Leukotriene binding, signaling, and analysis of HIV coreceptor function in mouse and human leukotriene B4 receptor-transfected cells.

The mouse leukotriene B4 receptor (m-BLTR) gene was cloned. Membrane fractions of human embryonic kidney 293 cells stably expressing m-BLTR demonstrated a high affinity and specific binding for leukotriene B4 (LTB4, Kd = 0.24 +/- 0.03 nM). In competition binding experiments, LTB4 was the most potent competitor (Ki = 0.23 +/- 0.05 nM) followed by 20-hydroxy-LTB4 (Ki = 1.1 +/- 0.2 nM) and by 6-trans-12-epi-LTB4 and LTD4 (Ki > 1 microM). In stably transfected Chinese hamster ovary cells, LTB4 inhibited forskolin-activated cAMP production and induced an increase of intracellular calcium, suggesting that this receptor is coupled to Gi- and Go-like proteins. In Xenopus laevis melanophores transiently expressing m-BLTR, LTB4 induced the aggregation of pigment granules, confirming the inhibition of cAMP production induced by LTB4. BLT receptors share significant sequence homology with chemokine receptors (CCR5 and CXCR4) that act as human immunodeficiency virus (HIV) coreceptors. However, among the 16 HIV/SIV strains tested, the human BLT receptor did not act as a coreceptor for virus entry into CD4-expressing cells based on infection and cell-cell fusion assays. In 5-lipoxygenase-deficient mice, the absence of leukotriene B4 biosynthesis did not detectably alter m-BLT receptor binding in membranes obtained from glycogen-elicited neutrophils. Isolation of the m-BLTR gene will form the basis of future experiments to elucidate the selective role of LTB4, as opposed to cysteinyl-leukotrienes, in murine models of inflammation.

Expression of two isoforms of the third sarco/endoplasmic reticulum Ca2+ ATPase (SERCA3) in platelets. Possible recognition of the SERCA3b isoform by the PL/IM430 monoclonal antibody.

Human platelets express several sarco/endoplasmic reticulum Ca2+ ATPase (SERCA) isoenzymes: SERCA2b of 100 kDa apparent molecular mass and two distinct enzymes of 97 kDa, one of them identified as being the SERCA3a isoform. The molecular identity of the third enzyme specifically recognized by the PL/IM430 monoclonal antibody has remained elusive. First, the study of the 3'-end part of platelet SERCA3 mRNA, by means of RT-PCR amplification using sets of primers covering the N-3 to N (ultimate) exons of the human SERCA3 sequence, revealed the presence of two distinct mRNA sequences, SERCA3a and a longer variant. Second, this additional sequence was identified as SERCA3b and found to refer to the insertion of a new exon of 73 bp, located at bp 349 from the beginning of the intronic sequence, linking the penultimate (N-1) exon to the last exon (N) of the human SERCA3 gene. Third, a relationship between the expression of this SERCA3b mRNA and the PL/ IM430 recognizable SERCA protein was observed. SERCA3b mRNA was found to be absent in epithelial HeLa cells not recognized by the PL/IM430 antibody and the expression of this SERCA3b RNA species correlated with that of the SERCA protein recognized by PL/IM430 which was down-modulated in the platelet precursor megakaryocytic CHRF 288-11 cell line as well as upon in vitro lymphocyte activation. Taken together, these results strongly support the notion of the presence of the SERCA3b protein in human cells by showing SERCA3b mRNA in platelets and the fact that the protein corresponding to this mRNA species is very likely the 97 kDa protein recognized by the PL/IM430 antibody.

Perceptions, safety behaviors, and learning needs of parents of children brought to an emergency department.

INTRODUCTION: This study was conducted at a level II Pediatric Trauma Center to assess the perceptions, safety behaviors, and learning needs of parents who brought their children to the emergency department. METHODS: Surveys were distributed in the emergency department, and 412 parents/caretakers responded. A descriptive design provided the framework for data analysis. RESULTS: A profile of caretakers of children in three age groups (1 to 4, 5 to 12, and 13 to 15 years) emerged. Whereas most parents and older children knew how to call 911, only half of the parents knew child CPR. Parents tended to underestimate their children's risks for motor vehicle-related and immersion injuries and were more concerned about kidnapping and assault. Less than half of the parents believed that most injuries can be prevented. Learning needs were indicated by 34% of parents, and CPR was mentioned most frequently. Parents' desires for learning tended to focus on care after injuries happened. DISCUSSION: Health professionals need to spend more time teaching parents about the link between child development and risks for injury, to emphasize prevention. Interventions based on study results include the hospital Safety Helmet Discharge Plan.

