The incidence and epidemiological significance of Campylobacter jejuni/coli serotypes in Romania.

The distribution per serotypes of 401 Campylobacter strains was studied by Lior's procedure, 241 strains being identified as C. jejuni (60.1%) and 160 as C. coli (39.9%): out of these 329 were of human origin (82%), including 225 C. jejuni and 104 C. coli, and 72 were isolated from fowls and swine (16 C. jejuni, 56 C. coli). Out of the total of 401 strains, 363 (90.5%) were typable. Among human strains, 32 serotypes were identified, more frequently encountered being (in a decreasing order) 29, 4, 21, 2, 1, 9, 59, 44, 36, 48, 28, 8, 47, 5, 11, 57, 10, 32, 55 which sum up 89.5% of the 296 typable strains. Within C. jejuni species, serogroup 4 was the predominant one and for the C. coli one--29. Animal strains were distributed into 16 serogroups out of which 29, 26, 48, 55, 44 were more frequently encountered. The serogroups identified among animal strains were encountered, with a single exception, among human strains, too, but with a different frequency. The importance of serogrouping as an epidemiological marker, was confirmed by serogroup identity of the strains within some family foci, by the oneness, as serogroups, in the case of repeated isolations from the same patient as well as by establishing an epidemiological diagnosis, in the case of an epidemic, occurring among the members of a given community.

[Variations in plasma angiotensin-converting enzyme and renin in sarcoidosis].

A comparative study of plasma angiotensin-converting enzyme and renin activity was carried out in 55 patients with sarcoidosis and other pulmonary diseases. The converting enzyme activity was significantly increased in active sarcoidosis, showing a tendency to return to normal values after corticoid treatment, while plasma renin activity remained unaltered both before and after corticotherapy. The obtained results confirm the usefulness of the converting enzyme measurements, as already shown by others. Attention is drawn on the possible involvement of the pulmonary renin-angiotensin system in the local vascular and enzymo-chemical alterations, suggesting the possibility of associating the converting enzyme inhibitors to the corticotherapy of sarcoidosis.

[A comparative study of plasma converting enzyme and renin in sarcoidosis].

The paper reports on a comparative study of the conversion enzyme and plasmatic renin performed on a group of 55 patients, with clinical diagnosis of sarcoidosis and other lung diseases. Whereas the conversion enzyme showed important increases in active sarcoidosis and a tendency to the normal state after treatment with corticoids, the activity of the plasmatic renin remained the same, both before and after corticotherapy. The result obtained prove the practical usefulness of the dosage test of the conversion enzyme, in agreement with the literature data. They also point to the possible participation of the pulmonary SRA in the vascular and local enzymatic-chemical imbalances, suggesting the possibility of associating the conversion enzyme inhibitors to the treatment of sarcoidosis with corticoids.

[The microbiological diagnosis of chronic gastropathy associated with Campylobacter pylori].

The efficiency was studied, in experimental and practical conditions, of media for the isolation of C. pylori, as well as criteria for diagnosis and identification. The highest sensitivity was obtained with Columbia gelose, and a lower sensitivity was achieved with Campy I. C. gelose, and with heart-brain gelose, prepared as chocolate gelose, and Columbia gelose with integral blood. Both variants of the Mueller-Hinton gelose, even in the chocolate formula, gave negative results. Of the fluid media, supplemented with blood or with serum, only broth for I. C. blood cultures, and the heart-brain broth permitted the development of small cultures, while the soya-tripticase broth and the Mueller-Hinton broth remained sterile. None of the media, either solid or fluid, as such or supplemented with factors X, V, or X + V, did allow the development of C. pylori. In natural conditions, by inoculating 92 samples of gastric mucosa from patients with gastritis, with or without ulcers, confirmed histologically on Columbia chocolate gelose as such, or in a selective variant, 50 positive results were obtained with both variants, 9 positive results on the selective variant, and one positive result on the nonselective variant. The direct microscopic examination of samples from the mucosa disclosed the presence of C. pylori in all 60 samples that had also been confirmed by culture, as well as in another 21 samples from a total of 32 samples with negative cultures (88%). The direct urease test performed directly from the sample was done in 90 cases and was positive in 64 out of 79 samples that had been confirmed bacteriologically (77.2%), but in none of the 11 negative samples. The positive prediction index is thus of 100%, and the negative index is of 37.9%. Procedures for identification consisted in a definition of morphological characteristics, and in the cultivation, and biochemical features (including catalase and oxydase determination), which can define the Campylobacter genus, and the urease test, the sensitivity to cephalotine and the resistance to nalidixic acid for the differential species diagnosis. In conclusion cultivation of C. pylori presumes the use of media with superior quality peptones, supplemented with blood lactate in the gelose chocolate formula. The direct urease test is very useful as a procedure for diagnosis, and this can be done in the departments of gastroenterology.
