[Spectrum of pneumotropic pathogens in pediatric patients with acute bronchitis and pneumonia].

One hundred eighty nine children with acute bronchopulmonary infectious pathological processes were examined bacteriologically and serologically for typical pneumotropic pathogens, 47 of them being as well examined for atypical organisms. Microbial associations mainly with Mycoplasma and Pneumocystis and to a lesser extent with Chlamydia were isolated from the majority of the children. Reactivation of the cytomegalovirus infection was observed in 25 per cent of the children. Pneumonia and bronchitis due to Mycoplasma pneumoniae either as a monoagent or in associations were mainly stated in children over 7 years of age. No significant changes between the indices of the infection due to a definite organism and the active progression of the infectious process of the same etiology were revealed, though in the cases of chlamydiosis the changes reached almost 10 per cent. In cases of acute bronchitis and pneumonia the chlamydial or cytomegalovirus infection could be assumed to be of the persisting nature, mainly acute in cases of pneumococcal infection, mixed in cases of hemophilic or pneumocystic infection, primary contamination with a tendency to prolonged in cases of mycoplasmic infection. The findings of the examination and the clinical and anamnestic data showed that the clinical picture of acute pneumonia had specific features associated with the supposed etiological agents, still it could change under the action of associations of pneumotropic pathogens.

[Disruption of microbiocenosis of the large intestine and the immune and interferon status in children with bacterial complications of acute viral infections of the respiratory tract and their correction by high doses of bifidumbacterin forte].

One hundred and twenty-nine children with acute viral and bacterial infection of the respiratory tract were examined and disturbance of the large intestine microflora was detected. It was characterized by significant reduction of lactobacilli, moderate growth of opportunistic bacteria and higher contents of Candida. Changes in T- and especially B-cellular immunity were observed in 35 per cent of the patients. In patients with decreased avidity of the immunoglobulins G in the peripheral blood the changes were observed in 82-100 per cent of the cases. In the majority of the patients the capacity for interferon genesis was suppressed. A shorten course (5 days) of the bifidumbacterin forte therapy in a dose of not less than 10(9) CFU/ml. normalized the intestinal microflora, improved the indices of the B- and T-cellular immunity (including the subpopulation of the T-helper cells but not the T-suppressor cells), stimulated NK and improved the ability to induce alfa- and gamma-interferons of the peripheral blood leukocytes. The experience with using high doses of bifidumbacterin forte was evident of its good tolerance and possible value in increasing the patient resistance to infection.

[Clinical manifestations and treatment of intestinal dysbacteriosis in patients with Flexner's dysentery].

Flexner's dysentery is often accompanied with intestinal dysbacteriosis. Disbiotic changes in the intestine contribute to specific shigella endotoxins entering blood flow thus prolonging clinical symptoms of the underlying disease. Administration of bacterial biological preparations (bifidumbacterin forte, lactobacterin) relieves specific endotoxemia, reduces the duration of the disease and has an immunomodulating action.

[Treatment of severe dysentery].

The paper presents analysis of clinical efficacy of etiotropic drugs in severe Flexner's dysentery chosen by sensitivity of the causing agent, schemes of antibacterial drugs of choice in the treatment of this disease, principles of detoxication therapy. Use of bacterial biological drugs in combined treatment of dysentery is validated.

Isolation and characterization of Escherichia coli strains producing microcins of B and C types.

A screening of 11956 enterobacteria isolates resulted in selection of seven active microcin-producing strains. The microcins were shown to be peptides or their derivatives with a rather broad spectrum of activity, mainly against Gram-negative bacteria. According to cross-immunity criteria, the microcins studied belonged to two of the previously suggested types, B (five strains) and C (two strains). Those of type B could be further classified into two subtypes on the account of differences in the spectrum of antibacterial activity. In five cases out of seven the microcin-producing ability has been attributed to plasmids that the strains harboured. The effect of microcins on sensitive cells was shown to depend on ompR and ompF gene products.

[Characterization of enterobacteria producing the broad-spectrum antibiotics microcins].

As a result of screening among 11956 enterobacteria strains isolated from feces of normal children, grown-ups and lambs, seven active microcin-producing strains were obtained. The microcins were shown to be peptides or their derivatives with a low molecular weight (less than 10,000) and a broad spectrum of activity, mainly against gram-negative bacteria. According to cross immunity criteria the microcins studied belonged to two different types. Those of type I could be further classified into two subtypes on the account of difference in the spectrum of antibacterial activity. In 5 cases out of 7 the microcin-producing ability and immunity to microcins have been attributed to plasmids that the strains harboured. The effect of microcins on sensitive cells depended on ompR and ompF gene products.

