Varicella-zoster virus IgG antibodies during primoinfection in competent and transfer factor modulated immunocompromised host: comparison of three indirect assays.

Ten patients with acute leukaemia and next three with Hodgkin's or non-Hodgkin's lymphoma, suffering from varicella-zoster-virus (VZV) primoinfection, were given 1 to 2 doses of ultrafiltrate of the human leukocytes lysate (LLU) containing transfer factor (TF) activity (1 dose being equivalent to the product of 10(8) leukocytes). Only LLU administered to patients with acute lymphocytic leukaemia (ALL) at early phases of the illness (days 1 and 2) displayed a notable benefit on the clinical course of varicella. No influence upon the infection, on the other hand, was observed following LLU administration to subjects with lymphoma. The convalescent levels of IgG antibodies to VZV, as detected by indirect immunoperoxidase assay to membrane antigen (IPAMA), demonstrated no significant difference between infected competent and immunocompromised untreated and LLU treated individuals. The performance characteristics of IPAMA are compared with indirect immunofluorescence method (IFA) and non-competitive enzyme-linked immunosorbent assay (ELISA) on the same panel of specimens.

Prevalence of HIV-antibodies in patients with haemophilia in Hungary.

Incidence of HIV antibodies have been studied in 617 patients with congenital bleeding disorders. Screening was performed with the Organon ELISA test, repeatedly positive samples were tested with four different confirmatory assays. HIV antibodies were found in 3/356 patients with haemophilia A, 21/114 patients with haemophilia B, 3/123 patients with von Willebrand disease and in 1/24 patients with other types of congenital coagulation disorders. All the 28 seropositive patients were treated with imported coagulation factor, mostly with factor IX concentrates. No patient treated exclusively with nationally produced factor preparations between 1981 and 1985 was found to be anti-HIV positive.

Early intrathecal production of specific IgM and IgG antibodies and alpha-interferon in herpes simplex virus encephalitis.

A complex approach was used in order to establish non-invasively the aetiology in three cases of encephalitis presumptively caused by herpes simplex virus (HSV). As indicative of brain HSV infection was considered the lowered serum to cerebrospinal fluid specific antibody ratio, which also assessed the humoral immune response within the CNS. For this purpose, during ongoing brain tissue infection, the early intrathecal (ITH) production of IgG and IgM antibodies was analysed by a differential enzyme-linked immunosorbent assay (ELISA) along with changing levels of complement-fixing (CF) antibodies in the serum and cerebrospinal fluid (CSF). Antiviral antibody (AA) response was markedly preceded by the appearance of alpha-interferon (IFN) in the serum and CSF. From the CNS biopsy and autopsy specimens one HSV-1 and one HSV-2 strain were recovered.

Focus assay for varicella-zoster virus in human embryo cells stained with immunoperoxidase method.

Rapid titration of varicella-zoster virus (VZV) in human embryonic fibroblasts (HEF) based on staining of virus-infected cells by indirect immunoperoxidase technique (IPA) is described. Cell monolayers were grown in wells of plastic plates (two different diameters). Foci of virus-infected cells as revealed by IPA could be counted either 48 hr post-infection, if cell-associated virus (VZV infected cells) was used as inoculum, or 72 hr p. i. if cell-free virus was used. A linear relationship was observed between virus dilution and number of foci. The first virus was detected 12 hr p. i., the highest titre at 36 hr, when cytopathic effect (CPE) involved about 50% of the monolayer.

This article outlines and stresses the importance of proper rehabilitation of hand injuries in athletes. Rationale for guidelines in splinting, modalities, and appropriate progression of various types of exercise is discussed.

Protective splinting for the hand and wrist.

Providing adequate protection is a very important component of proper treatment of athletic injuries of the upper extremity. This article reviews the anatomic considerations involved in fabricating splints for various joints of the hand and wrist that will allow the athlete to compete as early as possible while preventing reinjury or permanent disability.

Immunological alterations in anti-HTLV-III negative haemophiliacs and homosexual men in Hungary.

