[Ziksorin in the treatment of unconjugated familial functional jaundice--the Gilbert-Meulengracht syndrome].

Zixoryn (Flumezinolum) is a new enzyme inductor without the adverse effects of the other known substances with similar inductor activity (phenobarbitals, DDT derivates, etc.). It was applied to 12 patients with unconjugated nonhemolytic familial jaundice--Gilbert-Meulengracht's syndrome. The dose was 600 mg'6 capsules of 100 mg taken as a single dose) once in a week in the course of 2 months. This treatment led to a 50% reduction of the hyperbilirubinemia in all patients treated. In the patients with initial bilirubin level under 35 mmol/l Zixoryn led to normalization of the bilirubin level with a normal ratio between the conjugated and unconjugated fractions. The discontinuance of Zixoryn treatment led in the course of 20-25 days to a gradual rise of bilirubin to its previous pathologic level. No side effects were registered in the patients treated with Zixoryn. Zixoryn is a new effecient drug for treatment through enzyme induction of bilirubin transport and glucuronizing enzyme systems in the hepatocytes. It is especially useful, in patients with high unconjugated hyperbilirubinemia as well as in patients with deterioration of the disease with an increase of the bilirubin level--total and unconjugated.

[Campylobacter pylori in patients with chronic gastritis and gastric and duodenal peptic ulcer].

The presence of Campylobacter pylori was studied in biopsy material from gastric mucosa taken by guided biopsy during fiber gastroduodenoscopy from 101 patients with chronic gastritis (n = 50), peptic gastric ulcer (n = 28), peptic duodenal ulcer (n = 7), gastric cancer (n = 10) and gastric polyposis (n = 6). Campylobacter pylori was found in various quantity--moderate (++) and considerable ( )--in 64% of the patients with chronic gastritis, in 85.7% of the patients with peptic gastric ulcer and in 100% of the patients with peptic duodenal ulcer as well as in half of the patients with gastric cancer and polyposis. The quantity of Campylobacter pylori correlated with the severity of the inflammatory process and the degree of atrophy of the gastric antral mucosa. A tendency toward seasonal incidence of Campylobacter pylori infection of gastric mucosa was established: 78.69% of the patients examined during spring time (April-May) and 60.0% of the patients examined during winter time (January-February) had Campylobacter pylori infection. This explains to a certain extent the seasonal exacerbations of the pathological process in chronic gastritis and peptic ulcer. The development of a well expressed chronic atrophic gastritis is in direct relation with the greater quantity of Campylobacter pylori in the gastric mucosa. The study reveals the pathogenetic relations between the presence and quantity of Campylobacter pylori and the development and evolution of chronic gastritis and peptic gastric and duodenal ulcer.

[Doxycycline esophagitis and esophageal ulcer].

Four cases of acute doxycycline injury of the esophageal mucosa--acute catarrhal doxycycline esophagitis and acute doxycycline ulcer are presented. The pathogenesis, clinical course, endoscopic pattern, prophylaxis and treatment of these complications of doxycycline treatment are discussed in detail. The gastroesophageal and duodenogastric reflux, which are most often the result of functional failure of the lower esophageal sphincter, are pointed out as the basic pathogenetic mechanism of the esophageal mucosa injury in oral doxycycline treatment.

Acute Disease↗

[Leiomyoma of the stomach].

A case of a 72-year-old man with a leiomyoma of the stomach is presented. The clinical course and the X-ray examination resembled that of a stomach cancer with hemorrhagic complications. The patient died. While alive, he refused fiber gastroscopic examination and biopsy which could have led to the correct diagnosis and to gastric resection, thus this benign tumor was not recognized before the postmortem examination.

[New possibilities in the treatment of Wilson-Konovalov disease (hepatolenticular degeneration)].

The favourable therapeutic effect of a new copper chelator--trientine (triethylenetetramine dihydrochloride) is reported. The drug was applied to 3 patients, 18 to 25 years of age, with Wilson-Konovalov disease (hepatolenticular degeneration) who in the course of chronic D-penicillamine treatment developed drug intolerance with signs of nephrotoxicity (in one patient) and myelotoxicity with leucopenia and thrombocytopenia (in two patients). The treatment was carried out orally with optimal daily dose of 1.8 gr trientine which led to a 4-6 times increase of the 24 hour urine copper excretion. The drug is well tolerated and no side effects of allergic reactions have been registered up to now in the patients treated.

[A case of rupture of the valvular cusp in a Björk-Shilley heart valve prosthesis].

A case of a patient with mitral valvular disease with a mitral valve prosthesis type Björk-Shilley of two year duration is reported. The patient was admitted to the intensive care cardiac unit in grave condition--shock and left cardiac failure. The patient died after 14 hours in spine of the efforts to over come the cardiogenic shock. The post mortem examination revealed a broken prosthesis and lack of one of the cusps which was found in the abdominal section of the aorta 10 cm above the bifurcation.

[The necessity for intragroup individualization of diet No. 5 in hepatobiliary diseases].

The frequency of cholelithiasis in Bulgaria has increased 9 times for the last 15 years. At the same time 40.5% of the patients with cholelithiasis are obese--II-IV degree of obesity. This fact calls for individualization of diet No 5 which in this country is applied to patients with liver and biliary diseases. A variant diet No 5-x for patients with cholelithiasis and abnormal body mass is proposed. This diet keeps the protein and fat components of diet No 5 but reduces the carbohydrates quantity from 346 g to 195-200 g which leads to the reduction of the daily intake of calories from 2369-2450 to 1700. The individualization of diet No 5 and the introduction of diet No 5-x in patients with biliary diseases will ensure a greater prophylactic and therapeutic efficacy of the dietetic treatment of patients with cholelithiasis and other biliary diseases combined with various degree of obesity.