Spatial distribution of myosin heavy chain isoforms and lactate dehydrogenase M4 in the limb musculature of two crossbred lambs.

Sixteen different skeletal muscle samples were distributed in the cross-section of eight hip and thigh muscles. Contractile characteristics were assessed by measuring myosin heavy chain (MHCI, MHCIIa, MHCIIb) composition by electrophoresis. Glycolytic capacity was estimated by immunochemical quantitation of the LDH-M4. Histochemistry was used as a reference. The MHC isoform composition of most of the muscles in this study was heterogeneous. When an intramuscular transversal regionality was observed (semitendinosus, vastus lateralis, vastus medialis and rectus femoris muscles), MHCI percentage increased toward deeper layers, while MHCIIb and LDH-M4 decreased. The pattern of MHCIIa isoform distribution was less evident. Within semimembranosus and gluteus medius muscles, proportions of MHC isoforms were constant. Gradients of variation of MHCI and MHCIIb isoforms across rectus femoris and vastus medialis muscles were sharper than those of semitendinosus and vastus lateralis muscles. For the vastus lateralis muscle, these gradients may also be modified according to the breed. Breed effect was mainly shown by MHCIIb and MHCI isoforms and was not observed at all the sampling points of the muscles. These observations show that breed effect on muscle contractile and metabolic characteristics is not uniformly expressed throughout the muscle. Results of a comparison may differ according to the muscle and sampling location.

Detection of human Toxoplasma-specific immunoglobulins A, M, and G with a recombinant Toxoplasma gondii rop2 protein.

The Toxoplasma gondii rhoptry protein Rop2 was expressed in Escherichia coli as a fusion protein containing 44 kDa of the 55-kDa mature Rop2, supplied with six histidyl residues at the N-terminal end (Rop2196-561). Humoral response during Toxoplasma infection of humans was analyzed by immunoglobulin G (IgG), IgA, and IgM enzyme-linked immunosorbent assay with Rop2196-561 as the antigen substrate. The analyzed sera were divided according to T. gondii-specific serological tests (IgG, IgA, or IgM indirect immunofluorescence and IgA or IgM immunosorbent agglutination assay) as group A (IgG+ IgA- IgM-; n = 35), group B (IgG+ IgA+ IgM+; n = 21), group C (IgG+ IgA+ IgM-; n = 5), and group D (IgG+ IgA- IgM+; n = 16). Twenty-six T. gondii-seronegative sera from individuals with other infections were also included (group E). Anti-Rop2 IgG antibodies were detected in 82.8% of group A sera and in 97.6% of the sera with acute-phase marker immunoglobulins (groups B, C, and D). The percentage of IgA antibody reactivity against Rop2196-561 was 17.1% in group A, 50% in group D, and 80.8% in groups B and C. The percentage of IgM antibody reactivity was 0% in groups A and C and 62% in groups B and D. Sera from group E failed to show IgA, IgM, or IgG antibody reactivity. Since T. gondii Rop2 elicits a strong humoral response from an early stage of infection, it is suggested that recombinant Rop2196-561 would be suitable for use in diagnostic systems, in combination with other T. gondii antigens, to detect specific IgG, IgA, and IgM antibodies.

[Large cell variant of small cell carcinoma of the ovary with hypercalcemia].

We describe an original distinct type of ovarian small cell carcinoma: large cell variant. The distinctive histologic features of tumor cells were the presence of large nuclei with prominent nucleoli and abundant eosinophilic cytoplasm. Immunohistochemistry revealed strong diffuse vimentin, smooth muscle actin positivity and slight reactivity with epithelial markers. Electron microscopy showed aggregates of intermediate filaments, intercellular attachments and no dense core granules. This tumor is associated with paraendocrine hypercalcemia in two thirds of cases. Parathyroid hormone-related protein was focally positive. This tumor is characterized as a very lethal neoplasm, occurring primarily in young women.

