Investigations of the estrogen (ER-ICA-test) and the progesterone receptor in the prostate and prostatic carcinoma on immunohistochemical basis.

Estrogen (ER) and Progesterone receptors (PR) were demonstrated immunohistochemically on frozen sections from 11 prostatectomy and 7 cystoprostatectomy specimens in the nuclei of various cell types. The periglandular fibrocytes and smooth muscle cells were extensively positive, the interglandular stromal cells were only partly so. Normal basal cells stained focally positive, hyperplastic basal cells stained extensively. The glandular secretory epithelium and atrophic glands were negative. The same findings were obtained in hyperplastic nodules. Both ER and PR also occurred in the urothelium of central prostatic ducts and of the prostatic urethra. The fibrous stroma around the ejaculatory ducts and seminal vesicles was extensively positive while the epithelium was negative. The smooth musculature of the seminal vesicles was only partly positive. On large field sections, the ER as well as the PR were numerically equally distributed throughout the inner zone of the prostate and the prostate proper. 12 prostatic carcinomas (G I-G III) were ER- and PR-negative. Estrogens may contribute to nodular hyperplasia by triggering a stromal proliferation with a secondary inductive epithelial growth. Obviously they do not act directly on prostatic carcinoma but inhibit growth via the hypophyseal-testicular axis. The biological significance of the PR in the prostate is unknown.

Immunohistological determination of proliferative activity in seminomas.

The proliferative activity and growth pattern of 20 seminomas were determined immunohistologically with the monoclonal antibody Ki-67. A growth fraction of tumour cells between 50 and 80% was found in seminomas with an almost even distribution of proliferating cells in all sections, regardless of tumour size. There was a slight tendency towards a greater growth fraction in tumours at an advanced histopathological stage. No positive correlation could be found between growth fraction and tumour size or lymphocytic infiltration. The results confirm the well known sensitivity of seminomas to radiation and chemotherapy and show that the determination of proliferative activity should be included in the histopathological routine diagnosis of malignant tumours with regard to systemic treatment and prognosis.

Testicular cysts: differentiation with US and clinical findings.

In 34 of 847 patients (4%), testicular cysts were discovered with high-resolution ultrasound (US). Sixteen had a cystic testicular tumor, and US images showed multiple cysts and solid tumor portions in 12 of these 16 (75%). Eighteen patients had nonneoplastic cysts: Five had cysts of the tunica albuginea, and 13 had intratesticular cysts. All tunica albuginea cysts were palpable as small masses, and only US images showed that the masses were cystic. None of the 13 intratesticular cysts was palpable; all were accidentally discovered with US, which showed that 12 were single cysts with a marginal location (92%). On histologic studies, the nonneoplastic cysts were related to the spermatic ducts (rete testis, efferent ductules) and frequently appeared as postinflammatory alterations. Only five patients with intratesticular cysts were followed up with US, and no changes occurred. The combination of clinical and US findings facilitated the differentiation between nonneoplastic and neoplastic testicular cysts.

Intrascrotal metastasis of renal cell carcinoma. Case reports and review of the literature.

In a series of 235 patients with testicular tumors 2 elderly patients (0.9%) had secondary testicular tumors: 1 from a renal cell carcinoma and 1 from an unclassified anaplastic neoplasm. The pathogenesis of intrascrotal metastases of renal carcinoma is demonstrated by angiography of the dilated spermatic vein. In a literature survey the incidence rate of secondary testicular tumors is found to range from 0.3 to 3.6%. 30 cases of intrascrotal secondaries of renal cell carcinoma have been identified in the literature.

[Campylobacter pylori in the stomach, duodenum and colon of gastroenterological patients. An epidemiologic study of 120 subjects].

In a prospective study of 120 gastroenterological patients in Berlin, Germany, the prevalence of Campylobacter pylori was determined. When the gastric mucosa was normal, the prevalence was one in 19 patients (5.3%). In 101 patients with chronic gastritis it was cultured in 55 (54.5%). In 31 patients with chronic atrophic gastritis the organism was cultured in 25 (81%); in 60 patients with severe gastritis it was present in 75%, in 35 with moderate or severe chronic active gastritis in 82.8%. The diagnosis of gastric ulcer (9) or duodenal ulcer (12) was associated with the isolation of Campylobacter pylori in 55.6 and 91.7%, respectively. The prevalence of this organism in antral mucosa of this group of patients thus corresponds to that in Australia, England and North America. The organism was also demonstrated in the duodenum of 10 among 25 patients examined. But in none of 25 patients was it demonstrated in the sigmoid colon.

[The angiographic and morphologic features of angiomyolipoma of the kidney (author's transl)].

The angiographic and morphologic findings of angiomyolipomas of the kidney are described in three patients. These benign tumours contain three types of vessel in addition to fat and smooth muscle. One vessel type is thick-walled with fibrosis of its walls and no internal elastic lamina. The second type consists of sinuses with considerable variations in calibre. The third type resembles capillaries and is found in the muscular parts of the tumour. Because of the abnormal structure of their walls, these vessels are easily damaged and may cause massive bleeding.
