibroblast proliferation, connective tissue generation and intracellular regeneration. In type II epithelial cells more intensive formation of osmiophilic bodies and lamellar bodies contained in them is observed. The alveolus formation in the wound lips by the 7-8th day after lung injury is going on under the carnosine influence, which is due, probably, to massive excretion of osmiophilic contents from type II epithelial cells and surfactant production. This may be connected with rapid decrease of atelectasis and rapid restoration of lung airness within the wound.

[Experimental regeneration of the lungs].

In experiments on 150 guinea pigs, processes of lung wound healing were followed and the more active cellular sources of regeneration were identified. Measurements of cyclic adenosine monophosphate components in the course of lung healing indicated that histologic and electron-microscopic studies would be more informative if done at days 1, 3, 4, 7, and 14 after lung damage. Both cellular and intracellular mechanisms of regeneration were involved in the healing processes. The type II pneumonocyte be designated as a cambial cell of pulmonary parenchyma, for this cell displays the highest proliferative potency and is responsible for the epithelial lining of newly formed alveoli. The formation of these is shown to be assured through functional activity of all cellular alveolar elements (involving intracellular hyperplasia and hypertrophy of ultrastructures and especially dynamic morphological and functional alterations in osmiophilic bodies of type II pneumonocytes) and capillary neoformation.

[The interrelation of the "local" and the "general" in inflammation].

The relationships between the local and the general in inflammation are analysed basing on the literature and original data. Local chemoattraction is postulated to be an underlying factor initiating primary local cooperation of cells relevant to inflammation. Being essential in this cooperation, macrophage seems to warrant both the local developments and triggering of general mechanisms of regulation which are relevant to control over subsequent secondary cell cooperation. The latter is biologically aimed at localization of the inflammation focus and separation of its pathogenic factors from intact internal medium. General mechanisms of inflammation control are provided by neuroendocrine, immune, vascular, coagulative, fibrinolytic and other systems, and operate through the products of the acute phase, by immune defence factors and rearrangement of nervous regulation of homeostasis in intact organs and tissues. The result of the regulation manifests with sequential presentation of the inflammation stages in time, correlation of local and general responses intensity. Eventually, local inflammation and lesion involve stress and intoxication which are not considered direct attributes of inflammation, nevertheless can influence general regulatory systems concerned with the course of local inflammation. It is concluded that inflammation implies dialectic unity of local and systemic responses of the body outlined to resolve inflammation and restore homeostasis.

[An analysis of diagnostic discrepancies due to subjective factors in the clinics of Moscow].

Discrepancies between clinical and pathological diagnoses (classes 1-3) attributed to subjective factors have been analyzed for the period 1981-1985. The investigation pooled the data from three largest Moscow hospitals: S. P. Botkin, No. 1 and 7. Subjective misdiagnosis comprises from 34 to 45.6% of overall diagnostic discrepancies. As a rule, it resulted from inadequate clinical examination, underestimation of clinical evidence, overestimation of anamnestic data, misinterpretation of laboratory findings and those of consulting physicians as well as provided by adjuvant investigations. The errors of the above kind are most typical for patients over 60. The authors refer these diagnostic discrepancies to iatrogenesis.

[Seventy years of the Great October Socialist Revolution and the development of Soviet pathology].

In reviewing briefly the changes that have taken place during the past decade (1977-86) in the theory and practice of pathology and, in the theory and practice of pathology and, more particularly, in the Pathology Service within the overall Soviet Health care system, it should be noted first of all that this period has been productive with regard to both the practical work of the pathology departments in hospitals and the research activities of scientific institutions. Pathology is gaining importance as a basic biomedical discipline. Considerable successes have been scored in fundamental research not only by the central scientific institutions but also by laboratories for morphologic research and departments of pathology at educational establishments in most of the Union Republics. Many of such laboratories and departments are well equipped and conduct work at a high technological and methodological level in areas of high priority. The number of interdisciplinary research projects has increased. A series of monographs has been published, devoted to various aspects of methodology on medicine, to the relationship between form and function, or to a critique of erroneous theories. Scientific cooperation with pathologists of other Socialist countries has been expanding, as has the participation of Soviet pathologists at international congresses, symposia, and other meetings. An important achievement has been the establishment of a network of pathology departments of hospitals and of pathologic service bureaus. There is little doubt that these bureaus which have so far been set up on a pilot basis, will become an important component of the Pathology Service on which this will rely for research progress and for keeping abreast of technological developments. They are also called upon to promote better services in rural areas. The construction of new buildings for hospital pathology departments has been expanded, but the rate and magnitude of this activity cannot yet be regarded as satisfactory. Over the past decade, pathology laboratories at hospitals have become important diagnostic centers indispensable for accurate diagnosis of many diseases through examination of biopsy specimens, and they are now better equipped than before, although the improvements have been less spectacular than expected. The quality of postmortem and clinicopathologic analysis has improved. A notable event was the session of the Collegium of the USSR Ministry of Health devoted to the Pathology Service development.(ABSTRACT TRUNCATED AT 400 WORDS)

[Changes in the lung parenchyma in the early phases of bacterial toxemia and acute diffuse peritonitis in an experiment].

