[Local hyperthermia and hyperglycemia in ORL oncology].

Two-year experience of radiotherapy combined with local hyperthermia, hyperglycemia and chemotherapeutic drugs in patients with tumors of the upper respiratory tract showed advantages of this method. The results were compared with those of the control group of patients on radiotherapy only. Three therapeutic schemes were employed. Hyperthermia was performed in 2 temperature regimens. Of chemotherapeutic drugs platidiam, methotrexate, 5-FU and vinblastine were used.

[Cytological mechanisms of the development of mucosal candidiasis after exposure to a steroid aerosol preparation].

Mucosal candidosis was induced in CBA mice by intraoral inoculation following treatment with corticosteroid-containing aerosol (beclomethasone dipropionate). Histologically, in hormone treated mice the adherence of the pathogen to the mucosal surface was found during the first hours after inoculation. This is followed by the formation of the germ tubes and invasion in the epithelial layer. Pseudomycelial invasion in the malpighian layer is accompanied by the leukocyte response that limits the further spread of the fungal cells. In intact mice, the inoculation is not followed by the effective attachment of the fungal cells to the mucosal surface and induction of mycotic lesions. In vitro experiments have demonstrated the enhanced adherence of fungal blastospores to the epithelial cells of the hormone treated animals, that appears to be one of the mechanisms in the pathogenesis of candidosis in these animals.

[Pathogenesis and morphologic features of candidal colpitis].

Pathomorphogenesis of candidal colpitis was studied using the murine model with induced oestrus. After intravaginal inoculation, the adherence of pathogen to the mucosal surface of vagina is noted which is followed by its invasion into superficial and deep epithelial layers with the formation of germ tubes and pseudomycelium. The fungal cells are eliminated by the epithelial desquamation and by leukocytes migrating into the epithelium and forming intraepithelial abscesses. The sanation of mucosa is achieved by the sequestration of infected epithelium from the non-infected one. Tissue reactions appear to be in close correlation with the activity of pathogen during all the phases of mycotic process.

[Basic characteristics of the process of Candida adhesion to human epitheliocytes].

The adhesion of fungi belonging to the genus Candida to the epithelial cells of the mouth cavity reached its maximum at pH 6.2-7.0. The process of adhesion had similar dynamics at temperatures of 37 degrees, 28 degrees and 25 degrees C, but the adhesive activity decreased 2 times when temperature dropped from 37 degrees to 25 degrees and 4 times when temperature dropped to 4 degrees C. The introduction of the ions Ca2+ (1 and 10 mM) and Mg2+ (10 mM) led to the increase of adhesion by 80, 100 and 24% respectively. The heating of the fungal cells at 100 degrees C (for 1 hour) and at 63 degrees C (for 2 hours) decreased adhesion to 8 and 24% respectively, and treatment with formaldehyde (for 24 hours) decreased adhesion to 70% of that observed in experiments with live Candida cells.

[Multifield local hyperthermia as a method for heating deeply located neck tissues].

Distribution of the thermal field in neck tissues in local SHF-hyperthermia is studied. The two-field method of irradiation permits a uniform heating of more deeply located layers of the tissue due to summation of the radiant energy. So, the contact method of irradiation using "Luch-2" and "Luch-3" sets permits elevating the temperature inside the larynx up to 41-41.5 degrees C and maintaining it under given conditions for 25-30 min, but the heat loading on the skin and subcutaneous fat remains high. When using noncontact irradiators and "Parus" set the deeply located neck tissues are heated to 42-43 degrees C, the temperature of surface layers not exceeding 41-42 degrees C. This temperature rate is possible to be maintained for 25 min; its prolongation induces a sharp rise in the temperature of surface layers. Long-term SHF-hyperthermia of deeply located neck tumours necessitates cooling of the integument surface.

[Radiation therapy with local UHF hyperthermia in the late stages of laryngeal cancer].

Altogether 47 patients with advanced laryngeal and laryngopharyngeal tumors were treated. Twenty-six patients received multimodality therapy including local UHF-hyperthermia and gamma-beam therapy; 21 patients receiving radiotherapy only were entered in the control group. Hyperthermia was induced by the electromagnetic field with the frequency of 2450 and 915 MHz. Parus and Plot units were used. External irradiation was provided. Clinically, complete primary tumor regression was achieved in 20 of 26 patients. A positive effect was absent in 3 cases when metastasis size was 10 X 12 X 8 cm. In the control group, clinically complete tumor regression was noted in 2 patients only. In 4 cases the effect was insignificant, in the rest of 15 cases various degrees of a decrease in tumor and metastasis sizes were revealed.

[Problem of candidiasis in surgery].

On the basis of personal investigations and literature data the authors analyse etiopathogenesis of Candidiasis. The suppressed immune system due to administration of corticosteroids, antibiotics, immunodepressive drugs, the presence of the initial disease and operation proper are responsible for the increased amount of fungi on mucous membranes and their more frequent colonizations.

[Pathogenesis and morphogenesis of candidiasis].

During the interaction with the host the fungi of Candida species are able to attach to the epithelial cells of the mucous membranes, to invade the epithelium with its destruction and penetration into the underlying connective tissue where their further spread may be limited or progress, being accompanied by the penetration of the vessels, hematogenous dissemination and formation of the secondary infectious foci. The severity, depth and extent of lesions in candidiasis depend on the state of the host immune system. This review describes the individual stages of the host-parasite interaction in candidiasis and the host defense responses typical for each stage.

[Semiquantitative methods of analyzing the changes in the thyroid gland].

Precise quantitative methods may not always be used to solve various morphological problems. In certain instances it is more expedient to apply semiquatitative estimations of histological preparations which give sufficient information on the phenomena under study, requiring the minimum time. Using the study of normal and pathological morphology of the thyroid gland as an example, different variants of the semiquantitative estimates are considered: the estimate of individual features, the estimate of the entire morphological pattern (generalized semiquantitative analysis and differential semiquantitative analysis). The conditions for the application of each method are discussed. The advantages and limitations of the semiquantitative methods are analysed.

[Stereologic analysis of the thyroid gland (review of methods)].

The most important stereological methods applied for quantitative morphologic investigation of the thyroid gland are considered: estimation of relative volumes of the organ's components, of follicular surface, of follicular surface/volume ratio, of number of cells and follicles per volume unit and in the whole organ, their distribution according to their size. The main formulae are presented and the most effective ways in application of stereological of stereological methods for investigation of normal and pathological gland in men and animals are described.
