[The role of the exercise test in patients with ischemic heart disease in estimating the risk of sudden death].

The authors evaluated after 4-year interval the results of coronarographic and spiroergometric examination after a work load on a bicycle ergometer up to the limiting work syndrome in 51 all patients with the aim to reveal "risk factors" of sudden death in CHD patients. The following parameters were recorded: age, duration of treatment number of cigarettes smoked, systolic blood pressure at rest and the daily dose of the most frequently used drugs. After the work load the limiting values of pulse rate, systolic blood pressure, oxygen consumption, blood lactate level and the work ECG were evaluated. In the group of 7 sudden deaths of CHD patients the authors found statistically significant (p less than 0.001) a lower mean limiting value of oxygen consumption/kg and a deeper (p less than 0.0001) mean depression of the S-T segment in the work ECG when compared with 35 CHD patients surviving for 4 years. Both indicators are considered the main "risk factors" for sudden death.

[The significance of changes in fibrinogen levels during thrombolytic therapy in myocardial infarct].

The submitted work was concerned with two problems: a) investigation of the dynamics of changes of the fibrinogen and fibrin breakdown products after a single dose of streptokinase, b) evaluation of the effectiveness of thrombolysis from changes in the fibrinogen concentration. Two groups of patients were examined with the Q form of acute myocardial infarction who were treated at an intensive care unit. The first group comprised 62 patients (mean age 56.3 years) who were given 1 million units of streptokinase by the i.v. route. The second group comprised 52 patients who were given 1.5 million units of streptokinase. The authors investigated the fibrinogen plasma concentration and the fibrin breakdown product concentration in serum after 3-hour intervals. Concurrently they examined the creatine kinase curve. They proved a significant increase of the fibrinogen and fibrin degradation products before thrombolysis (p less than 0.001). The fibrinogen concentration declined in both groups highly significantly with a maximum after 12 hours. The fibrinogen level reached normal levels in both groups after cca 24 hours. Patients with a high initial fibrinogen level (above 5 g/l) did not attain hypofibrinogenic values during thrombolysis. This phenomenon did not depend on the size of the streptokinase dose. The decline of fibrin breakdown products in both groups declined to baseline values within 24 hours. The authors confirmed that the effectiveness of thrombolysis is influenced among others also by the residual fibrinogen value.

Changes in the activity of thrombocytes by smoking.

Platelet function was investigated in 11 healthy non-smokers, 11 medium smokers (less than 10 cigarette/day) and 11 heavy smokers (greater than 20 cigarette daily). Cigarette consumption was in medium smokers 6.2 cigarette/day, and in heavy smokers 30.7 cigarette/day. Erythrocyte count, platelet count, plasma levels of cholesterol and triglycerides, and circulating platelet aggregates showed no significant difference between smokers and non-smokers. Thromboxane B2 level in plasma was higher in heavy smokers (20.1 +/- 17.3 pg/0.1 ml) than in non-smokers (58.8 +/- 37.1 pg/0.1 ml) and medium smokers (65.4 +/- 32.1 pg/0.1 ml) (P less than 0.01). Platelet factor 4 level was high in non smokers prior to smoking 1 cigarette and normal in medium and heavy smokers. Plasma concentration of beta thromboglobulin and 6-ketoprostaglandin F1 alpha showed no significant changes between non-smokers and heavy smokers. No dependence on smoking 1 cigarette was found in any group. The data suggest that long term cigarette smoking increases platelet activity manifested by an increased activity of thromboxane A2. Cigarette smoking contributes to the development of coronary artery disease also by means of activation of platelet functions.

[Screening for venous thrombosis and pulmonary embolisms in patients with stroke using an adaptive classifier].

The use of adaptive linear neuron is referred to in recognizing veinous thromboses and lung embolism in a total of 88 acute palsies. As the input signs, the accessible clinical and laboratory data were used. Both training and adaptation of neuron have been performed in a part of total [35 patients], and 53 patients were submitted to the classification of patients with unknown distribution. In classified group with present complications, the screened diseases have been recognized in 16/20 patients. In 29 from 33 patients without thromboembolic states, the distribution by classificator appeared as correct one. The level of correct resolution was as high as 88% in all.

[Electrophysiologic effects of orally administered amiodarone].

The authors made an invasive electrophysiological examination of the heart before and after administration of amiodarone by the oral route in 18 patients (15 men and 3 women). Amiodarone was administered in maintenance doses of 200 or 400 mg/day for 3 to 13 months (mean 4.7 months). Comparison of values revealed a significant protraction of R-R, P-Q and QRS intervals on the surface ECG. A-H and H-V intervals on the electrogram of the bundle of His as well as effective refractory periods of the ventricular myocardium were significantly protracted. During high-frequency stimulation of the atria the frequency at which an a-v block grade II type I develops declined. No significant changes developed in Q-Tc intervals on the surface ECG, the PA-A on the electrogram of the bundle of His and in the corrected recovery period of the s-a node.

[Monitoring the effectiveness of amiodarone in the treatment of ventricular ectopic arrhythmias].

