Molecular weight analysis of Fc gamma-binding proteins of lymphoid leukemia, myeloid leukemia, and hairy-cell leukemia.

The molecular weights of EDTA-mercaptoethanol-soluble Fc gamma-affined proteins isolated from chronic lymphocytic leukemia of the B type, prolymphocytic leukemia of the B type, chronic myeloid leukemia and hairy-cell leukemia were compared. SDS polyacrylamide gel electrophoresis of the Fc gamma-binding material obtained from all six cases of B type leukemia revealed a single peak with an apparent molecular weight of 28,000. The Fc gamma-affined material isolated from the cells of two cases of chronic myeloid leukemia showed two peaks, one with an apparent molecular weight of 42,600 and one with an apparent molecular weight of 18,800. The Fc gamma-affined material isolated from the cells of two cases of hairy-cell leukemia electrophoresed in the form of a closely spaced double peak. One component of the double peak had an apparent molecular weight of 28,000 and thus corresponds to the Fc gamma-binding material of leukemic B cells. The second component had a slightly lower molecular weight. The latter component is not present on either leukemic B cells or myeloid cells. The results indicate that the EDTA-mercaptoethanol-soluble Fc gamma-affined proteins of different types of cells differ in molecular weight, and thus in molecular structure.

Comparing cell kinetic studies of the effect of ftorafur and 5-fluorouracil on the L 1210 ascites tumor.

Ftorafur (FT), a 2-tetrahydrofuryl derivative of 5-fluorouracil (5-FU) was introduced into cancer chemotherapy with the hope to obtain a therapeutic effect comparable to 5-FU with a smaller dose and less side effects. A comparison of the effect of FT and 5-FU on the proliferation of L 1210 ascites tumor cells in the present work has shown that both drugs result in an inhibition of DNA synthesis due to an inhibition of thymidylate synthetase. However, the extent of the effect of FT is reduced, i.e., to achieve a cell kinetic effect comparable to that of 5-FU, 60 times the equimolecular FT dose has to be applied. Thus, there is no dose-saving effect of FT compared to 5-FU. A depot-like 5-FU effect of FT due to a slow release of 5-FU from FT as described in the literature could not be confirmed in the present study either. The effect of FT on the proliferation of L 1210 ascites tumor cells did not last longer than that of 5-FU. In contrast to 5-FU a severe side effect, viz., a drastic decrease of the body temperature from 39.1 degrees to 31.6 degrees C, was observed. That means that FT cannot only be effective by a release of 5-FU. This toxic side effect must be due to a different mechanism of action of FT. A significant increase in the median life span could only be achieved by the application of 5-FU (180 microgram/g). An equimolecular FT-dose did not result in an increase of the median life span. Based on the present study, the advantages of FT compared to 5-FU described in the literature cannot be confirmed. The present work shows that cell kinetic studies in animals are a useful tool to test the effect of new drugs in chemotherapy.

[Current concepts of the drug therapy of glomerulonephritis].

The concept of treatment of chronic glomerulonephritis, practiced more than 2 decenniums led to unsatisfying results. All over the world the chronic glomerulonephritis is the most expensive in economic respect renal disease. It is still the most frequent indication to dialysis and transplantation. The judgment of the success of therapy was rendered difficult by several factors. Different nomenclatures, inhomogeneous dosages and durations of therapy, inhomogeneous judgments of the beginning of therapy, of the degree of activity and the judgment of effectivity render the comparison difficult. The results from 32 studies are given in form of tables. Newer knowledge of the immune state and of the mediators of inflammation will broaden the therapeutic basis in a generally accepted immunogenesis of the chronic glomerulonephritis and will lead to an equalized selective immunotherapy.

[IgA nephropathy in diabetes mellitus (author's transl)].

Report on a diabetic patient with IgA nephropathy whose kidneys were examined by biopsy four times during a period of four years. Morphologically the typical findings were observed: Outstanding deposits of IgA, hypercellularity and increase of matrix within the mesangium. The findings persisted during the period of observation. The differential diagnosis between IgA nephropathy and a slight diffuse glomerulosclerosis in diabetes mellitus is discussed. IgA nephropathy is diagnosed with certainty only by the aid of immunohistology.

[The significance of electron microscopy for the assessment of membranous glomerulonephritis (author's transl)].

In 35 biopsy specimens taken from 21 patients with membranous glomerulonephritis (mGN) the ultrastructure of the glomeruli was investigated with special regards to the degradation of the immune complexes. Electron microscopy allows the most comprehensive assessment of the manifold basement membrane changes. This is of high value for the practical diagnosis.