[Immunobiological properties and therapeutic effectiveness of preparations from Klebsiella bacteriophages].

The purified preparations of Klebsiella bacteriophages, viz. the monovalent preparation of K. pneumoniae bacteriophage and the polyvalent bacteriophage preparation for the treatment of infections caused by K. ozaenae, K. rhinoscleromatis scleromatis and K. pneumoniae sensu lato, have been obtained. The bacteriophage preparations have proved to be nontoxic and safe for laboratory animals after the intraperitoneal injection of these preparations followed by the pathomorphological study of the internal organs of the animals. The clinical study of the newly developed bacteriophage preparations in the course of the treatment of purulent inflammatory diseases in 109 patients has revealed that the preparations are not reactogenic and exhibit sufficient effectiveness in the therapy of ozena, rhinoscleroma and Klebsiella infections with different localization of the infectious process.

[The resistance plasmid of Klebsiella pneumoniae that controls its cytotoxic activity].

R-plasmid (40 MD) isolated from K. pneumoniae hospital strain makes Escherichia coli strain J62 capable of inducing a cytotoxic effect which can be detected in Hep-2 cell culture. In contrast to the initial E. coli strain J62 producing no changes in the monolayer, E. coli J62 cells containing P-plasmid induced pronounced cytotoxic changes and a sharp increase in the number of nonviable Hep-2 cells by hour 24 of interaction.

[The prevention of the translocation of intestinal microflora after the rescue of an organism from a terminal state].

In experiments on guinea pigs, the animals were resuscitated from clinical death caused by the acute loss of blood and subsequently treated intragastrically with enterosorbents: activated carbon fibrous material, alone and in combination with polymyxin B, polyphepan (a lignin derivative), polymethyloxan hydrogel and the sorbent Enterocat. In the animals, not treated during the postresuscitation period, a high population level of enterobacteria and Gram-positive aerobic cocci was registered in the contents of the small and large intestines and their translocation to mesenteric lymph nodes, liver, spleen and blood was observed. The amount of lactobacilli in the small intestine was decreased. Enterosorbents were found to decrease a high population level of intestinal microflora, to prevent the translocation of Gram-positive aerobic cocci and to inhibit the penetration of enterobacteria through the enteric barrier in the postresuscitation period. Combined use of activated carbon fibrous material with polymyxin B proved to be most effective for the elimination of enterobacteria.

[The characteristics of the colonization resistance of the intestines in conventional animals with a history of clinical death].

After modeling the terminal state caused by the acute loss of blood in rats their intestinal microflora was studied, as was their resistance to colonization. Decreased resistance to colonization was registered early after resuscitation (up to 3 days), which was confirmed by the translocation of bacteria into internal organs, decreased number of lactobacilli in the contents of the small intestine and elevated level of enterobacteria in the intestine. Disturbances in resistance to colonization was also manifested by prolonged colonization of the digestive tract of the resuscitated animals by Escherichia coli indicator strain K12pSS-120 carrying Shigella sonnei (phase I) invasiveness plasmid.

[Protection from virulent Salmonella groups B and D after the peroral immunization of chicks with a hybrid of Salmonella typhimurium and Salmonella dublin].

Chickens over 10 days old, infected orally with virulent salmonellae, were found to remain alive. Histologic investigation showed the development of mild enteritis and more pronounced, lasting for more than two weeks, inflammation of the cecum, dissemination and focal lesions in the liver (granulomas, necrosis). In experiments on the oral immunization of 3-day old chickens the bivalent hybrid of S. typhimurium vaccine strain 274 and S. dublin induced only pronounced blast transformation in lymphatic follicles of the cecum, hyperplasia of activated macrophages and formation of granulomas from these macrophages and lymphocytes. After oral challenge of the immunized chickens with virulent salmonellae of group B (S. typhimurium) and group D (S. enteritidis, S. gallinarum-pullorum) the chickens exhibited sharply pronounced protection against adhesion, colonization and invasion, and a few penetrating bacteria were rapidly destroyed by immune macrophages. Hybrid strain 274/O9 proved to be suitable for use as oral bivalent vaccine against salmonellosis in chickens.
