[Favorable outcome of recurrent schizophrenia].

The work was performed on a material of a non-randomized group of patients with attack-like schizophrenia registered (January 1976) at the psychoneurological dispensary of one of the districts of Moscow. Using the method of a clinico-epidemiological catamnesis as well as a definite method of determining the arrest of the process of attack formation in a group of patients, 641 patients could be distinguished. The condition of those patients was qualified as "outcome", "++post-morbid". The rate of such cases in the whole patients' group amounted to 62.5%. The data on the clinical structure of the outcomes, including productive psychopathological and negative symptomatology as well as the level of social and work adaptation of the patients enabled one to distinguish groups with a favourable outcome (471 cases) and unfavourable outcome (170 cases), which accounted for 46.0 and 16.6% of the entire non-randomized group of patients. Therefore, based on the populational regularities of the course of attack-like schizophrenia the authors succeeded in demonstrating a possibility of arresting the process of attack formation at any disease stage, to determine the rate of such cases in the patients' population as well as to objectively prove the domination of a favourable variant of outcome of the morbid process.

[Mental disturbances in the victims of the earthquake in Armenia (based on examination data in the medical institutions of Moscow)].

As a result of examining 211 persons with severe somatic pathology (the crush syndrome, multiple fractures, spinal cord injuries, and so forth) received during the earthquake, 179 (84.8%) manifested psychopathological disorders. Of these, 70 persons (33.2%) had asthenia, consciousness disturbances, insomnia which were viewed as somatogenic, 17 persons (8.1%) with craniocerebral injury had the signs of the establishment of the psycho-organic syndrome, and in 109 persons (51%), the clinical picture was determined by psychogenically conditioned depressive, anxious-phobic and psychopathic reactions. The data obtained allow one to judge the characteristics of the mental status during the subacute period (from the first to the second week) after the earthquake.

The effect of sodium nucleinate on allergic and immunological reactions.

Experiments on immunized rabbits and guinea pigs indicated that sodium nucleinate (SN) was capable of weakening or entirely eliminating anaphylactic and skin reactions of delayed type hypersensitivity to repeated administration of staphyloanatoxin, APDT vaccine. The findings on patients with the infectious form of bronchial asthma and chronic rheumatism showed that sodium nucleinate attenuated reactions to the subcutaneous administration of staphylococcal and streptococcal allergens. The treatment of patients suffering from infectious-allergic bronchial asthma and rheumatism with SN resulted in the recovery of deficient T cells, T-suppressors, normalization of immunoglobulin concentrations. In children with acute glomerulonephritis sodium nucleinate normalized decreased T-suppressor cells and increased IgG and circulating immune complexes (CIC), resulting in a pronounced remission of disease. The mechanism of desensitization and elimination of CIC by SN has not been explored, however, the parameters of SN-induced immunomodulation are known rather completely. It is suggested that SN brings about accumulation in the cell of cyclic AMP which diminished membrane permeability, activates monoaminooxidase resulting in the degradation of histamine and other biogenic amines, enhances the synthesis of endogenous corticosteroids with their desensitizing properties. All these effects contribute to the elimination of delayed type hypersensitivity. The role of SN in the inhibition of delayed type hypersensitivity remains obscure.

[Study of the incidence of single-attack course of schizophrenia based on epidemiological data].

A non-randomized sample of patients suffering from attack-like schizophrenia and registered at the PND of 3 districts of Moscow served as materials for investigation. There were altogether 3576 persons. Patients living in the Proletarsky district were subjected to a catamnestic study. By the moment of examination they had suffered one attack. In view of the fact that the occurrence of the second attack obeys the law of exponential distribution, it was determined that the mono-attack course was recorded in 26.2% of all the registered patients with attack-like schizophrenia. It was shown that the proportion of patients with mono-attack schizophrenia increased with the disease standing. The data provide an opportunity of the probability forecasting of the disease stabilization after the first attack and can be used in the solution of the problems of the further disease course and registration of this patients, group.

[Various patterns in the duration of paroxysms and remissions among patients with schizophrenia and manic-depressive psychosis with monopolar depressions].

Characteristics of the course of endogenous psychoses (schizophrenia and manic-depressive psychoses) with a pattern of depressive attacks were studied on randomized material, using a clinical-statistical method. The duration of paroxysms and remissions was shown to decrease with the progression of disease; differences between nosological groups with regard to lowered duration of remissions were detected. New statistical regularities in the distribution of duration of paroxysms and remissions were elicited which corroborated the data obtained and also allowed the researchers to determine the probability of the cessation of paroxysms and remissions of any ordinal number and to evaluate the representativeness of the group of patients under study.

[Clinico-epidemiologic characteristics of endogenous psychoses with depressive episodes].

A randomized sample of patients with endogenous psychoses marked by depressive attacks was examined by the clinicoepidemiological method. A comparative analysis of the major clinico-statistical characteristics, constituting this group of diagnostic forms, was carried out, showing a considerable homogeneity of the underlying pathogenetic mechanisms. Such clinical signs as the premorbid personality make-up of the patients, the age at the onset of the disease, the character of the premanifest period, the structure of the depressive syndrome, and the sex of the patients were disclosed to be of prognostic value.