[Cystic-stump-syndrome (author's transl)].

When persistent or recurrent pain appears following a cholecystectomy, precise clarification is required (ERCP). A long stump of the cystic duct should be cited as one of numerous different causes. If reoperation is indicated, the stump must be searched for radiomanometry should be implemented in order to ascertain possible choledochus stones or a stenosis of the papilla and redress these in the course of the same operation. The existing stump must be resected in any case. Among 5020 operations for benign diseases of the bile duct (1961 to 1977) 88 cystic duct stump resections were performed without fatalities. With the exception of 2 cases the operation remedied the patients' complaints.

[A contribution to the psychopathology of patients with open heart surgery (author's transl)].

The results of a larger study of psychopathological disorders in relation to pre-operative and post-operative examined heart surgery patients, where the heart-lung machine was used, will be shown, to begin with, on 20 patients. Pre-operative examination showed quite clearly, among other things, that patients suppress obvious operation risks. General mental disorders were seen post-operative in 4 patients, and these were diagnosed as organic psychosis. A comparison between psychotic and other patients shows, among other things, that somatic predictors such as cerebral sclerosis and atrophy, or a cerebral trauma tend to promote post-operative psychosis. A releasing cause could be a cerebral hypoxia brought about by the extracorporeal blood circulation. In our opinion psychological factors can only be found in the contents of psychosis.

[Comparative studies on the protective effect of etilefrin and dihydroergotamine in circulatory instability after blood donation].

In 20 female and 40 male subjects with circulatory instability, the response of pulse rate and blood pressure to blood loss and orthostasis was studied under the condition of protective pretreatment with drugs known to improve venous return by increasing the tone of capacitance vessels. Before blood loss (420 ml), in a cross-over double-blind procedure, each subject obtained 10 mg etilefrin or 2 mg dihydroergotamine, both administered orally. The procedure was repeated after at least 8 weeks. At this time, the subject obtained the drug which was not given in the first examination. By comparison of pulse rate and blood pressure taken after pretreatment, the statistical evaluation did not reveal any differences in the effectiveness of one or the other drug on the response to hypovolemic and orthostatic conditions. Patients preferred etilefrin.

Cardiovascular effects of tricyclic and tetracyclic antidepressants.

Cardiovascular effects of therapeutic doses of tricyclic or tetracyclic antidepressants (TCA) were examined in 66 patients. After three weeks of therapy, heart rate and PR interval were increased (P less than .02, P less than .05), while prolongation of the QTc time and the QRS interval did not reach significant levels. We observed significant flattening of T waves (P less than .05), which was not associated with changes in the serum potassium level. These changes were reversible after treatment was discontinued. When therapy was maintained for 13 months, only the heart rate continued to be increased, whereas all other ECG values had returned to normal. The TCA therapy led to a significant prolongation of the preejection period (P less than .01) and slight shortening of the left ventricular ejection time, indicating a decrease in myocardial contractility. There was no difference of effects on the values studied between tricyclic and tetracyclic antidepressants.

[Congenital aplasia of the diaphragm].

The present paper reports on a complete left-sided aplasia of the diaphragm. In this especially risky case (birth weight 2000g) the left hepatic lobe was used for separation of the abdominal from the thoracical space in order to quickly terminate the surgical operation. Very probably the child would not have survived a major plastic operation. As the control examination (annual control) showed, the child is developing quite normally. Until now tests of the hepatic function did not render any pathologic findings either.

[Contribution to the effect of tri and tetracyclic antidepressive agents on the heart and blood circulation].

In 47 patients ECG tracings were recorded and cardiovascular values determined before therapy, during treatment with antidepressive agents after it had been in progress for at least 3 weeks, and 4 weeks after withdrawal of therapy. In a further 19 patients in whom antidepressive therapy could not be withdrawn, the same test battery was repeated after an average period of 13 months. No serious disturbances of cardiac rhythm were detected and certain changes in ECG criteria (prolongation of PR interval, widening of QRS complex, prolongation of QTc time and T-wave flattening) proved to be reversible. There was no difference between tricyclic and tetracyclic antidepressive agents. We are nevertheless of the opinion that ECG and cardiac function should be carefully monitored in elderly patients and in those on prolonged therapy with high doses of antidepressives. The results of this study are discussed and compared with previously published findings.