[Erosive gastritis and gastroduodenitis--clinical, diagnostic and therapeutic studies].

Diagnostic, clinical and therapeutic studies were carried out, under clinical conditions, on 80 patients with endoscopically confirmed chronic erosive gastritis and gastroduodenitis. The subjects aged between 20 and 59 are most often affected, the males being 2 times more often affected. The histological study on the purposeful biopsy of the mucosa of the stomach and duodenum established, most frequently, the presence of chronic superficial gastritis and duodenitis, followed by initial chronic atrophic gastritis. The clinical symptoms were studied in details--pain (91.20%), dyspeptic syndrome (83.7%), hemorrhagic manifestations (hematemesis or melena)--23.7%. In 40% of the patients with erosive gastritis and gastroduodenitis, the disease was combined with duodenal ulcer and in 7.5%--with gastric ulcer. The etiological factors of importance are: food factors, dietetic errors, alcoholic consumption, drug effects (first of all derivatives of Rauwolfia serpentina, followed by the peroral broad spectrum antibiotics, etc), duodenogastric reflux and toxic effect of the bile acids on the antral mucosa. The endoscopic studies are of decisive importance for the diagnosis and determining the effect of the treatment. The treatment with a dietetic regimen, gastro-protective drugs (almagel, preparations of deglycyrrhized licorice--alcid V, vitamin A, spasmolytics) eliminated the erosive alterations in 75% of the patients after 3-week treatment.

[Clinico-diagnostic, therapeutic and prognostic studies of Wilson-Konovalov disease. The results of 25 years' experience].

The author's 25 years experience in clinical, diagnostic, therapeutic and prognostic study of 24 patients with Wilson-Konovalov's disease (hepatilenticular degeneration) is presented. The great diagnostic importance of copper metabolism (plasma copper level, urine copper), plasma ceruloplasmin concentration, histochemical and quantitative determination of copper in liver cells (after a liver biopsy) is pointed out. These are of special importance in young patients (under 25 years) and in children with chronic parenchymal liver disease. In 1/4 of the patients the early stage of the disease resembled chronic active hepatitis (predominantly liver type of the disease). The basic therapeutic drug is D-penicillamine hydrochloride and when it is not tolerated the drug Trien may be used. Very good therapeutic results and considerable improvement of prognosis, social and working rehabilitation of the patients have been achieved. The preconditions for good therapeutic and prognostic results are the early discovery, exact diagnosis and prolonged purposeful pathogenetic treatment (chelatory and hepatoprotective). Zinc sulfate and potassium sulfide are only secondary therapeutic means in patients with hepatolenticular degeneration.

[Action of iron, ethanol and tetrachloromethane on lysosomal membrane permeability].

The authors examined the isolated and combined influence of oral administration of iron and ethanol as well as subcutaneous administration of CCl4 on the activity of two lysosomal enzymes in serum as the experiments were carried out on 72 male white rats of the Wistar strain with initial weight of 130 to 190 gm. The two exmained enzymes were: beta-N-acethylglucosaminidase and alpha-manosidase. The increased activity of these enzymes under the action of ethanol, iron plus ethanol, CCl4 was interpreted as a manifestation of increased permeability of lysosomal cellular membrane.

[Effect of alcohol on hepatocyte mitochondria].

The authors studied ultrastructural changes of hepatocytes under the effect of alcohol. The greatest changes were revealed in the mitochondria. Physical activity and hypoprotein diet produced a significant influence on the results of alcohol poisoning. The first factor reduced, and the second enhanced the deleterious effect of alcohol.

[Immunological indices in chronic liver diseases].

The study aimed at the confrontation of the results from the investigation of the immunoglobulines G, M, and A (IgG, IgM and IgA), the test for spontaneous rosette formation and the determination of Australian antigen (HBsAg) in 110 patients with chronic liver disease (liver steatosis, chronic persisting hepatitis, chronic aggressive hepatitis and liver cirrhosis). The diagnosis was morphologically confirmed and some of the patients were repeatedly examined. No significant interrelations among the three main classes of immunoglobulines were established in the majority of the groups of chronic liver diseases, speaking for the individual information, carried by the separate immunoglobuline classes. Correlation was established between IgG and IgM in chronic active hepatitis. The mean values of IgG and IgM are considerably higher in cases with splenomegaly. IgG is more often increased when the patients are carriers of HBsAg, being in agreement with the contemporary concepts about the differences in immune pathological mechanism in chronic liver diseases, with and without HB5-antigenemia.

[Immunoglobulins in the gastric juice in some stomach and duodenal diseases].

IgA, IgM and IgC content in blood serum and gastric juice were determined in 89 patients with different gastric and duodenal diseases (57 with duodenal ulcers, 13 with gastric ulcers 8 with chronic atrophic gastritis, 9 with superficial chronic gastritis and gastroduodenitis and two with gastric cancer). Immunoglobulin was established in gastric juice (over 0,3 mg/100 ml) in 49 per cent of all the patients examined with gastric and duodenal diseases. No correlation was found between serum immunoglobulin level and their presence in the gastric juice. No correlation exists between the volume of the basal secretion and the presence or absence of immunoglobulins in the gastric juice. Immunoglobulins were established more often in the gastric juice with lower secretion of hydrochloric acid (in chronic atrophic gastritis and gastric ulcer), while in cases of higher secretion they are more frequently absent. A certain parallelism was established between the changes of hydrochloric acid output and JgA secretion in the gastric juice after stimulation of gastric secretion by Pentagastrin or histamine.