Hungary can be considered as a low risk area for AIDS since no patient with full-blown AIDS or AIDS-related complex has been found in the country. A complex clinical and immunological (T cell subsets, DNCB sensitization test, circulating immune complexes, acid-labile alpha interferon) investigation was performed between November, 1983 and June, 1984 in order to study whether alterations found in symptom-free homosexuals and haemophiliacs in the risk areas for AIDS can be observed in Hungary as well. 38 patients with mild haemophilia, 35 patients with severe haemophilia and 40 homosexual men were investigated in parallel to 37 heterosexual blood donors as controls. Anti-HTLV-III antibodies were measured later in the stored serum aliquots from the same subjects by the indirect membrane immunofluorescence assay. Although specific anti-HTLV-III antibodies were not detected in the haemophiliacs or homosexuals, immunological alterations characteristic for the members of AIDS risk groups in the high risk areas (decrease in the percentage of OKT4 cells and/or decrease of the OKT4/8 ratio) were found in one-third of the homosexuals and haemophiliacs tested. In addition, a significant part of these subjects did not develop delayed type hypersensitivity skin reaction on DNCB rechallenge. These findings indicate that an immunodeficiency independent of HTLV-III infection can be present in two major AIDS risk groups, in homosexual men and haemophiliacs.

Antigen-specific transfer factor from mice immunized with an attenuated flavivirus: augmentation of inducing activity in semipurified splenocytic dialyzates.

Three large batches were prepared of lyzed splenocytic leukocyte dialyzate from SPF outbred mice, immunized with a live attenuated virus from the tick-borne encephalitis (TBE) complex. Total mass of freeze-dried dialyzates was 1.73 g. One mg of respective batches contained 2 X 10(5), 2 X 10(4) and 2 X 10(3) units of the transfer factor, specific for the flavivirus group-antigen, as estimated according to the capacity to induce specifically cytotoxic T-cells in the recipient C3H mice. The amount of protein and orcinol-reactive material (purine-bound ribose), the presumed components of the inducer's substrate, ranged in individual dialyzates from 9.9-12.4 and 0.72-0.80% of their dry mass. Materials from each batch obtained after double precipitation by ethanol were subjected to permeation chromatography on Sephadex G-25 columns and subsequent lyophilization of the peak with specific inducing activity. The final product represented on average 3.7 per cent of dry mass of the starting material. In comparison to the crude material, in one mg of the final product the protein and the orcinol-reactive material were reduced by 80 and 37 per cent, respectively, but an increment in the antigen-specific inducing capacity comprising 2-3 log10 units was observed. These findings add to the concept that a) macromolecules carrying the inducing activity can be separated from other constituents of the crude dialyzate and b) an increase in antigen-specific inducing activity titre was, besides partial concentration, mainly due to removal of suppressor or inhibitory factor(s) present in the crude dialysates and probably acting in vivo.

UV-absorbance profile of human leukocytic ultrafiltrate after affinity chromatography on immobilized m-aminophenyl boronic acid: implication for transfer factor purification.

The UV-absorbance of the lysed human leukocyte ultrafiltrate (LLU) was investigated by Sephadex G-15 gel permeation chromatography before and after affinity chromatography on immobilized m-aminophenyl boronic acid. This biospecific fractionation caused substantial changes in 260/280-nm absorbance ratio of individual peaks as revealed by Sephadex G-15 gel permeation chromatography. While the material which had passed through the affinity support unretarded (W) appeared to be partly or completely deprived of peak IV, peaks IV and V were fairly enriched in the retarded material (R). This material, on the other hand, nearly completely missed peaks III, VI and VII. Peak III was copiously represented in material W. Boronate affinity chromatography caused enrichment in the protein content as well as in the content of purine/pyrimidine-bound ribose of material R, i.e. cis-diol group containing (RNA-like) material, considered to represent functional transfer factor activity. The described findings further support the use of immobilized derivatives of boronic acid in the separation of coplanar cis-diol group containing compounds from other constituents of LLU.

Traumatic partial arrest of tibial physis.

