[Failed postpartum sterilisation--a comparison of 5 methods].

A comparison of the standard Vienna method of postpartum sterilisation (3,580 patients with 18 failures and a sterilisation failure rate of 5.03/1,000) with 4 other methods at Paarl Hospital is presented: the Pomeroy method (892 patients with 19 failures and a sterilisation failure rate of 21.31/1,000); the total fimbriectomy method (1,578 patients with 23 failures and a sterilisation failure rate of 14.58/1,000); the Filshie clip method (808 patients with 18 failures and a sterilisation failure rate of 22.28/1,000); and the Irving method performed at caesarean section (456 patients and 1 failure). As no totally permanent technique exists to prevent pregnancy, a plea is lodged for legalised abortion after failed sterilisation.

Postpartum sterilisation and demographic progress at Paarl Hospital.

The success of the postpartum sterilisation campaign at Paarl Hospital, CP, irrefutably supports the claim that a quality family planning programme can in itself reduce fertility. In 1971 only 10% of women undergoing sterilisation had 4 or fewer children--this incidence increased to 71% in 1986. Women with more than 10 children are now very rarely found--in 1970 they still accounted for 20% of all patients sterilised. Parity at time of sterilisation has levelled to about 4 in contrast with 7.52 in 1971. It is probable that as many as 15,000 unwanted and unplanned pregnancies have been prevented in Paarl as a result of this sustained effort. The ideal of the 2-child family is increasingly possible.

[Postpartum sterilization and the private practitioner].

A postpartum programme of sterilization was initiated by private practitioners at the Paarl Hospital in 1968. The unit was later taken over by the University of Stellenbosch and a total of 4704 procedures had been completed by the end of 1983. Most of the doctors trained in the method are now in private practice. The sterilization-to-delivery ratio of 1 in 5 indicates that +/- 200 000 postpartum sterilizations would be requested in South Africa, if the Paarl figures are projected to the rest of the country. The most effective method of mobilizing our medical manpower would be a fair fee per procedure. South Africa cannot afford the present continued burden of unwanted and unplanned births.

[Postpartum sterilization and maternal mortality in Paarl Hospital].

A single maternal death associated with postpartum sterilization has been encountered at Paarl Hospital among the 3 742 such procedures performed between January 1972 and December 1982 (19,75% of all the mothers delivered were sterilized). The safety of postpartum sterilization is confirmed. A strong plea is voiced for the expansion of this procedure on a massive national scale because of its simplicity and effectiveness.

[Postpartum sterilization with the Filshie titanium silicon rubber clip].

This provisional report on 101 postpartum patients indicates that Filshie clip application to the fallopian tubes is a satisfactory method for postpartum sterilization. Particular advantages are speed and the minimal damage of tissue, with possibly the best present potential for reversal. When compared with the Vienna method of tube sterilization, however, the extra cost militates against its general application. In selective cases, such as patients with two children who request sterilization, it appears to be the best method available today.