The authors included in their investigation a total of 18 patients (15 men and 3 women) with electric instability of the ventricular heart muscle during programmed stimulation of the ventricles. They administered amiodarone to the patients by the oral route, 200 or 400 mg per day for a period of 3 to 13 months. They compared the subjective symptomatology, the finding on monitoring of the ECG with the patient in bed or by means of Holter's system and the results of programmed stimulation of the ventricles before and after amiodarone administration. There was a marked decline in the frequency of subjective symptoms reported by the patients (above all syncopes disappeared completely) and there was a marked decline or complete disappearance of serious ventricular ectopic dysrhythmias. Amiodarone eliminated electric instability of the ventricles as proved by programmed stimulation of the ventricles completely in 39% and partly in 11% of the subjects.

[Acute phase proteins in acute myocardial infarct].

The authors investigated changes of proteins of the acute stage (transferrin, prealbumin, orosomucoid, alpha-1 antitrypsin) after the development of acute coronaries. Blood specimens were collected from 84 patients with the diagnosis of acute myocardial infarction (AMI) on the 1st, 3rd, 7th and 21st day of hospitalization. The authors observed a significant reduction of transferrin and prealbumin values on the third day after the development of AMI and a significant rise with a maximum on the 7th day in orosomucoid and alpha-1 antitrypsin (p less than 0.05). The assessed changes are consistent with data reported in the literature. When the patients were divided into groups with AMI type Q and AMI type non Q, the authors observed a significant rise of alpha-1 antitrypsin on the 7th day in group AMI type Q (p less than 0.01). In patients with AMI type non Q alpha-1 antitrypsin was within the normal range, while in group AMI type Q it was on the 3rd and 7th day beyond the normal range. In the remaining proteins of the acute stage no significant differences were found on comparison of the two groups.

[Relation between the surface ECG and the bundle of His electrogram in 200 patients].

The relationship was assessed between different types of conduction disorders in surface ECG and pathological changes of intervals in electrograms of the His bundle during spontaneous cardiac action and during artificial right atrial stimulation in 200 patients (130 men, 70 women). For the detection of prognostically less favourable intra- and infraHisal conduction disorders it was found important to study the bundle of His electrographically in the following types of surface ECG: complete block of the right Tawara node with left posterior and left anterior hemiblock, complete block of the left Tawara node, left posterior hemiblock, atrio-ventricular block of the 1st degree (particularly when associated with intraventricular conduction disorder), IInd - degree atrio-ventricular block 2:1 (3:1), less so atrio-ventricular block of the IIIrd degree. Even apparently normal surface ECG can sometimes conceal major damage to the His-Purkinje system. Invasive electrophysiological tests are justified by the presence of syncopes or their equivalents due to unknown causes.

[Differential diagnosis of acute myocardial infarct in hypothyroidism].

The authors demonstrate the case of a female patient with hypothyroid myopathy where they recorded repeatedly a rise of enzymes used for the diagnosis of acute myocardial infarction, whereby myocardial infarction was not revealed. The authors followed up the disease for two years. They evaluated the relationship between the state of substitution and the activity of the mentioned enzymes and myoglobin.

The development of symptomatology and functional condition in patients with negative or minimal findings on coronary vessels after 5 to 10 years.

The authors compared two groups of men: the group with minimal sclerotic wall changes (n = 18) and the group with a normal coronary angiographic finding (n = 32). All the examinations were repeated after 5 to 10 years (mean 8.4 years). The two groups did not differ in the occurrence of the main IHD risk factors, complaints, kind and amount of principal drugs and working history. No statistically significant differences were found between the groups in the reasons for discontinuing bicycle ergometric test or in the exercise ECG finding. The two groups studied did not differ in the mean values of limiting metabolic and cardiologic parameters (oxygen consumption, lactate, pulse rate, RPP index) either. The conclusion can be made that in the functional and clinical prognosis the group of symptomatic men with minor sclerotic wall changes does not differ practically from individuals with completely normal coronary angiography.

[Sensitivity and specificity of programmed stimulation of the cardiac ventricles].

The authors used programmed pacing of the ventricles in a total of 55 subjects (41 men and 14 women). In a group of 36 patients with chronic relapsing ventricular tachycardias they assessed the sensitivity of this method as 83.3%. In 19 subjects without serious organic heart disease and without clinically confirmed tachyarrhythmias programmed pacing of the ventricles did not reveal diagnostic ventricular tachycardias. The specificity of the method was 100%. The highest detection rate of diagnostic ventricular tachycardias was obtained when four extrastimuli were applied during the sinus rhythm and 2-4 extrastimuli, using the basic stimulation rate of 100/min.

The effect of bopindolol on lipids and platelet aggregation.

An open ambulatory study was performed to assess the effect of bopindolol in 19 hypertensive patients (W.H.O. stage I-II). A statistically significant decrease in systolic and diastolic pressure as well as heart rate (never below 60 beats per minute) was demonstrated. No undesirable subjective side-effects of the drug were reported. Compared with the serum concentrations after placebo, a decrease in all low-density fractions was observed after 12 months. The total cholesterol/HDL cholesterol ratio did not deteriorate and the apolipoprotein A/apolipoprotein B ratio increased. The values of triacylglycerols during 8 months of therapy did not exceed initial serum concentrations following placebo administration. Patients treated with bopindolol exhibited decreased platelet aggregation due to collagen.