Two children with premature partial arrest of the tibial physis, one proximal and the other distal, were treated by resection of the bony bar and by the Langenskiold method of interposition of fat. In one child the arrest was the result of an open fracture and bony loss of the upper tibia. In the other child the arrest was caused by a Salter IV fracture. Preservation of the fat implant 20 months after operation was observed histologically. Correction of the deformity was accomplished in one case but not in the other.

Optimalized conditions of tick-borne encephalitis virus production in vitro.

Conditions of the efficient production of tick-borne encephalitis (TBE) virus in chick embryo cell (CEC) monolayer cultures were investigated. The prerequisites of high recovery of the produced virus appeared to be: the use of dense cell monolayers, reasonable frequency of harvests and a suitable buffering of nutrient media. Using low input multiplicity of infection, the peak of the serum-free virus production was observed between 24 and 48 hr post infection (p.i.). The procedure elaborated was useful for isotopic labelling of the virus.

Concentration of murine antigen-specific transfer factor of defined potency.

Dialysates containing transfer factor (TF) activity were prepared from lyzed splenic cells of SPF mice immunized with live, peripherally avirulent Langat virus (TP21 E5 "14" clone) from the tick-borne encephalitis (TBE) complex. The amount of TF was estimated by its capacity to generate in recipient inbred C3H mice cytotoxic T lymphocytes inducing lysis of TBE virus-infected target cells as demonstrated by 51Cr--release assay. A 100 to 1000-fold concentration of TF activity was achieved by combination of the two-step ethanol precipitation of crude dialysates with subsequent fractionation on Sephadex G-25 by exclusion chromatography. Materials from individual concentration steps showed reduced amounts of admixtures, as revealed by absorbance profiles of their chromatograms. In the final product the protein content was most decreased.

Restricted replication of the E5"14" clone of Langat TP21 virus (from the tick-borne encephalitis complex) in CNS of subadult mice.

Subadult ICR mice were infected with the low virulent Langat virus TP21 E5 strain clone "14" belonging to the tick-borne encephalitis (TBE) complex by subcutaneous (s.c.) and intracerebral (i.c.) routes. From 5 to 6 days post infection (p.i.), no virus was detected in cultured brain fragments of mice, which received 10(6) ic LD50 into interscapular area. Acute lethal encephalitis with lesions confined to the vicinity of the inoculation area (parietal cortex, basal ganglia, thalamus) has developed in all mice, which received greater than or equal to 3 PFU of the virus by i.c. route. However, no virus was recovered from the cultured fragments of brain stem and cerebellum of these animals, although direct isolation attempts were regularly positive from brain cortex and basal ganglia. Survivors, which did not succumb to i.c. administration of approximately equal to 1 ic LD50 (0.3 PFU) of the attenuated Langat strain were autopsied between 53-74 days p.i. Attempts to isolate the virus from cultured fragments of brain cortex and basal ganglia remained negative despite of the presence of focal residual histological lesions in g. hippocampi in 15% of of animals examined.

Dialysable specific transfer factor in mice immunized with attenuated Langat virus from the tick-borne encephalitis complex: generation, action and quantitative assay.

Cytolytic T lymphocyte assay was developed in order to measure the response of inbred C3H mice to dialysable specific transfer factor (STF), induced in subadult outbred mice by one shot immunization with the attenuated Langat virus. The first STF activity in mice splenic leukocytes was detected between 48-72 hr after virus administration. The conversion of splenic T-cell cytotoxic response in C3H mice in vivo occurred between 15-21 hr after STF administration. The killing activity of T-cells, induced by STF, showed cross-reactive traits within the genus Flavivirus. STF, given prior to the live virus, augmented the specific cytolytic T-cell response. In the live virus-primed mice the booster effect was markedly enhanced when administration of STF preceded the second immunization dose. In the serum of STF recipients, interferon was irregularly detected attaining low levels for short time periods. Temperature of 56 degrees C for 60 min abolished the activity of least 10(4) murine STF units, temperature of 37 degrees C lowered after 24 hr this activity by 3 log10units. Chromatography of the dialyzed leukocyte lysate on Sephadex G-25 column yielded usually five peaks. The second peak showed an increased content of ribose-bound and protein materials and, as a rule, a relatively concentrated STF activity.
