The neuroleptic malignant syndrome. A priority system.

The neuroleptic malignant syndrome (NMS) of hyperthermia, rigidity, hyperautonomicity and altered consciousness results from the use of neuroleptic agents. Although rare, it is important to recognize and treat the syndrome early because it is potentially lethal. It bears striking similarities to several other conditions, particularly malignant hyperthermia. NMS probably reflects a disorder of the hypothalamic and basal ganglia dopamine systems. No specific treatments are available, but dantrolene sodium, bromocriptine, amantadine and phenytoin have been used successfully.

Carbamazepine as adjunctive treatment in nonepileptic chronic inpatients with EEG temporal lobe abnormalities.

A 15-week double-blind, randomized, within-patient design was used to assess carbamazepine versus placebo as adjunctive treatment in chronic hospitalized psychiatric inpatients (predominantly schizophrenic) with EEG temporal lobe abnormalities but no clinical evidence of epilepsy. A statistically significant improvement on carbamazepine was found on all clinical rating measures used (Overall Clinical Rating, Brief Psychiatric Rating Scale, and Global Assessment). The results also appeared to be clinically significant.

Non-epileptic symptoms of temporal lobe dysfunction.

A review of the common and uncommon nonepileptic symptoms found (experimentally or clinically) to be associated with anomalous functioning of the temporal lobes of the brain, is presented. Inter-ictal temporal lobe features are also reviewed.

Symptomatology of temporal lobe epilepsy.

The numerous and varied symptoms of temporal lobe epilepsy are reviewed in detail. Partial elementary, partial complex, secondary generalized, post-ictal and predisposing aetiological symptoms are examined.

Is déjà vu a symptom of temporal lobe epilepsy?

The definition and conceptualization of the déjà vu phenomenon are interpreted in various ways. The common occurrence of déjà vu is the general population stresses the need for the development of specific qualitative features which will be valuable in the diagnosis of temporal lobe epilepsy.

The use of carbamazepine to treat benzodiazepine withdrawal in a geriatric population.

Rapid withdrawal of short to intermediate half-life benzodiazepines may be hazardous, particularly in the elderly. The use of carbamazepine to facilitate withdrawal has been reported in younger patients. We describe four elderly patients (average age, 72.5 years) who had each experienced at least one unsuccessful attempt at alprazolam withdrawal and who were subsequently successfully withdrawn via the use of carbamazepine over a period ranging from 2 to 6 days. These geriatric patients experienced no major withdrawal symptoms, but mild symptoms were common. There was no correlation between dose or duration of alprazolam use and extent of withdrawal symptoms. We recommend use of this treatment regimen in a hospital setting only, where close monitoring can occur.