Involvement of DNA methylation in the control of the expression of an estrogen-induced breast-cancer-associated protein (pS2) in human breast cancers.

pS2 gene has been used to investigate the relationship between alterations of DNA methylation patterns in human tumors and gene expression. The expression of pS2, which is transcriptionally controlled by estrogens in breast cancer cell lines, is restricted to estrogen-receptor-rich human breast tumors. We found that the CCGG site within the promoter/enhancer sequence of pS2 was hypomethylated in estrogen-receptor-rich breast tumors expressing this gene. The amount of DNA molecules unmethylated at this site was related to the amount of pS2 mRNA detected in the samples. The demethylation of this region, which contains the estrogen responsive element, was confirmed by genomic sequencing. Transient expression of functional human estrogen receptors stimulated the expression of the endogenous pS2 in HeLa cells, but failed, in BT-20 cells, to stimulate expression of this gene. Since the promoter/enhancer region of pS2 is unmethylated in HeLa cells and methylated in BT-20 cells, these data also support the hypothesis that DNA methylation might be involved in the control of pS2 expression.

Involvement of cyclic nucleotide-dependent protein kinases in cyclic AMP-mediated vasorelaxation.

1. The involvement of cyclic AMP-dependent protein kinase (PKA) and cyclic GMP-dependent protein kinase (PKC) in the effects of cyclic AMP-elevating agents on vascular smooth muscle relaxation, cyclic nucleotide dependent-protein kinase activities and ATP-induced calcium signalling ([Ca2+]i was studied in rat aorta. Cyclic AMP-elevating agents used were a beta-adrenoceptor agonist (isoprenaline), a phosphodiesterase 3 (PDE3) inhibitor (SK&F 94120) and a PDE4 inhibitor (rolipram). 2. In rat intact aorta, the relaxant effect induced by isoprenaline (0.01-0.03 microM) was decreased by a specific inhibitor of PKA, H-89, whereas a specific inhibitor of PKG, Rp-8-Br-cyclic GMPs, was without effect. NO significant difference in PKA and PKG activity ratios was detected in aortic rings when isoprenaline 10 microM was used. At the same concentration, isoprenaline did not modify ATP-induced changes in [Ca2+]i in smooth muscle cells. Neither H-89 nor Rp-8-Br-cyclic GMPs modified this response. These findings suggest that PKA is only involved in the relaxant effect induced by low concentrations of isoprenaline (0.01-0.3 microM), whereas for higher concentrations, other mechanisms independent of PKA and PKG were involved. 3. The relaxant effects induced by SK&F 94120 and rolipram were inhibited by Rp-8-Br-cyclic GMPS with no significant effect of H-89. Neither SK&F 94120, nor rolipram at 30 microM significantly modified the activity ratios of PKA and PKG. Rolipram inhibited the ATP-induced transient increase in [Ca2+]i. This decrease was abolished by Rp-8-Br-cyclic GMPS whereas H-89 had no significant effect. These results suggests that PKG is involved in the vascular effects induced by the inhibitors of PDE3 and PDE4. Moreover, since it was previously shown that PDE3 and PDE4 inhibitors only increased cyclic AMP levels with no change in cyclic GMP level, these data also suggest a cross-activation of PKG by cyclic AMP in rat aorta. 4. The combinations of 5 microM SK&F 94120 with rolipram markedly potentiated the relaxant effect of rolipram. This relaxation was decreased by H-89 and not significantly modified by Rp-8-Br-cyclic GMPS. Moreover, the association of the two PDE inhibitors significantly increased the activity ratio of PKA without changing the PKG ratio. The present findings show that PKA rather than PKG is involved in this type of vasorelaxation. The differences in the participation of PKA vs PKG observed when inhibitors of PDE3 and PDE4 were used alone or together could be due to differences in the degree of accumulation of cyclic AMP, resulting in the activation of PKA or PKG which are differently localized in the cell. 5. These findings support for both PKA and PKG in cyclic AMP-mediated relaxation in raT aorta. Their involvement depends on the cellular pathway used to increase the cyclic AMP level.

Comparison of statistical indicators for the automatic detection of 80 Hz auditory steady state responses.

