A stroke unit in a medical department. Organization and the first 100 patients.

A non-intensive stroke unit of 10 beds has been started in the Medical Department, Serafimerlasarettet, Stockholm. The aim is to make diagnostic and therapeutic studies in unselected stroke patients. Patients with suspect cerebrovascular disease in the Casualty Department are admitted to the unit non-selectively and without any age limit. Relevant physical findings and laboratory data are followed and registered by code on special charts to make evaluation by computer possible. A preplanned investigative programme is adhered to. Strict criteria for diagnosis and treatment are followed. The experience and results from the first 100 patients treated in the Stroke Unit indicate that the unit is a good basis for both education and research.

The hypothenar radial arch and parathenar loop in palmar dermal topology: sex, bimanual and regional variation in samples from the British Isles.

Consideration is given to the regular recording of parathenar distal loops (Pa) and hypothenar radial arches (Ar) in palmar dermal topology. Frequencies are given for four British populations (the North Pennine Dales, the Mid Welsh Borderland, Pembrokeshire and Ireland) and regional variation noted together with sex and bimanual differences. Both Pa and Ar occur infrequently in normal populations, generally in less than 5% of individuals, Pa significantly less than Ar. Both attributes are similar in that they are significantly more common on right than left hands and in females than males. Three out of twelve comparisons between the North Pennine Dales, Welsh Borderland and Pembrokeshire data attained formal significance, in each case in the males. The former two regions were distinguished for both Pa and Ar; the latter two for Ar. This is more than expected by chance at the 5% level.

ABH secretion in the population of Pembrokeshire, Wales.

Saliva samples collected from 482 Pembrokeshire school-children were tested for secretor status. The Pembrokeshire gene frequencies agree closely with those available for other Welsh samples. Moreover, there are no statistical differences between the secretor-status frequencies reported in north and south Pembrokeshire.

Accuracy of bedside diagnosis in stroke.

The clinical diagnosis of the type of acute cerebrovascular diseases is often considered unreliable, although this has not been validated prospectively in representative patients. The accuracy of bedside diagnostics was, therefore, tested in 206 patients consecutively admitted to the Stroke Unit of the Serafimerlasarettet in Stockholm. Bedside diagnosis turned out to be correct in 69%. In 24% the diagnoses were altered after hospital investigation and in the remaining 7% no defined preliminary and/or final diagnosis could be made. When the diagnoses were considered "fairly certain" they were accurate in 87%, compared to 53% when regarded as only "probable". The diagnostic accuracy improved during the period studied. Sensitivity in identifying hemorrhages was much lower (39%) than for cerebral infarctions (83%). It is suggested that new investigational methods should be compared with what can be accomplished with bedside methods alone.

Disablement and quality of life after stroke.

This study concerns the quality of life of patients after stroke and how this is influenced by disablement and emotional factors. Ninety-six consecutive patients of mean age 71 years were followed for two years. At the end of that time 23% had experienced a recurrence of stroke and 27% were deceased. Of the survivors 76% were independent as regards activities of daily life (ADL) and lived in their own homes. Age as well as initial function were prognostically important factors. Patients who could participate in interviews marked on a visual analogue scale their evaluation of quality of life before and after stroke. Most of them had experienced a decrease and no improvement was observed during the two years. The deterioration was more pronounced in ADL dependent patients than among the independent. However, depression and anxiety were found to be of similar importance for quality of life as was physical disablement. These findings call for a greater emphasis on psychological support in the care of post stroke patients. The visual analogue scale can be a useful tool for detecting special needs.

The interaction of DNA-targeted platinum phenanthridinium complexes with DNA in human cells.

DNA-targeted platinum phenanthridinium complexes were investigated in intact human cells and in tumour-bearing mice. The DNA sequence specificity of platinum phenanthridinium complexes was examined in intact human cells using a Taq DNA polymerase stop assay. It was found that the platinum phenanthridinium complexes had a similar sequence specificity to that of cisplatin. However, the rate at which DNA was damaged in intact human cells was 6-fold greater for the platinum phenanthridinium chloride complexes compared with cisplatin. These results are consistent with a DNA-targeting hypothesis where the attachment of an intercalating group to cisplatin places the platinum in close proximity to DNA and increases the rate of DNA platination. Platinum phenanthridinium iodide complexes were also tested, but damaged DNA at a rate similar to cisplatin. The platinum phenanthridinium complexes with shorter linker chain lengths damaged DNA more efficiently than the longer linker chain length complexes. The platinum phenanthridinium chloride complexes also showed significant anti-tumour activity in tumour-bearing (P388) mice.