A comparative study of changes in the pulmonary parenchyma of guinea pigs with toxemia caused by intraperitoneal injection of staphylococcal toxin or Pseudomonas aeruginosa endotoxin and in those with peritonitis induced by i. p. injection of a suspension of human or guinea-pig feces has shown the changes to be rather similar in these conditions. In particular, as soon as 10 min after the onset of disease, signs of damage were detected in lung cells and capillary endotheliocytes of the blood-air barrier; later on, microthrombi were detected in pulmonary capillaries. In guinea pigs with feces-induced peritonitis, microfoci of pneumonia were seen. Following the injection of human fecal suspension, early signs of the sludge syndrome were present. The observed changes resembled those described for the adult respiratory distress syndrome (shock lung).

[Characteristics of the restructuring of the cardiomyocyte contractile apparatus in chronic ischemic heart disease].

The contractile apparatus of cardiomyocytes was examined at the ultrastructural level in 20 heart biopsy specimens obtained during operations for aortocoronary bypass in patients with chronic ischemic heart disease (IHD). Myofilaments were found to have sustained compensatory-adaptive and destructive changes as well as changes indicative of impaired intracellular regenerations under conditions of chronic hypoxia and energy deficiency experienced by the muscle cells, with the result that myocardial contractility was substantially reduced. Together, these processes led to progressive restructuring of both the contractile apparatus of the cardiomyocytes and the cytoarchitectonics in general. This combined with signs of increasing hyperfunction of the contractile myocardium, which made the energy deficit worse and thus interfered with plastic processes in, and diminished the structural and functional capabilities of, the myocardium. On the other hand, distinct changes were noted in the cardiomyocytes that reflected their adaptation to the adverse conditions created by progressing coronary atherosclerosis. It is concluded that therapy for patients with chronic IHD should include measures aimed at promoting such adaptive changes.

[Transbronchial biopsy in the diagnosis of sarcoidosis and certain other interstitial lung diseases].

Results of transbronchial pulmonary biopsy studies of patients of different sex and age suffering from interstitial pulmonary lesion are presented. Different diagnostic value of transbronchial pulmonary biopsy (TBBP) is shown in relation to different lesions. In case of sarcoidosis its informative value, for example, is high, while in the diffuse processes such as cryptogenic fibrosing alveolitis, the data allow only suggestive diagnosis and recommendation of an open lung biopsy. In chronic nonspecific pulmonary diseases TBBP helps to clarify the character and the activity of the process involved. In some cases it is possible to reveal a latent cancer. It is suggested to use the term "allergic alveolitis" (indicating the mechanism of development of changes mentioned) when such changes as alveolitis, small granulemas and intraalveolar outgrowths are found in the biopsy specimens.

[Role of neutrophils and macrophages in localizing purulent infection].

Early stages of bordering the focus of purulent and aseptic inflammation in man and in experimental animals are studied with a particular analysis of the role of polynuclear neutrophils (PNN) and macrophages (MP) in this process. Two variants of a purulent inflammation are studied: widespread diffuse (peritonitis) and circumscribed (abscess). Aseptic diffuse peritonitis and circuscribed inflammation of muscles are studied as control. Methods of light and electron microscopy are used. It is shown that in a purulent and aseptic inflammation a phylogenetically predetermined reaction develops which is directed to the localization and limitation of an inflammatory focus. This response is brought about by blood cells--PNN and MP--between which an interconnection exists stipulated by their function. PNN are the first to appear in the inflammatory focus and their main function is to reveal the presence or the absence of the infection, localization and destruction of bacteria. Macrophages appear in the inflammatory focus later, their function is in the circumscription of the focus and in the stimulation of the connective tissue capsula around the purulent zone or the development of scar in the aseptic inflammation.

[Alcoholic lesions of the brain].

Brains of 117 alcoholics admitted in the state of alcoholic coma or abstinence syndrome with an increase of blood pressure or development of acute psychosis were studied. The changes in the human brain were compared with those occurring in the brains of 30 rats with chronic alcoholic intoxication and formation of withdrawal syndrome. It is shown that in alcoholic coma and different manifestations of abstinence syndrome vascular and cellular changes take place, that can be attributed to the action of ethanol or its metabolites, as well as catecholamines. In the brain of the animals studied the same cellular and vascular changes were found as in the mentioned alcohol-induced manifestations in man. Progression of cephalic changes due to withdrawal syndrome is associated with more marked structural changes of the hemato-encephalic barrier.

[Rod-shaped inclusions in the heart and brain in the experimental alcoholization of rats].

Experiments were made on rat alcoholized by inhalation. Electron microscopy of the myocardium and brain cortex showed the dystrophically altered cells of these organs to contain unusual rod-shaped and sagittal cytoplasmic inclusions unrelated to organelles and having a certain structural similarity to alcoholic hyaline detected in men. The necessity of further studies to identify the inclusions in question is discussed.

[Young scientists--the future of pathological anatomy].

The question on the necessity of intensification of efforts of older generation scientists, those working at medical and research institutes, scientific pathology societies in the preparation and education of young scientific staff is raised. Some criteria for the evaluation of such a preparation and educational principle based on the deep study by young scientists of The Marxist-Leninist philosophy, formation of the communist mentality are discussed. The responsibility of leading pathologists of the country and the importance of such work is pointed out as those who are young to-day will have to put forward pathology at the XXI century.