OBJECTIVE: To evaluate, using receiver operation characteristic (ROC) curves, the performance of several statistical indicators in the objective detection of 80 Hz steady state auditory evoked responses. DESIGN: Steady state auditory evoked responses elicited by amplitude modulated tones of 500 and 1000 Hz, were obtained in 16 normal adults. Recordings were made at intensities ranging from 80 to 30 dB SPL and without stimulation. Four statistics: coherence synchrony measure, circular T2, a new variant of Hotelling T2 (labeled HT2) and a test for hidden periodicity (F test) were calculated. The statistics were compared using ROC curves and bootstrapping techniques. Two outcome measures were considered: behavioral threshold prediction and averaging efficiency. RESULTS: All indicators were highly accurate to detect a response (8 to 9 dB above mean behavioral threshold for the 1000 Hz and 14 to 16 dB for the 500 Hz carrier). Responses could be reliable detected after averaging about five individual epochs of long duration. No statistically significant differences were evidenced though in their capability to predict behavioral threshold or their averaging efficiency. CONCLUSIONS: Despite the more adequate statistical properties of some of these indicators no significant differences were found in their performance. Thus all of these indicators could be recommended for automatic detection of 80 Hz auditory steady state responses.

Induction of nitric oxide production by polyosides from the cell walls of Streptococcus mutans OMZ 175, a gram-positive bacterium, in the rat aorta.

The cardiovascular dysfunctions associated with septic shock induced by gram-negative or gram-positive bacteria (gram-positive or gram-negative septic shock) are comparable. In gram-negative septic shock, lipopolysaccharide (LPS) induces nitric oxide (NO) synthase, which contributes to the vascular hypotension and hyporeactivity to vasoconstrictors. The role of NO in gram-positive septic shock and the nature of the bacterial wall components responsible for the vascular effects of gram-positive bacteria are not well known. This study investigated the vascular effects of cell wall serotype polyosides, rhamnose glucose polymers (RGPs), from Streptococcus mutans, in comparison with lipoteichoic acid (LTA) from Staphylococcus aureus, on the induction of NO synthase activity in the rat aorta. We show that 10 microg of both RGPs and LTA per ml induced hyporeactivity to noradrenaline, L-arginine-induced relaxation, increases of 2.2- and 7.8-fold, respectively, of cyclic GMP production, and increases of 7- and 12-fold in nitrite release. All of these effects appeared after several hours of incubation and were inhibited by N(omega)-nitro-L-arginine methyl ester (L-NAME), an inhibitor of NO synthase. Electron paramagnetic resonance spin trapping experiments demonstrated directly that RGPs and LTA induced NO overproduction (four- to eightfold, respectively) in rat aortic rings; this production was inhibited by L-NAME and prevented by dexamethasone. These results demonstrate directly the induction of NO production in vascular tissue by LTA and show that another, chemically different component of gram-positive bacteria can also have these properties. This result suggests that different components of the gram-positive bacterial wall could be implicated in the genesis of cardiovascular dysfunctions observed in gram-positive septic shock.

Intercorrelations and sources of variability in three mutagenicity assays: a population-based study.

The purpose of this study was to evaluate the intercorrelation between three genetic assays in 112 subjects. The group was pooled from two originally separate but homogeneous subgroups of 56 persons each. Procedures included assays for hprt mutant frequencies, micronuclei in human lymphocytes, and mutations at the glycophorin A (gpa) loci. We found no statistically significant or biologically important intercorrelations among the three biomarkers. We did, however, observe significant correlations between log(e) hprt mutant frequency and cloning efficiency (inverse correlation for these 2 variables), age and log(e) hprt mutant frequency, an inverse relationship between cloning efficiency and age, and an important differential sex effect favoring a greater micronuclei frequency in females than males. No significant correlations between the covariates of interest and glycophorin A variant frequencies NN or NO were observed. Using multivariable linear regression, age was found to account for the majority of the variability in hprt mutant frequency (greater than sex and/or smoking); for micronuclei data, only sex contributed a statistically significant and biologically important proportion to the total variation. We conclude that despite observing no significant intercorrelations between the three assays performed simultaneously from the same individuals in a large population database, a significant correlation between age and hprt mutant frequency and an inverse association between cloning efficiency and hprt do exist; furthermore, we verified the strong differential sex-specific effect on micronucleus frequencies.

Cisapride as a treatment for gastroparesis in traumatic brain injury.

Gastroparesis is a well-documented finding among patients with severe traumatic brain injuries. The treatment of choice has been metoclopramide, but it is less than ideal given its central dopaminergic blocking activity. Cisapride is a newer prokinetic agent without side effects on the central nervous system and might be a superior treatment. The case of a healthy 22-year-old man who suffered a severe traumatic brain injury and multiple trauma in a motor vehicle accident is reported. The patient required gastrostomy tube placement but developed recurrent aspiration pneumonia once feedings were initiated. Despite receiving metoclopramide, barium studies revealed gastroparesis, a significant decrease in gastric peristalsis, and reflux. A jejunostomy tube was placed to prevent further aspiration. The administration of cisapride was begun, and 2 days later the patient was changed back to G-tube feedings with no evidence of residual aspirates. Repeat barium study showed definite improvement in peristalsis. This case demonstrates the potential usefulness of cisapride in patients with traumatic brain injury. Although further research is necessary, efficacy without central dopamine blockage may make cisapride the treatment of choice for gastroparesis and reflux in traumatic brain injury.

Artifact of blood pressure recording using heparin-filled catheter: effects on blood pressure and coagulation parameters.

A usual complication of catheterization procedures of arteries for blood pressure recording is the clogging of catheters due to activation of coagulation. This is usually avoided by filling the catheters with a heparinized saline solution. We have studied rats implanted with four catheters, one of which was used to monitor blood pressure for 6 h. Catheters were filled with 100 IU/ml heparin in saline. Using this standard protocol, approximately 50-200 IU of heparin were injected into the animals. This induces significant anticoagulation. The activated partial thromboplastin time (APTT) increased from 27 s to more than 240 s. We devised a modified "low heparin" protocol, in which the concentration of heparin in the wash solution of the catheters was lowered from 100 to 0.1 IU/ml, and the pressor transducer was back-perfused with saline solution without heparin. Using this new protocol, no significant modification of the APTT was observed, indicating that only trace amounts of heparin were injected. Subsequent controlled administration of heparin induced a significant decrease in blood pressure. To avoid all effects associated with this unwanted infusion of heparin, mainly a decrease in blood pressure and a major anticoagulant effect, we suggest the use of such "low heparin" catheterization protocol.

Frequency-specific audiometry using steady-state responses.

OBJECTIVE: To evaluate the audiometric usefulness of steady-state responses to multiple simultaneous tones, amplitude-modulated at 75 to 110 Hz. DESIGN: Steady-state responses to multiple tones amplitude-modulated at different rates between 75 and 110 Hz and presented simultaneously were recorded at different intensities in normal adults, well babies, normal adults with simulated hearing loss, and adolescents with known hearing losses. Response thresholds were compared with behavioral thresholds. RESULTS: In normal adults the thresholds for steady-state responses to tones of 0.5, 1, 2, and 4 kHz were 14 +/- 11, 12 +/- 11, 11 +/- 8, and 13 +/- 11 dB, respectively, above behavioral thresholds for air-conducted stimuli, and 11 +/- 5, 14 +/- 8, 9 +/- 8, and 10 +/- 10 dB above behavioral thresholds for bone-conducted stimuli. In well babies tested in a quiet environment, the thresholds were 45 +/- 13, 29 +/- 10, 26 +/- 8, and 29 +/- 10 dB SPL. In adolescents with known hearing losses, the steady-state responses thresholds predict behavioral thresholds with correlation coefficients (r) of 0.72, 0.70, 0.76, and 0.91 at 0.5, 1, 2, and 4 kHz, respectively. CONCLUSION: Steady-state responses to tone amplitude-modulated at 75 to 110 Hz can be used for frequency-specific objective audiometry. The multiple-stimulus technique allows thresholds to be estimated for eight different stimuli at the same time.

[Unilateral asterixis and focal brain lesions. 12 cases].

Asterixis is a involuntary movement with spontaneous interruptions and intermittent muscle tone. It occurs during posture maintenance. It is usually bilateral and, in this case, is observed in metabolic encephalopathy. Unilateral asterixis is more uncommon. We report 12 cases which occurred in patients with focal brain lesions. In all cases asterixis involved the upper limb and the lower limb in only 2 cases. Asterixis was transient, discrete and always associated with other neurological signs. The causal lesions (7 infracts, 2 haematomas, 2 abscesses, 1 meningioma) were unique in 7 cases and multiple in 5. Asterixis was always contralateral to the unilateral lesions. Lesions mainly involved the thalamus (7 cases) and other structures (lenticular nucleus, frontal lobe, internal capsule, precentral regions and cerebellum). The frequency of thalamic involvement suggested dysfunction of the thalamo-cortical loop.
